Sutureless enterostomy for extremely low birth weight infants.

OBJECTIVE: In recent years, improved survival rates of extremely low birth weight infants (ELBWIs) have led to an increasing number of enterostomy performed for those with meconium obstruction of prematurity (MOP)1,2, spontaneous intestinal perforation (SIP)3,4. To prevent serious stoma-related complications such as stoma side perforation, prolapse, fall and surgical site infection, we introduce our new "sutureless enterostomy" technique. METHODS: We present the procedures in detail. We reviewed the medical records of twelve patients who underwent "sutureless enterostomy" in our neonatal inte

Oxygen Targeting in Extremely Low Birth Weight Infants.

The use of supplemental oxygen plays a vital role in the care of the critically ill preterm infant, but the unrestricted use of oxygen can lead to unintended harms, such as chronic lung disease and retinopathy of prematurity. An overly restricted use of supplemental oxygen may have adverse effects as well. Ideally, continuous monitoring of tissue and cellular oxygen delivery would allow clinicians to better titrate the use of supplemental oxygen, but such monitoring is not currently feasible in the clinical setting. The introduction of pulse oximetry has greatly aided the clinician by providin

Low birth weight associates with hippocampal gene expression.

Birth weight predicts the lifetime risk for psychopathology suggesting that the quality of fetal development influences the predisposition for mental disorders. The connectivity and synaptic network of the hippocampus are implicated in depression, schizophrenia and anxiety. We thus examined the underlying molecular adaptations in the hippocampus as a function of the fetal conditions associated with low birth weight. We used tissues from the non-human primate, Macaca fascicularis, to identify changes in hippocampal gene expression early in postnatal development associated with naturally occurri

Guidelines for feeding very low birth weight infants.

Despite the fact that feeding a very low birth weight (VLBW) neonate is a fundamental and inevitable part of its management, this is a field which is beset with controversies. Optimal nutrition improves growth and neurological outcomes, and reduces the incidence of sepsis and possibly even retinopathy of prematurity. There is a great deal of heterogeneity of practice among neonatologists and pediatricians regarding feeding VLBW infants. A working group on feeding guidelines for VLBW infants was constituted in McMaster University, Canada. The group listed a number of important questions that ha

Peer Victimization in Extremely Low Birth Weight Survivors.

BACKGROUND: Extremely low birth weight (ELBW; <1000 g) children may be at risk for experiencing peer victimization. We examined retrospectively reported peer victimization in ELBW and control children in the oldest known, prospectively followed, population-based birth cohort of ELBW survivors. METHOD: We compared levels of verbal and physical peer victimization in ELBW and control children. We also predicted peer victimization in the ELBW sample from child characteristics. RESULTS: ELBW children, especially girls, were at an increased risk for verbal, but not physical victimization. In addit

Lung Function in Very Low Birth Weight Adults.

BACKGROUND AND OBJECTIVES: Lung function attained in young adulthood is 1 of the strongest predictors of obstructive airways disease in later life. Adults born preterm at very low birth weight (VLBW; <1500 g) who have experienced bronchopulmonary dysplasia (BPD) have reduced lung function. We studied the association of lung function in young adulthood with preterm birth at VLBW and with BPD and other prenatal and neonatal conditions. METHODS: We performed spirometry for 160 VLBW subjects (29 with BPD according to Northway criteria) aged 18 to 27 years and 162 term control subjects group-match

HIV transmission to extremely low birth weight infants.

BACKGROUND: Prematurity increases the perinatal HIV transmission rate compared with term infants. There is sparse literature documenting the risk of transmission of HIV to extremely low birth weight (ELBW) infants. OBJECTIVE: To determine the risk of perinatal transmission of HIV to ELBW infants in a tertiary neonatal unit in South Africa. METHODS: A prospective database was maintained on all inborn ELBW infants over a 1-year period from March 2010 to February 2011. Survival and DNA HIV polymerase chain reaction results at 6 weeks were recorded. RESULTS: Of the 180 ELBW infants, 51 (28%) of

Neural tube defects and associated low birth weight.

OBJECTIVE: To estimate the association between neural tube defects (NTDs) and low birth weight. STUDY DESIGN: This was a retrospective cohort study of women undergoing ultrasound from 17 to 22 weeks from 1990 to 2008. Presence or absence of fetal NTD defined the two study groups. The primary outcomes were intrauterine growth restriction (IUGR), birth weight <10th percentile, and severe IUGR <5th percentile. Subgroup analysis was performed to observe if the association with IUGR persisted. RESULTS: Of 66,956 women, 170 were found to have fetal NTD. Only the rate of advanced maternal age diffe

Renal calcification in very low birth weight infants.

BACKGROUND: Renal calcification in preterm infants has been described frequently. The etiologic factors have not yet been fully clarified. The objective of this study was to evaluate the incidence of and risk factors for renal calcification in our population. METHODS: We retrospectively reviewed the charts of very low birth weight preterm infants during a 1-year period. Renal ultrasound scans were performed at term or before discharge and at a corrected age of 1 year. RESULTS: Six infants (6%) had renal calcification at term or before discharge compared with 96 who did not. Factors significa

[Type of afterbirth and twin birth weight discordance].

UNLABELLED: Twin pregnancies are regarded as high risk pregnancies. It results from the possibility of occurrence of numerous complications both like the ones in singleton pregnancies and those characteristic for multiple pregnancy only. The complication occurring exclusively in multiple pregnancies is discordant intrauterine growth of the fetuses. THE AIM: a response to the question if there a relationship between the type of afterbirth and twin birth weight discordance. MATERIAL AND METHODS: Material of the research consisted of 186 afterbirths derived from successful twin pregnancies. In

[Association between periodontitis and preterm low birth weight].

There were increasing studies on the association of periodontitis with preterm low birth weight (PLBW) in the recent 10 years. PLBW is associated with about half of all perinatal mortality. Known factors which could induce PLBW include smoking, genetics, alcohol, prenatal care, nutrition, urinary tract infections, However, there are still about 25% of PLBW cases occur with etiology unknown. Many evidences supported the association between periodontitis and PLBW. Periodontitis is a risk factor of PLBW, but the mechanism is unclear. Current theory suggests that periodontal inflammation adversely

Factors affecting birth weight in sheep: maternal environment.

Knowledge of factors affecting variation in birth weight is especially important given the relationship of birth weight to neonatal and adult health. The present study utilises two large contemporary datasets in sheep of differing breeds to explore factors that influence weight at term. For dataset one (Study 1; n=154 Blue-faced Leicester x Swaledale (Mule) and 87 Welsh Mountain ewes, 315 separate cases of birth weight), lamb birth weight as the outcome measure was related to maternal characteristics and individual energy intake of the ewe during specified periods of gestation, i.e. early (1-3
