Dorsal lumbar interbody fusion for chronic axial, mechanical low back pain: a modification of two established techniques.

Lumbar spinal fusion is a popular method for treating degenerative disc disease leading to chronic low back pain. Some of the techniques most commonly undertaken are posterior lumbar interbody fusion and transforaminal lumbar interbody fusion. These techniques have their own advantages and disadvantages. In this article, we briefly describe a modified technique, dorsal lumbar interbody fusion, which retains the advantages and avoids the disadvantages of the others. The goal of this procedure is to combine the aggressive decompression of both the thecal sac and bilateral nerve roots and symmetr

Significance of occipitoaxial angle in subaxial lesion after occipitocervical fusion.

STUDY DESIGN: The significance of occipitoaxial angle in the development of subaxial subluxation after occipitocervical fusion was determined in a minimum 5-year follow-up study performed retrospectively. OBJECTIVE: To clarify the association between the position of the fixed occipital bone and axis and the development of subaxial subluxation. SUMMARY OF BACKGROUND DATA: There have been few reports describing the association between the position of fixation of the occipital bone and axis and subaxial lesion in occipitocervical fusion. MATERIALS AND METHODS: Thirty-eight patients with rheuma

Morphometric Analysis of Axis and Its Clinical Significance -An Anatomical Study of Indian Human Axis Vertebrae.

BACKGROUND: The atlas and axis vertebra have unique shape and complex relationship with vertebral artery. Fracture of dens of axis accounts for 7-27% of all cervical spine fractures, but surgeries in these regions are highly risky because of the reported incidences of vertebral artery injury. AIM AND OBJECTIVES: The study was designed to measure morphometric data of human axis vertebra, of Indian origin. The different anatomical parameters on dry specimen of human axis vertebrae were established and the results were compared with other studies. MATERIALS AND METHODS: Thirty intact human axis

Relationship Between the Axis of the Humeral Heads and the Spinal Sagittal Alignment in Asymptomatic Volunteers.

Background Currently, there is no known correlation between relevant parameters of respective non-adjacent spinal segments, which suggests the possibility of a third external factor. To our knowledge, no study has examined the humeral head position in consideration of spinal sagittal alignment. This study aims to establish new parameters related to the humeral head axis and evaluate the relationship between the axis of the humeral head and spinal sagittal alignment in asymptomatic volunteers. Methods Standing posteroanterior and lateral radiographs of the entire spine were obtained from 62 asym

Expression and purification of intact and functional soybean (Glycine max) seed ferritin complex in Escherichia coli.

BACKGROUND: Injuries of the upper cervical spine are a major cause of morbidity and mortality due to associated spinal cord and head injuries. The injury patterns of the upper cervical spine are numerous, and the neurologic sequelae are diverse. The axis (C2) is the most commonly fractured vertebra in the upper cervical spine; its unique anatomy and architecture pose difficulties in the diagnosis and the management of its fractures. METHODS: All cases of acute spinal injuries at Prince Mohammed Bin Abdulaziz Hospital in Riyadh, Saudi Arabia, were screened for fractures of C2 vertebrae. These

[Static-dynamic computerized tomography in the diagnosis of traumatic lesions of alar ligaments. Preliminary results].

The patients affected with cervical injuries often complain of cervical pain, headache and dizziness even when no bone fractures are detected. Such patients are likely to have a post-traumatic injury of the cervical ligaments. Twenty-five symptomatic patients (19 women and 6 men) were examined with upper spine CT and functional CT scans (right and left rotation) to detect ligament injuries and hypermotility of the craniocervical junction, both related to traumatic events. Eleven patients showed no alterations, while unilateral densitometric alterations of the alar ligaments were observed in 14

[Severe trauma of neck and skull base ].

The concept of internal decapitation comprises lesions within the skull base, upper spinal column with partial or complete destruction of the cervical part of the medulla. By means of casualties due to internal decapitation we demonstrate that, besides the known causes, a rotational damage at the juncture of the upper spinal column to the skull base as well as fractures by axial stretching may cause such traumas. A decrease of such accident's consequences is hardly possible because of the amount of energy involved causing the damage. Due to a steady increase of traffic, particularly by motor-b

Relationship between facet tropism and facet joint degeneration in the sub-axial cervical spine.

BACKGROUND: Facet tropism is the angular asymmetry between the left and right facet joint orientation. Although debatable, facet tropism was suggested to be associated with disc degeneration, facet degeneration and degenerative spondylolisthesis in the lumbar spine. The purpose of this study was to explore the relationship between facet tropism and facet degeneration in the sub-axial cervical spine. METHODS: A total of 200 patients with cervical spondylosis were retrospectively analyzed. Facet degeneration was categorized into 4 grade: grade I, normal; grade II, degenerative changes including

Medially-shifted rather than high-riding vertebral arteries preclude safe pedicle screw insertion.

We enrolled 100 patients who underwent preoperative CT angiography before cervical spine instrumentation and investigated the morphology of the C2 pedicle from the perspective of pedicle screw (PS) trajectory using volume rendering and multiplanar reconstruction. The narrowest portion of the pedicle was identified as the pedicle isthmus. Safe C2 PS insertion was regarded to be not feasible when the height of the medullary cavity of the pedicle isthmus and/or width of the medullary cavity of the pedicle isthmus was ⩽4mm. Forty-five (22.5%) pedicles were ⩽4mm in width, and safe insertion of a PS

Combined ossification of the posterior longitudinal ligament at C2-3 and invagination of the posterior axis resulting in myelopathy.

PURPOSE: Spinal stenosis at the C2-3 segment is a rare occurrence, and when it occurs myelopathy infrequently results. Furthermore, only a handful of cases involving congenital abnormalities of the posterior arch of the axis have been described resulting in cervical myelopathy many of which described simultaneous congenital abnormalities at adjacent levels and none of which identified ossification of the posterior longitudinal ligament (OPLL) at the same level. We report a case of a previously undescribed combination of abnormalities at the C2-3 segment resulting in clinical myelopathy. METHO

Axial loading injuries to the middle cervical spine segment. An analysis and classification of twenty-five cases.

Injuries to the cervical spine at the C3-C4 level involving the bony elements, intervertebral disks, and ligamentous structures are rare. We present 25 cases of traumatic C3-C4 injuries sustained by young athletes and documented by the National Football Head and Neck Injury Registry. Review of the cases reveals that the response of energy inputs at the C3-C4 level differ from those involving the upper (C1-C2) and lower (C4-C5-C6-C7) cervical segments. Specifically, the C3-C4 lesions appear to be unique with regard to the infrequency of bony fracture, difficulty in effecting and maintaining red

Avascular necrosis of the proximal end of the dens. A complication of halo-pelvic distraction.

Roentgenographic evidence of avascular necrosis of the upper part of the dens was observed in thirteen of ninety-four patients treated for tuberculous kyphosis or non-tuberculous scoliosis using halo-pelvic distraction. Four patients had some residual loss of motion. A significant relationship was demonstrated between avascular necrosis and the amount of distraction visible on roentgenograms.

CT evaluation of the pattern of odontoid fractures in the elderly--relationship to upper cervical spine osteoarthritis.

Odontoid fractures are common in the elderly following minor falls. Almost all of them have osteoarthritis of the cervical spine below the axis vertebra. As a result, there is increased stress on the spared upper cervical spine, resulting in a higher incidence of injuries. As movement in the upper cervical spine involves participation of five joints, degeneration in any one particular joint may affect the biomechanics of loading of the upper cervical spine. We aimed to analyse the relationship of odontoid fractures to the pattern of upper cervical spine osteoarthritis in the elderly. We studie
