Atherectomy offers no benefits over balloon angioplasty in tibial interventions for critical limb ischemia.

BACKGROUND: Endovascular adjuncts, like atherectomy, were developed to improve outcomes of endovascular arterial interventions. The true impact of atherectomy on endovascular outcomes remains to be determined, and little data exist on the influence of atherectomy on tibial interventions. Our study compares early and late outcomes of tibial intervention with angioplasty vs atherectomy-assisted interventions. METHODS: We completed a retrospective review of all tibial interventions between 2008 and 2010. Outcomes were analyzed using single and multivariate analysis, Cox regression, and Kaplan-Me

Successful treatment of renovascular hypertension due to fibromuscular dysplasia by intravascular ultrasound-guided atherectomy.

A 22-year-old man presented with renovascular hypertension, based on a stenosis of the distal portion of the right renal artery with a "string of beads"-like appearance. An intravascular ultrasound image at the renal artery lesion revealed irregularity of the vascular wall. Directional atherectomy was performed and histopathology of atherectomised tissues showed medial fibroplasia, a common type of fibromuscular dysplasia. After atherectomy his hypertension was markedly improved. We report here a case of renovascular hypertension due to fibromuscular dysplasia, successfully diagnosed and treat

Low prevalence of Chlamydia pneumoniae in atherectomy specimens from patients with coronary heart disease.

Coronary atherectomy specimens from 50 patients with coronary heart disease were examined for the presence of Chlamydia pneumoniae by two different methods of polymerase chain reaction (PCR) and by in situ hybridization. C. pneumoniae DNA was detected by PCR in atherosclerotic plaques of four patients (8%). Two patients' coronary atheromas were positive, both by a single-step 16S rRNA-based PCR and by an omp1-based nested PCR. The other two patients' specimens were positive only by the nested PCR. In contrast, C. pneumoniae was not detected by in situ hybridization in any of the cardiovascular

Clinical and angiographic results of transluminal extraction coronary atherectomy in saphenous vein bypass grafts.

BACKGROUND: Transluminal extraction coronary (TEC) atherectomy is a relatively new device that has recently been approved by the Food and Drug Administration. Because of its ability to aspirate clot and atheromatous material, TEC atherectomy may be useful in patients with stenoses in saphenous vein bypass grafts. METHODS AND RESULTS: TEC atherectomy was performed on 158 saphenous vein graft lesions in 146 consecutive patients with a mean age of 65 +/- 8 years (78% men). Clinical indications for atherectomy included stable angina (37%), unstable angina (54%), and postinfarction angina after re

Relation between activated smooth-muscle cells in coronary-artery lesions and restenosis after atherectomy.

BACKGROUND: Neointimal proliferation leading to restenosis frequently develops after coronary angioplasty. This process is associated with a change in vascular smooth-muscle cells from a contractile (quiescent) phenotype to a synthetic or proliferating (activated) one. We investigated whether the presence of activated smooth-muscle cells in coronary lesions at the time of coronary atherectomy predisposes patients to subsequent restenosis. METHODS: We used in situ hybridization to study the expression of messenger RNA in coronary-atherectomy specimens from 20 patients. Plaque material was hybr

Successful Cutting and Retrieval of an Unraveled Embolization Coil With Rotational Atherectomy: A Case Report.

Coil unraveling is a rare but potentially serious complication during transcatheter coil embolization. Once the braided coil has unraveled and loosened into wire filaments, percutaneous retrieval becomes extremely challenging. Although various percutaneous techniques have been described for retrieving migrated or unraveled coils, surgical removal or leaving the coil in situ may ultimately be required. We describe the first case of successful cutting and retrieval of an unraveled coil during coil embolization for a coronary pulmonary artery fistula (CPAF) using rotational atherectomy (RA). This

Retrieval of stuck burr by pullback orbital atherectomy with RESCUE technique: a case report.

BACKGROUND: Rotational atherectomy (RA) burr entrapment is an uncommon but potentially life-threatening complication. Additional percutaneous bailout is necessary when conventional percutaneous bailout strategies are unsuccessful in patients with prohibitive surgical risk. CASE SUMMARY: A 91-year-old man with a history of coronary artery bypass graft surgery presented with unstable angina caused by severely calcified diffuse stenosis of the right coronary artery. During lesion preparation, an RA burr became entrapped after crossing a heavily calcified segment. Multiple conventional percutaneo

Safety and Efficacy of a Novel Rotational Atherectomy System in Coronary Calcifications: The CORECT Trial.

BACKGROUND: Calcified coronary lesions present challenges during percutaneous coronary intervention, often impeding successful stent delivery and expansion. Rotational atherectomy (RA) facilitates plaque modification in such cases. However, clinical performance of a novel RA system remains unclear. OBJECTIVES: This noninferiority study evaluated the safety and efficacy of the novel system. METHODS: This trial included 2 prospective phases, a first-in-human (FIH) study and a pivotal randomized controlled trial. In the FIH study, 15 patients underwent percutaneous coronary intervention with th

Racial Differences in the Trends, Clinical Characteristics and Outcomes of Patients Undergoing Coronary Atherectomy.

Racial disparities persist in cardiovascular care, but little is known about differences in outcomes following coronary atherectomy by race or ethnicity. This study examines in-hospital outcomes and utilization trends of coronary atherectomy among Non-Hispanic Black (NHB) and Non-Hispanic White (NHW) patients using data from the National Inpatient Sample database (2016-2021). NHB patients were younger and exhibited a higher prevalence of comorbidities such as diabetes, hypertension, and chronic kidney disease compared with NHW patients. Despite this, multivariate analyses revealed that NHB pat

Mechanism of Stent Failure in Patients With Eruptive Calcified Nodule Treated With Rotational Atherectomy.

BACKGROUND: Percutaneous coronary intervention (PCI) for lesions with eruptive calcified nodules (CNs) is associated with worse outcomes compared with that for other calcified lesions. We aimed to clarify the relationship between eruptive CNs at index PCI, optical coherence tomography (OCT) findings at the 8-month follow-up, and clinical outcomes using serial OCT. METHODS: This retrospective observational study used data from a prospective, single-centre registry. We conducted consecutive PCI for calcified lesions requiring rotational atherectomy (RA) with OCT guidance. We categorized 51 pati

Injury of the superficial femoral artery and posterior tibial artery associated with an atherectomy device.

ObjectivesRotational atherectomy can offer a viable treatment for occlusive peripheral artery disease; maintaining the minimal invasiveness of an endovascular procedure, while allowing for a more complete lesion debridement compared with balloon angioplasty. This case report outlines a complication of guidewire entrapment associated with rotational atherectomy in the superficial femoral artery (SFA).MethodsA 57-year-old male underwent an atherectomy with Rotorex for left lower limb foot pain. During the procedure, the guidewire was suctioned into the atherectomy device, preventing any further

Comparison of Coronary Intravascular Lithotripsy and Rotational Atherectomy in the Modification of Severely Calcified Stenoses.

Debulking techniques are often necessary for successful lesion preparation in percutaneous coronary intervention. The aim of this study was to compare plaque modification of severely calcified lesions by coronary intravascular lithotripsy (IVL) with that of rotational atherectomy (RA) using optical coherence tomography (OCT). ROTA.shock was a 1:1 randomized, prospective, double-arm, multicenter noninferiority trial designed to compare final minimal stent area after IVL with RA for lesion preparation in percutaneous coronary interventional treatment of severely calcified lesions. Mid-term results of orbital atherectomy in heavily calcified lesions above and below the knee.

Background: Severe arterial calcification poses a major challenge to endovascular therapy (EVT) in peripheral artery disease (PAD). Orbital atherectomy (OAS) is used to modify plaque and enhance luminal gain, potentially improving EVT outcomes, but long-term evidence remains limited. Patients and methods: This retrospective single-center study assessed the mid-term outcomes of OAS in heavily calcified infrainguinal disease. We included symptomatic PAD patients (Rutherford 2-6) with ≥70% stenosis undergoing OAS for above- and below-the-knee (BTK) arterial lesions. Results: From 2018 to 2023, 78
