[Herpes zoster and post-herpetic neuralgia].

Pain associated with herpes zoster arise from the virul neuritis of the suffered trigeminal or spinal dorsal ganglion. Prolonged neuritis makes an irreversible nerve injury and continuous pain impulse develops a central sensitization. A post-herpetic neuralgia is thought to be a neuropathic pain due to the irreversible nerve injury and sensitization. It is important to treat herpetic pain completely before the development of the post-herpetic neuralgia, because there are few effective therapies to cure post-herpetic neuralgia. A sympathetic nerve block increases the nerve blood flow supply, an

A comparative study between etidocaine and bupivacaine in ulnar nerve block.

The long-acting local anaesthetics bupivacaine and etidocaine were compared in a double-blind study using ulnar nerve block. Eleven volunteers took part in the study. Solutions used were: bupivacaine 0.25% and 0.5% and etidocaine 0.5% and 1 %. All solutions contained adrenaline 5 mug/ml. The time of onset and duration of analgesia and motor block, degree of motor block, and changes in skin temperature were investigated. The results did not show any differences in the duration of sensory of motor block. Differences were seen in the degree of motor block in favour of etidocaine. The sympathetic

Vagal and sympathetic nerve activities influenced by posterior cerebral circulation in rabbits.

Vagal and sympathetic nervous activities in the rabbit were recorded while vertebral blood flow was partially blocked by the injection of adenosine 5'-diphosphate (ADP; platelet aggregator). When a small dose of ADP (0.2 mg/kg) was administered into a unilateral vertebral artery, sympathetic nerve (SN) activity increased, and its magnitude was inversely correlated with the extent of the decrease in blood pressure (BP). A larger dose (2 mg/kg) of ADP suppressed SN activity on the injected side, whereas the change was small on the non-injected side. Vagal nerve (VN) activity showed a monophasic

Comparison of sympathetic block and hemodynamic effects of erector spinae plane block and thoracic epidural analgesia in breast surgery: a randomised controlled trial.

BACKGROUND: In this randomized controlled, double-blind study, we aimed to investigate the involvement of sympathetic nerve fibers in erector spinae plane block (ESPB) and to compare it with thoracic epidural analgesia (TEA) in patients scheduled for unilateral mastectomy. Additionally, we compared sympathetic blockade-related hemodynamic effects, sensory blockade distribution, and analgesic effects. METHODS: Thirty-eight female patients aged 18-70 years, American Society of Anesthesiologists (ASA) classification I-III, were included in the study. The patients were divided into Group E (those

Capsaicin increases modulation of sympathetic nerve activity in rats: measurement using power spectral analysis of heart rate fluctuations.

We assessed the sympatho-vagal activities of the heart after administration of capsaicin by measuring the power spectral analysis in rats. There were major two frequency components of heart rate variability, which we defined as high (1.0 Hz <, HF) and low (LF, < 1.0 Hz) frequency components. Vagal blockade by atropine abolished the high frequency component, and lowered the amplitude of the low frequency component. On the other hand, under conditions of sympathetic blockade by propranolol, the low frequency component was reduced. Combined vagal and sympathetic blockade abolished all heart rate

Autonomic regulation of heart rate and blood pressure in hemorrhaged toads.

Mean arterial blood pressure (P(a)) and heart rate (HR) were studied in normal conscious cane toads (Bufo marinus) or in autonomically blocked animals treated variously with bretylium, atropine, phentolamine, and propranolol. Partial or total autonomic blockade had no effect on resting values of the variables tested, with the exception that atropine treatment of bretylium-treated toads caused an increase of heart rate by 5.3 +/- 1.3 beats/min. In graded hemorrhage, aliquots of blood (1.68 ml/100 g body wt) were removed at 60-min intervals. There was extensive plasma recruitment, and blood coul

Prolonged Treatment of Inappropriate Sinus Tachycardia with Continuous Stellate Ganglion Blockade: A Case Report.

Inappropriate sinus tachycardia (IST) presents challenges in diagnosis and treatment due to its unclear etiology and limited therapeutic options. This case report explores the use of continuous stellate ganglion block (CSGB) as a potential treatment avenue. A 23-year-old woman with refractory IST underwent several CSGB placements, resulting in prolonged symptom relief and decreased median heart rate. Despite the eventual recurrence of symptoms, the sustained effects of CSGB suggest its efficacy in managing IST. This report underscores the potential of CSGB as a promising therapeutic approach f

Stellate ganglion block in disparate treatment-resistant mental health disorders: A case series.

OBJECTIVES: A significant subset of patients with mental health disorders (MHDs) fail to respond to standard management and are termed as treatment-resistant. This cohort has limited options for managing their condition. Autonomic dysfunction has been reported in the neurobiology of MHDs including anxiety, depression, obsessive-compulsive disorder (OCD), panic disorder, and bipolar disorder (BD). Stellate ganglion block (SGB) is an emerging treatment that dampens sympathetic activity and has been shown to be of benefit in the management of post-traumatic stress disorder. METHODS: Patients wit

Effectiveness, safety, and predictors of response to ultrasound-guided stellate ganglion blockades for the treatment of patients with chronic migraine: A retrospective and observational study.

BACKGROUND: Whether ultrasound-guided stellate ganglion block (SGB) is a valuable therapeutic option for the treatment of patients with chronic migraine (CM) is worth exploring. If SGB is proven to be effective for CM, the identification of potential predictors for the effectiveness of SGB warrants further investigation. This study aimed to investigate the effectiveness and safety of SGB in patients with CM and to explore the predictive factors for its treatment effectiveness. METHODS: This is a retrospective observational study. We retrospectively analyzed the effects of SGB for the treatmen

Regulation of acute reflectory hyperinflammation in viral and other diseases by means of stellate ganglion block. A conceptual view with a focus on Covid-19.

Whereas the autonomic nervous system (ANS) and the immune system used to be assigned separate functions, it has now become clear that the ANS and the immune system (and thereby inflammatory cascades) work closely together. During an acute immune response (e. g., in viral infection like Covid-19) the ANS and the immune system establish a fast interaction resulting in "physiological" inflammation. Based on our knowledge of the modulation of inflammation by the ANS we propose that a reflectory malfunction of the ANS with hyperactivity of the sympathetic nervous system (SNS) may be involved in the

[A case of intractable chronic abdominal pain caused by hepatomegaly associated with primary amyloidosis, successfully treated with celiac plexus block].

A 51-year-old female previously diagnosed as primary amyloidosis suffered from recurrent abdominal pain. The result of thorough examination indicated that the main cause of the pain was severe hepatomegaly. Continuous venous administration of narcotics and other alternative therapies did not provide symptomatic relief, and thus the patient was treated with celiac plexus block, which resulted in effective pain control and improved ADL level. Though the procedure of celiac plexus block is simple and celiac plexus block is applicable without causing severe complication, it is not widely used. Fro

Retroperitoneal abscess after neurolytic celiac plexus block from the anterior approach.

BACKGROUND AND OBJECTIVES: The anterior approach for celiac plexus block has the potential risks of infection, hemorrhage, and fistula formation. We report a case of a patient who developed a retroperitoneal abscess with the formation of a vascular-enteric fistula after a neurolytic celiac plexus block from the anterior approach. CASE REPORT: A 60-year-old female with a history of pain secondary to chronic idiopathic calcifying pancreatitis (VAS 7-8) underwent a subtotal resection of the head of the pancreas with an end-to-side pancreatojejunostomy using a Roux-en-Y loop. Pain continued secon

[Chronic pain syndrome of the penis following cerebrovascular insult].

Complex regional pain syndromes (formerly sympathetically maintained pain syndromes or reflex sympathetic dystrophy) encompass symptoms of pain, dysfunction and sympathetic disorder. They occur spontaneously or after peripheral or internal lesions (e.g. stroke or myocardial infarction) and predominantly affect the limbs, rarely the face or the trunk. This case report describes a 64-year old man who after a stroke suffered from heavy burning pain in the penis and perineum, which did not ameliorate after established conservative therapy. Sympathetic blockade, however, provided pain relief. The d

Physiologic effects of stellate ganglion block: a result of complete ganglion blockade or the vertical spread of local anesthetic?

Traditionally, stellate ganglion blockade has been used for the diagnosis and treatment of upper extremity sympathetic pain. However, this treatment has not been shown to provide adequate sympathetic blockade of the upper extremities. This study demonstrates that a carefully performed upper thoracic sympathetic block with imaging guidance can result in a successful sympathetic blockade of the upper extremities. This study furthermore demonstrates that the occurrence of a Horners syndrome is not a testimony to a successful sympathetic block of the upper extremities.
