Noninvasive peripheral vascular function and atrial fibrillation in the general population.

BACKGROUND: Arterial stiffness is a strong predictor of atrial fibrillation in the community. Whether noninvasively measured conduit artery function and peripheral vascular reactivity are related to atrial fibrillation remains unknown. METHODS AND RESULTS: In 15 010 individuals of the population-based Gutenberg Health Study, mean age 55 ± 11 years, 50.5% men, we determined noninvasive vascular function by flow-mediated dilation (FMD) and peripheral arterial tonometry (PAT) in relation to manifest atrial fibrillation (N = 466). Patients with atrial fibrillation exhibited a higher mean brachial

Aggressive atrial volume reduction for bilateral giant atria improves respiratory function.

Giant bilateral atria with mitral and tricuspid regurgitation can cause postoperative respiratory dysfunction. In this article, we describe a case of giant atria with poor respiratory function that was improved by atrial volume reduction. A 79-year-old woman was referred to our institution for valve surgery. Her vital capacity was 1,080 mL. The mitral and tricuspid valves were repaired during surgery. We removed a circular section of the left atrial wall. The right atrial wall and interatrial septum were removed; this improved her vital capacity to 1,370 mL. We conclude that aggressive volume

PITX2c loss-of-function mutations responsible for congenital atrial septal defects.

Congenital heart disease (CHD) is the most common form of developmental anomaly and is the leading non-infectious cause of infant mortality. A growing body of evidence demonstrates that genetic risk factors are involved in the pathogenesis of CHD. However, CHD is a genetically heterogeneous disease and the genetic determinants for CHD in most patients remain unclear. In the present study, the entire coding region and splice junction sites of the PITX2c gene, which encodes a homeobox transcription factor crucial for normal cardiovascular genesis, was sequenced in 150 unrelated patients with var

Impact of transcatheter closure of atrial septal defects on cardiac function.

PURPOSE: The purpose of this study was to investigate the effects of transcatheter closure of atrial septal defects (ASD) on left ventricular function. METHODS: Conventional and three-dimensional echocardiographic methods (3D) were performed in 20 patients who underwent ASD closure. Diastolic and systolic volumes (LVVd and LVVs) and ejection fractions (EF) were determined by 3D methods. Doppler and tissue Doppler indices were also measured before and 1 day after the procedure. We determined the predictors of patients with a mitral early Doppler/tissue Doppler velocity ratio (E/E') ≥8 after cl

Validation of transthoracic tissue Doppler assessment of left atrial appendage function.

This study aimed to validate left atrial (LA) wall contraction velocity during atrial contraction (LAWV) in assessing LA function in 22 patients with paroxysmal atrial fibrillation. LAWV at the posterobasal LA wall was measured by transthoracic tissue Doppler echocardiography. LAWV was lower in patients with paroxysmal atrial fibrillation than in control subjects. It was more correlated with LA appendage velocity (r = 0.81) and fractional shortening (r = 0.85) than with parameters related to mitral inflow velocity or ring motion. With a LAWV cut-off value of 1.0 cm/s, receiver operator charact

Left atrial myocardial function and compliance in pre-eclampsia with preserved left ventricular systolic and diastolic function.

Left atrial (LA) adaptive changes associated with pre-eclampsia (PE) have not been adequately addressed. This study aimed to evaluate the usefulness of LA myocardial function indices in detecting early cardiac alterations in PE. Using speckle tracking echocardiography (STE), LA volumetric and myocardial parameters were acquired in PE women with preserved left ventricular (LV) systolic and diastolic function based on standard criteria and normotensive controls. LA compliance was assessed by the ratio of reservoir strain (LASr) to the estimated LA pressure (E/e'). Subtle systolic and diastolic a

Right atrial functional remodeling for early risk stratification of tricuspid regurgitation progression in atrial fibrillation.

Tricuspid regurgitation (TR) is related to a poor prognosis in atrial fibrillation (AF). Although right atrial (RA) structural remodeling contributes to TR progression, the impact of RA functional remodeling on the progression of TR remains unclear. We investigated whether RA function influences TR progression in AF. Among 664 AF patients with baseline and follow-up echocardiography, 484 (mean 68 ± 12 years, 38% female) with insignificant (mild or less) TR at baseline were included. Patients with prior cardiac surgery, cardiac implantable electronic devices, significant (at least moderate) val

Left Atrial Posterior Wall Functional Remodeling in Paroxysmal and Persistent Atrial Fibrillation. A Pilot Study.

BACKGROUND: The left atrial posterior wall (LAPW) is often targeted in atrial fibrillation (AF) ablation due to its shared properties with pulmonary veins. However, substrate-guided approaches using structural markers have shown inconsistent results. We investigated posterior wall conduction time (PWCT) as a novel and functional marker of LAPW remodeling in AF patients. METHODS: In our pilot study, 40 patients undergoing first-time AF ablation (20 paroxysmal, 20 persistent AF) were included. AF cycle length (AF CL), PW linear conduction, PW conduction area (PWCA) and PWCT were evaluated on el

Circulating transthyretin with atrial morpho-functional phenotypes and atrial fibrillation risk, and the modifying role of BMI.

BACKGROUND: Low circulating transthyretin (TTR) concentration has been suggested as a biomarker of transthyretin tetramer instability, a prerequisite for the development of transthyretin cardiac amyloidosis. This study aimed to evaluate the associations between circulating TTR levels with incident atrial fibrillation (AF) and other arrhythmias. METHODS: This study used data from the UK Biobank. Participants with available TTR data and without prior arrhythmias were included. The primary outcome was new-onset AF. The secondary outcomes were new-onset supraventricular arrhythmias (SVA), bradyar

Left Atrial Plication for Left Ventricular Posterior Wall Bending With Atrial Functional Mitral Regurgitation.

Left ventricular basal posterior wall (LVbp) inward bending in patients with rheumatic mitral regurgitation and giant left atrium (LA) is a risk for low cardiac output after mitral valve (MV) surgery, which is dramatically improved by aggressive LA plication. However, the effects of LA plication on LVbp bending in patients with atrial functional mitral regurgitation (aFMR) and giant LA have not yet been reported. In 2 patients with LVbp bending, aFMR, and giant LA, MV replacement/repair with aggressive LA plication improved LVbp bending. However, compared to patient 1, in which MV replacement

Altered physiological functions and ion currents in atrial fibroblasts from patients with chronic atrial fibrillation.

The contribution of human atrial fibroblasts to cardiac physiology and pathophysiology is poorly understood. Fibroblasts may contribute to arrhythmogenesis through fibrosis, or by directly altering electrical activity in cardiomyocytes. The objective of our study was to uncover phenotypic differences between cells from patients in sinus rhythm (SR) and chronic atrial fibrillation (AF), with special emphasis on electrophysiological properties. We isolated fibroblasts from human right atrial tissue for patch-clamp experiments, proliferation, migration, and differentiation assays, and gene expres

Effect of diltiazem and metoprolol on left atrial appendix functions in patients with nonvalvular chronic atrial fibrillation.

OBJECTIVES: Thrombo-embolic events are the important cause of mortality and morbidity in patients with chronic atrial fibrillation (CAF). The origin of thromboembolism is often the left atrial appendix (LAA). Flow rate velocity (FRV) inside the LAA is the major determinant of thrombus formation. The aim of our study was to investigate the effects of diltiazem and metoprolol used for ventricular rate control on FRV of the LAA in CAF patients and thus to evaluate the positive or negative effects of these two drugs on thromboembolic events. METHODS: Sixty-four patients were included in the study

Improvement of Renal Function in Atrial Fibrillation Patients via Catheter Ablation and Left Atrial Appendage Closure: A Case Report.

BACKGROUND Atrial fibrillation (AF) is a common arrhythmia that often coexists with heart failure and chronic kidney disease (CKD). Approximately 30% of patients with AF have CKD, and this comorbidity not only increases the complexity of treatment but also limits the choice of anticoagulant drugs. In clinical practice, the treatment strategy for these patients needs to balance cardiac and renal functions comprehensively. CASE REPORT This article reports a case of a 69-year-old woman with persistent AF and stage G3a CKD complicated by acute kidney injury (AKI on CKD). Upon admission, the patien
