A proposed protocol for monitoring ototoxicity in patients who take cochleo- or vestibulotoxic drugs.

No widely accepted protocol or guideline exists for monitoring ototoxicity in patients who take powerful and potentially cochleotoxic and/or vestibulotoxic agents. Many physicians in other specialties who prescribe these drugs do not understand the important role of otolaryngologists and audiologists in pretreatment counseling and evaluation and the need for follow-up assessments of their patients' auditory function. Based on our combined experience of more than 50 years, we have developed a uniform yet flexible approach to monitoring cochlear and vestibular function in these patients. We disc

Facial nerve stimulation after successful multichannel cochlear implantation.

Postoperative rehabilitation after cochlear implantation may be difficult and frustrating for both patient and audiologist. The occurrence of facial nerve stimulation with associated twitching and discomfort can interfere with the rehabilitative process and be a cause of great concern to the patient. Such stimulation can occur at the time of initial "tune-up" or can be delayed by months or years. The incidence of postoperative facial nerve stimulation 109 consecutive patients operated by the same surgeon over a period of 7 years is reviewed. Possible causes for immediate and delayed stimulatio

Syllabic stress (trochaic vs. spondaic) and speech thresholds in normal and hearing-impaired children.

Speech thresholds for 6 common two-syllable words on tape with either trochaic (natural) or spondaic stress were collected from children aged 3.5-6.5 yrs, audiometrically grouped as normal-hearing (N:8), conductive (N:8) or sensorineural (N:6). Ss were further grouped as younger and older. No significant differences in mean threshold, nor in time to reach threshold, were observed for either pattern of stress or age, suggesting that audiologists may interchange spondaic with trochaic stress when measuring speech thresholds in young children, with no effect on the mean value or the efficiency of

Immittance screening for aural problems in school children.

The predominant aural health problem in school-age children is middle ear inflammation with effusion. The conventional hearing screening method has ineffectively detected half of those school children with middle ear problems. Acoustic immittance measurements, recently added to the audiologist's battery of tests, effectively and economically screens middle ear effusion. Unfortunately, acoustic immittance does not screen hearing sensitivity as does the conventional pure-tone sweep check. This article describes the principles of acoustic immittance and compares it with pure-tone audiometry; disc

Audibility-Based Informational Counseling in Pediatric Audiology Using the Speech Intelligibility Index: A Qualitative Study.

OBJECTIVES: To fully engage caregivers of children who are deaf or hard of hearing, audiologists need counseling tools that adequately convey the impact of permanent childhood hearing loss (HL) on development and the benefits of interventions like hearing technology. The present study examines the parent experience of receiving informational counseling during the diagnostic and intervention process with early amplification. We used qualitative methods to gather feedback on caregiver experiences with informational counseling. Based on themes identified from caregiver experiences, we sought to i

Assessing Language Skills as a Predictor of Children's Listening Difficulties: Validation and Reference Data on a New Auditory Language Task.

OBJECTIVES: Some children experience greater listening difficulties in noisy and reverberant environments than their peers, despite having normal peripheral hearing. In these cases, other potential causes have to be explored. One is a language disorder. Testing this possibility in audiology requires a language screening test that can be used by audiologists. This study aims to establish reference data for a screening test, the auditory cloze test (AudiCloze), to examine sex differences and to validate the results by comparing them with the standardized Clinical Evaluation of Language Fundament

DABS-MS: deep atlas-based segmentation using the Mumford-Shah functional.

PURPOSE: Cochlear implants (CIs) are neural prosthetics used to treat patients with severe-to-profound hearing loss. Patient-specific modeling of CI stimulation of the auditory nerve fiber (ANF) can help audiologists improve the CI programming. These models require localization of the ANFs relative to the surrounding anatomy and the CI. Localization is challenging because the ANFs are so small that they are not directly visible in clinical imaging. We hypothesize that the position of the ANFs can be accurately inferred from the location of the internal auditory canal (IAC), which has high cont

Complexities of Hemifacial Microsomia: A Case Study of Mandibular Hypoplasia and Ear Deformity.

Hemifacial microsomia (HFM) presents a complex congenital anomaly characterized by the asymmetric underdevelopment of facial structures, predominantly affecting the ear, mouth, and mandible on one side of the face. This case study examines the intricacies of HFM through the presentation of a 23-year-old female with congenital deformities of the left ear and mandibular hypoplasia. Clinical and radiological evaluations revealed significant facial malformations, including mandibular hypoplasia, left temporomandibular joint fusion, and maxillary abnormalities, confirming the diagnosis of HFM. The hidden risks of hearing tests and programmable ventriculoperitoneal shunt valves.

Programmable variable pressure valves were introduced in the 1980s, providing a non-invasive solution to post-operative alterations of the valve opening pressure to address problems of under or overdrainage. Since their increased use in the treatment of hydrocephalus, there have been case reports of unintentional alterations of the valve opening pressure following exposure to magnetic fields in everyday environments, from televisions to rollercoasters. Here we describe two cases of patients' programmable valves being altered following audiology assessments. We subsequently discuss some of the

Tinnitus Update.

This article provides an update on tinnitus for audiologists and other clinicians who provide tinnitus-specific services. Tinnitus can be attributable to hearing loss, somatosensory system dysfunction, or auditory cortex dysfunction, with hearing loss being the most common cause and serious underlying pathologies being rare. Hearing loss does not always lead to tinnitus, and patients with tinnitus do not always suffer from hearing loss. The first scenario is explained by a so-called inhibitory gating mechanism, whereas the second assumes that all tinnitus sufferers have some degree of hearing

Organic solvents and hearing loss: The challenge for audiology.

Organic solvents have been reported to adversely affect human health, including hearing health. Animal models have demonstrated that solvents may induce auditory damage, especially to the outer hair cells. Research on workers exposed to solvents has suggested that these chemicals may also induce auditory damage through effects on the central auditory pathways. Studies conducted with both animals and humans demonstrate that the hearing frequencies affected by solvent exposure are different to those affected by noise, and that solvents may interact synergistically with noise. The present article

The Impact of Pharmaceutical Side Effects on Audiological and Vestibular Measurements.

There are over 2,000 drugs with a combined total of over 400 side effects that could result in obtaining inaccurate case history information or inaccurate test results that may lead to misdiagnosing the patient's hearing or vestibular problem. The recommendations that are made could be inappropriate and thus can lead to management errors. A review of the auditory, vestibular, and cognitive side effects of many of the drugs patients take regularly (including drugs that can cause tinnitus) is provided. This article offers suggestions to obtain a more accurate case history. A review of preferred

Craniofacial malformations and their syndromes. An overview for the speech and hearing practitioner.

Congenital malformations of the craniofacial region represent an important class of human developmental disorders. Abnormalities of speech and hearing frequently occur in this vast array of conditions. Knowledge of the diagnostic criteria, genetics, and natural history of these conditions is important for audiologists and speech-language pathologists because of their close involvement with the children and families in their care-providing role. Awareness of the important distinction between individuals with isolated defects vs. individuals who have their facial defect as part of a syndrome is

Validity of the Multiple Auditory Processing Assessment-2: A Test of Auditory Processing Disorder.

Purpose A normative study using the Multiple Auditory Processing Assessment-2 (MAPA-2; Schow et al., 2018) was recently completed. With access to these data, the authors extend that work and support a definite construct for auditory processing disorder (APD). The goal here is to examine MAPA-2 reliability and validity (construct, content, and concurrent). Evidence for the APD construct is further buttressed by measures of sensitivity and specificity. Results of MAPA-2 testing on children diagnosed with learning disability (LD), attention-deficit/hyperactivity disorder (ADHD), and specific lang
