The edentulous maxilla presents diverse and complex rehabilitation problems for the clinician that are often not addressed with a conventional removable appliance. Implant therapy, although revolutionizing the management of the edentulous mandible, has yielded less than favourable treatment outcomes in the maxilla. Poor bone quality and small bone volume in the maxilla, at the time of implant surgery has been associated with more implant and prosthesis failure. Bone grafting appears to improve the survival rate of implants in the atrophic maxilla and many studies have been published outlining

Insufficient crestal bone is a common feature encountered in the edentulous posterior maxilla due to atrophy of the alveolar ridge and maxillary sinus pneumatization. Numerous surgical techniques, grafting materials, and timing protocols have been proposed for implant-supported rehabilitation of posterior maxillae with limited bone height. In the majority of potential implant sites, residual bone height is less than 8 mm and the clinician has to select either a lateral or transcrestal sinus-elevation technique or placing short implants as the correct surgical option. Nevertheless, guidelines f

BACKGROUND: Calvarial bone grafts are successful in the reconstruction of the severely atrophied maxilla as a pre-implant procedure. However, not much is known about graft incorporation at the microscopic level.
PURPOSE: This study aimed to assess calvarial bone conversion 4 months after being grafted in the edentulous maxillary bone.
MATERIALS AND METHODS: In 13 patients (age:65.3 ± 8.7 years) the atrophic maxilla was reconstructed with autologous calvarial bone. Biopsies were taken from fresh calvarial bone grafts and from the reconstructed maxillae after 4 months of healing. Micro-CT, his

BACKGROUND: The reduced bone height and proximity of the maxillary sinus are the most common limitations for placement of dental implants in the posterior maxilla. Reconstruction of the atrophic posterior maxilla can be performed with a sinus augmentation procedure. The aim of this cohort study is to compare the survival rate of implants placed in augmented sinus to implants placed in native bone in the posterior maxilla.
METHODS: This study was designed as a prospective cohort study and included consecutively treated patients. Patients who required the sinus augmentation (test group) were tr

BACKGROUND: Dental implants are used for replacing missing teeth. Placing dental implants is limited by the presence of adequate bone volume permitting their anchorage. Several bone augmentation procedures have been developed to solve this problem. Zygomatic implants are long screw-shaped implants developed as a partial or complete alternative to bone augmentation procedures for the severely atrophic maxilla. One to three zygomatic implants can be inserted through the posterior alveolar crest passing through the maxillary sinus, or externally to it, to engage the body of the zygomatic bone. A

INTRODUCTION: Edentulism causes progressive bone resorption of the maxillae, which can lead to altered maxillo-mandibular relationships. The aim of the study was to evaluate the applicability of guided bone regeneration (GBR) to Le Fort I osteotomies with interpositional bone grafts for treatment of patients with severe maxillary atrophy.
MATERIALS AND METHODS: Twenty consecutive patients characterized by severely atrophic maxillae were treated from January 2003 to January 2006 in order to resolve maxillary edentulism. All patients underwent pre-prosthetic surgery, including a Le Fort I osteo

Implant placement in the posterior maxilla is often limited by insufficient bone height. This report demonstrates a minimally invasive, guided crestal sinus augmentation using the Hiossen OneCAS hydraulic system. A 74-year-old woman presented with a nonrestorable maxillary molar. Extraction, ridge preservation, and guided crestal sinus augmentation were performed using the Hiossen OneCAS system. Implants were simultaneously placed at the molar and premolar sites with successful osseointegration and prosthetic rehabilitation. The guided approach minimized complications and achieved favorable st

Five Macaca irus monkeys were used in this study to determine cephalometric and histologic changes after total maxillary impactions. In the three experimental animals, the maxilla was impacted 5 to 7 mm. and lateral cephalograms were taken over a 150-day period. By superimposing on craniofacial implants, osseous and dental changes were measured. Excellent skeletal stability was demonstrated during the postoperative period. Histologically, there was normal osseous healing of the osteotomized maxillas. Accompanying the counterclockwise rotation (overclosure) of the mandible, there was extensive

BACKGROUND: Atrophic anterior maxilla rehabilitation can be a challenging procedure due to multiple factors that influence clinical decision making. After a prolonged loss of teeth, the residual bone often impedes the use of standard implant placement protocols and additional procedures are needed.
AIM: The aim of this study is to describe the multidisciplinary approach for the diagnosis and treatment of a 50-year-old woman with prolonged use of a removable maxillary partial denture.
CASE DESCRIPTION: This article presents a full-mouth-phased rehabilitation of an atrophic anterior maxilla wi

PURPOSE: Implant survival rates using a bone-added osteotome sinus floor elevation (BAOSFE) procedure with simultaneous placement of a non-submerged sand blasted with large grit and acid etched (SLA) implant are well documented at sites where native bone height is less than 5 mm. This study evaluated the clinical results of non-submerged SLA Straumann implants placed at the time of the BAOSFE procedure at sites where native bone height was less than 4 mm. Changes in graft height after the BAOSFE procedure were also assessed using radiographs for 5 years after the implant procedure.

STATEMENT OF PROBLEM: Because of the wide anatomic variations among individuals, the differences between procedures for placing zygomatic implants (ZIs) and between zygomatic and regular implants, the biomechanics of ZI in the restoration of an atrophic maxilla are not yet well known.
PURPOSE: The purpose of this finite element analysis (FEA) study was to critically review and clarify the biomechanical principles of the ZIs based on clinical reality.
MATERIAL AND METHODS: Two FEA models were created representing the most frequent anatomic situations for the posterior atrophic maxilla: zygoma
