Paranoia, experiential avoidance, and narcissism as predictors of outcome in residential treatment of borderline personality disorder.

INTRODUCTION: Previous research has suggested that residential treatment may be effective in reducing symptoms in patients with symptoms of borderline personality disorder (BPD). This study examined whether paranoia, experiential avoidance, and narcissistic personality features predicted response to residential treatment in a cohort of female patients with symptoms of borderline personality disorder. METHOD: Participants were 87 women with borderline personality disorder symptoms who participated in multimodal residential treatment incorporating elements of Dialectical Behavioral Therapy, Men

Facial rejuvenation: combining cosmeceuticals with cosmetic procedures.

Cosmetic patients are looking for a more youthful appearance without spending a lot of money, feeling any pain, or experiencing any postprocedure downtime. New cosmeceutical therapies can be used adjuvant to chemical peels, lasers, and injectables, making antiaging regimens less painful and requiring less postprocedural healing time. Adjunctive agents can be used to enhance chemical peels and decrease postinflammatory hyperpigmentation (PIH). Topical retinoids used prior to ablative laser treatments can aid in faster postprocedure healing and reepithelialization. Cosmeceuticals that contain bo

Antimony toxicity from the use of tartar emetic for the treatment of alcohol abuse.

Antimony is a poisonous element with toxic properties that mimic those of arsenic. Numerous reports describe gastrointestinal complications of vomiting, diarrhea and stomatitis associated with antimony exposure. However, antimony toxicity from the use of tartar emetic as a treatment for alcohol abuse has never been described previously. A 19-y-o man with a history of alcohol abuse ingested a 10 mL bottle of "Soluto Vital" (tartar emetic, 50 mg/mL), produced in Guatemala for treatment of alcohol abuse. He presented 60 min after ingestion with severe vomiting, abdominal cramps, diarrhea, weaknes

Smokeless Tobacco Pouch Keratosis: A Report of Two Cases.

Tobacco pouch keratosis (TPK) is a white lesion caused by chronic contact with smokeless tobacco (SLT) on the oral mucosa. The lesion develops as a result of chronic tobacco chewing or snuff dipping. The lesion exhibits precancerous characteristics; thus, dentists must educate patients about the dangers of tobacco use and advise them to quit. This article discusses the clinical findings of a 17-year-old adolescent boy and a 48-year-old man who were SLE users diagnosed with TPK and nicotine replacement therapy to averse from the SLT chewing habit due to nicotine dependence. The novelty of this

Supervised Disulfiram's Superior Effectiveness in Alcoholism Treatment: Ethical, Methodological, and Psychological Aspects.

Disulfiram (DSF) causes the ALDH-mediated deterrence of alcohol consumption. We review recent meta-analyses showing the superior effectiveness of supervised disulfiram (SD) in alcoholism treatment compared with oral naltrexone or acamprosate (ACP). The success of SD is also consistent with the almost complete absence of alcoholism in Japanese homozygotes for 'inefficient' ALDH. However, SD is an underused treatment and some clinicians have ethical objections to DSF. We examine these objections and argue that they are based on a misunderstanding of how DSF works. In particular, we argue that SD

Voyeurism: a review of literature.

The most striking thing about the literature on voyeurism is the relative lack of material in print. In the present work, the author was able to find only 15 articles and no books that deal specifically with voyeurism. Of the 57 references compiled for this review, not one involves an experimental manipulation of variables. An attempt is made to review as many aspects of voyeurism as possible, giving illustrative case material whenever available. The areas of troilism and coprophilia are covered in some detail to establish to what degree they belong to the deviation of voyeurism. Theory of per

Update on left atrial appendage closure for neurologists.

A significant proportion of patients with atrial fibrillation (AF) who need thromboembolic protection are not treated with or discontinue oral anticoagulation after its initiation. Undertreatment in clinical practice has not improved sufficiently despite the availability of direct oral anticoagulants, which are associated with less intracranial bleeding than vitamin K antagonists. Multiple reasons account for this phenomenon, including bleeding events or ischemic strokes while on anticoagulation, poor treatment adherence despite best educational attempts, or aversion to drug therapy. Percutane

Resetting the hippocampal buffer: a neurocognitive account of psychedelic therapy for anxiety-related psychopathology.

Psychedelics (hallucinogenic 5-HT2A agonists such as psilocybin) are being investigated for their potential to treat a range of conditions, including anxiety-related psychopathology. Despite promising results, the mechanisms by which psychedelic therapy may alleviate anxiety are not well understood. Here, we review neural and cognitive mechanisms underlying anxiety-related psychopathology and how psychedelics may impact these mechanisms. This review culminates in a tentative neurocognitive account of how psychedelics may promote long-term anxiolysis. We propose a model of anxiety related psych

Evaluating Alternative Designs of a Multilevel HIV Intervention in Maharashtra, India: The Impact of Stakeholder Constraints.

Background. Multilevel interventions combine individual component interventions, and their design can be informed by decision analysis. Our objective was to identify the optimal combination of interventions for alcohol-using HIV+ individuals on antiretroviral drug therapy in Maharashtra, India, explicitly considering stakeholder constraints. Methods. Using an HIV simulation, we evaluated the expected net monetary benefit (ENMB), the probability of lying on the efficiency frontier (PEF), and annual program costs of 5,836 unique combinations of 15 single-focused HIV risk-reduction interventions.

ACG Clinical Guideline: Diagnosis and Management of Eosinophilic Esophagitis.

Eosinophilic esophagitis (EoE) is a chronic immune-mediated disease of the esophagus. It is diagnosed in the setting of symptoms of esophageal dysfunction and an eosinophilic predominant infiltrate in the esophagus. The condition is rapidly increasing in incidence and prevalence and is commonly encountered in gastroenterology and allergy practices, emergency departments, and primary care settings. Over the past decade, there have been paradigm shifts in disease diagnosis and management, increases in knowledge about EoE risk factors, natural history, and pathogenesis, and development of validat

The authors provide an overview of the papers in the special issue of Memory on mental imagery and memory in psychopathology. The papers address emotional, intrusive mental imagery across a range of psychological disorders including post-traumatic stress disorder (PTSD), agoraphobia, body dysmorphic disorder, mood disorders, and psychosis. They include work on information processing issues including modelling cravings, conditioning, and aversions, as well as imagery qualities such as vividness and emotionality. The overview aims to place the articles in a broader context and draw out some exci

Lumbar spinal stenosis: can positional therapy alleviate pain?

METHODS: We analyzed a retrospective case series of 52 patients with spinal stenosis confirmed by spinal imaging and walking limitations treated with a wheeled walker set to induce lumbosacral flexion. RESULTS: Of the 52 patients, improvement in ambulation was classified as excellent for 30 (58%), good for 7 (13%), moderate for 8 (16%), and poor for 7 (13%). Among 48 patients with neurogenic pain, pain relief was classified as excellent for 22 (46%), good for 11 (23%), moderate for 7 (14.5%), and poor for 8 (16.5%). CONCLUSIONS: These retrospective data from a case series support the hypothe

"Down Syndrome is Not a Curse": parent Perspectives on the Medicalization of Down Syndrome.

BACKGROUND: Potential clinical interventions to mitigate or eliminate symptoms of Down syndrome (DS) continue to be an active area of pre-clinical and clinical research. However, views of members of the DS community have yet to be fully explored. METHODS: We conducted a survey with parents/caregivers of people with DS (n = 532) to explore interest in potential therapeutic approaches during fetal development or childhood that may improve neurocognition and modulate the DS phenotype. We qualitatively analyzed open-ended responses. RESULTS: Some respondents rejected the development of therapies

Cyanamide-induced liver injury. A predictable lesion.

Cyanamide, a drug frequently used for aversion therapy in chronic alcoholism, induces a liver-cell lesion consisting of cytoplasmic inclusion bodies, structurally similar to those of Lafora. In order to know how frequently the lesion is associated with the intake of this drug, a prospective study was carried out. A biopsy was performed in ten chronic alcoholic patients who had been treated with Cyanamide. Inclusion bodies were present in all patients (7 cases) whose biopsy was taken when they were on Cyanamide and in two other patients whose biopsies were taken 3 and 7 years after discontinuat

Experiences with Medications for Addiction Treatment Among Emergency Department Patients with Opioid Use Disorder.

INTRODUCTION: Medications for addiction treatment (MAT) are the evidence-based standard of care for treatment of opioid use disorder (OUD), but stigma continues to surround their use. We conducted an exploratory study to characterize perceptions of different types of MAT among people who use drugs. METHODS: We conducted this qualitative study in adults with a history of non-medical opioid use who presented to an emergency department for complications of OUD. A semi-structured interview that explored knowledge, perceptions, and attitudes toward MAT was administered, and applied thematic analys

Rationale and Efficacy of Sound Therapies for Tinnitus and Hyperacusis.

Sound therapies are a common component of treatments for tinnitus and hyperacusis. The original idea was to partially or completely mask tinnitus with broadband noise delivered by sound generators or hearing aids, for a few hours each day. Over several months, many patients reported that their tinnitus became quieter or easier to bear, and that loud sounds became less aversive. However, it wasn't always clear that these benefits could be attributed to sound therapy rather than to other aspects of treatment, such as counseling or hearing aid use, and not all patients reported benefits. During t

[The avoidant/restrictive food intake disorder: a new feeding and eating disorder in the diagnostic systems].

The Avoidant-restrictive Food Intake Disorder (ARFID) was included in the fifth edition of the DSM in 2013 and in the BNO-11 diagnostic manual in 2019. Prior to this, the problem was often unrecognised or diagnosed as an eating disorder not otherwise specified or feeding and eating disorders of infancy or early childhood. The new diagnosis allows for better targeting of patients who have not been diagnosed earlier, as well as for the diagnosis of patients over the age of six. The authors, in their review, describe the diagnostic features and symptom profile of ARFID, and they discuss the role

Emerging approaches to managing alcohol dependence.

PURPOSE: To review pharmacotherapies and psychosocial interventions for the treatment of alcohol dependence, including recent developments that may minimize nonadherence. SUMMARY: Alcohol dependence is a widespread, chronic disorder with enormous health consequences. Psychological and behavioral therapies have been the mainstay of treatment and are demonstrated to be effective, but they do not lead to reduced drinking or abstinence in all patients. Advances in neurobiology have led to the identification of drug targets and the development of novel drugs to treat alcohol dependence, and many p

Treatment hurts: Lay theories of graded exposure in the treatment of four anxiety disorders.

OBJECTIVE: This paper concerned the perceived suffering/side effects caused by various well-known treatments for personal problems. It looked at whether people understood whether potentially painful treatments that confront negative aversive affect were effective or not. METHOD: In total, 106 participants completed a long questionnaire assessing the 'psychological pain' ratings of 30 psychotherapy treatments, varying in fear exposure, for four relatively common anxiety disorders: social phobia, agoraphobia, post-traumatic stress disorder, and obsessive compulsive disorder. RESULTS: Factor an
