Visuospatial hemi-inattention following cerebellar/brain stem bleeding.

Neglect is a unilateral lack of responsiveness to stimuli caused by visuospatial hemi-inattention, a unilateral representation deficit and/or a unilateral hypokinesia. It results most frequently from right-hemisphere brain damage, particularly of the parietal lobe but also of the frontal cortex, the basal ganglia, the thalamus, and recently it has also been described after a cerebellar lesion. We report a patient with right-sided bleeding of the posterior inferior cerebellar artery, who developed a left-sided visual hemi-inattention. She had no visual field defects, yet she had problems detect

Gamma knife radiosurgery of the brain stem cavernomas.

Over 6 years (1992-1998) 26 patients with brain stem cavernomas were treated using the Leksell gamma knife in Prague. 25 patients had a follow up of 6-66, median 24 months. Annual risk of bleeding before radiosurgery was 4%. After gamma knife treatment sudden impairment of neurodeficit reported as rebleeding was observed in 4 patients at 6-51 months, median 16.5 months, after radiosurgery. This represented a 6.8% risk of rebleeding after radiosurgery, which is not significantly different from the risk before radiosurgery. MRI or CT was performed in 24 patients 6-48, median 24, months after rad

Auditory brain stem response in small acoustic neuromas.

OBJECTIVE: The auditory brain stem response (ABR) has been criticized recently as an insensitive measure for the detection of small acoustic neuroma (AN). This study was undertaken to evaluate our experience with the efficacy of ABR in detection of small tumors. STUDY DESIGN: Retrospective case review. Twenty-five patients with surgically proven small ANs measuring 1 cm or less were reviewed. In addition, 568 patients who underwent screening ABR were reviewed to evaluate the rate of false positive results at our institution. RESULTS: ABR was abnormal in 92% of patients with small AN in this

Moyamoya disease associated with a brain stem glioma.

An 8-year-old boy was found to have primary moyamoya disease associated with a brain stem glioma. For over 3 years the child had experienced transient ischemic attacks induced by hyperventilation. One month before referral to our hospital he had presented with progressive left facial nerve palsy. Magnetic resonance imaging showed a cystic mass in the lower pons. Angiography revealed severe bilateral stenosis of the internal carotid arteries and prominent moyamoya vessels in the basal ganglia. Partial resection of the tumor yielded a histological diagnosis of pilocytic astrocytoma. Local radiat

Brain stem arteriovenous malformation with large venous varix.

The case of a patient with arteriovenous malformation (AVM) with a venous varix that occurred at the medulla oblongata is reported. The AVM bled twice in a short period. The AVM was fed by the small ramus, possibly the anterior spinal artery, originating from the right distal vertebral artery and draining into a large venous varix and then into the venous systems surrounding the brain stem. The obstruction of the feeding artery was performed, and the varix subsequently disappeared. The remaining abnormal vasculature was then treated with stereotactic radiosurgery. The treatment of an AVM at th

[Clinical and image diagnostics of brain stem infarction].

Clinical features and image diagnostic characteristics of brain stem infarction were studied in 28 patients to facilitate early diagnosis. Crossed hemiparesis was found in all the patients with medulla oblongata and midbrain infarction. Pontine infarction had a variety of clinical syndromes as follows: (1) crossed-hemiparesis in 8 cases (44.4%) and ipsilateral hemiataxia in one case. (2) tetraplegia in 3 cases (16.7%). (3) pure hemiparesthesia in 4 cases. (4) hemiparesis and hemiparesthesia without cranial nerve palsy in 3 cases (16.7%). MRI was found to be a very useful diagnostic measure for

Tuberculomas presenting as isolated intrinsic brain stem masses.

We document the clinical presentation, management and outcome of histologically verified, solitary, intrinsic brain stem tuberculomas in six patients. The neurological symptoms and signs in these patients did not distinguish their lesions from other intrinsic masses of the brain stem. On contrast-enhanced computed tomography (CT), the tuberculomas appeared as ring enhancing lesions in three patients and nodular contrast enhancing lesions in the other three. Diagnosis was established on the basis of CT-guided stereotactic biopsy in all patients. All patients were treated with antituberculous dr

Epidermoid cyst of the brain stem. Case report.

Brain stem epidermoid cysts are extremely rare lesions. Only nine cases have been reported. Management of the epidermoid cyst is decompression of cyst contents and removal of cyst capsule. But in some cases resection of the cyst may result in a poor outcome because of cyst wall adhesion into the brain stem.

[Brain stem dysfunction in Arnold-Chiari II syndrome].

Among 76 patients suffering from myelomeningocele treated during 1978 to 1987 there were 12 children with brain stem signs as a sequel to Arnold-Chiari II syndrome. In 2 of these patients only stridor was seen, in 4 stridor with attacks of apnoea, in 2 attacks of apnoea with dysphagia, and in 4 children stridor, attacks of apnoea and dysphagia. Hence, it will be necessary to modify the classification given by Charney et al (4) in respect of brain stem patterns of signs according to three grades, since the signs of stridor, apnoea and dysphagia can be combined with each other in different ways.

Auditory brain-stem potentials with unilateral pontine hemorrhage.

Although there have been extensive anatomical and physiological studies in animals, the actual neural sources, or even the laterality, of some components of auditory brain-stem evoked potentials in humans are uncertain. We studied these responses in a 56-year-old patient who had a clearly demarcated pontine hemorrhage on the right side. The patient was somnolent, with dense left hemiplegia and signs of involvement of right cranial nerves V, VI, and VII. Stimulation of the left ear (ie, contralateral to the lesion) evoked a normal series of waves with clearly resolved positive components peakin

Brain stem auditory evoked responses in chronic alcoholics.

Brain stem auditory evoked responses (BAERs) were performed on 25 alcoholic patients with Wernicke-Korsakoff syndrome, 56 alcoholic patients without Wernicke-Korsakoff syndrome, 24 of whom had cerebellar ataxia, and 37 control subjects. Abnormal BAERs were found in 48% of patients with Wernicke-Korsakoff syndrome, in 25% of alcoholic patients without Wernicke-Korsakoff syndrome but with cerebellar ataxia, and in 13% of alcoholic patients without Wernicke-Korsakoff syndrome or ataxia. The mean value of the I-V interval was prolonged in all patient groups. There were more patients with prolonged

Auditory brain stem responses in patients with vertigo.

Sixty-two patients with vertigo were examined with ABR. Eight of the 30 patients with vestibular neuronitis had abnormal ABR indicating brain stem dysfunction. Most of the patients with abnormal ABR showed continued ABR abnormality after recovery from the disease. Only one patient with epidemic vertigo had abnormal ABR, the remaining 15 patients had no signs of brain stem involvement. All 16 patients with benign positional vertigo had normal ABR.

Central auditory battery testing in brain stem disease.

The CAT scan and the BERA have revolutionized the practice of otology. There is a tendency to accept a negative scan report as indicating a lack of pathology. The authors have found cases where central battery testing has indicated progressing brain stem disease in the presence of a normal CAT scan. In one case, the abnormal test preceded a hemiplegia by two weeks. This raises the possibility of delaying or aborting an infarct if the correct diagnosis is made early.

Brain stem and cerebellar dysfunction with Legionnaires' disease.

A 37-year-old man under treatment for manic-depressive illness developed pneumonia identified as Legionnaires' disease accompanied by a severe neurological disorder with profound dysarthria, ataxia, gaze paralysis, and downbeat nystagmus. At review six months later, he has made only a partial recovery with persisting limb and gait ataxia. Difficulties in diagnosing neurological complications of Legionnaires' disease in a patient with a psychiatric disorder requiring psychotropic medication are discussed.

[Brain stem potentials evoked by "click" (author's transl)].

The authors describe the results of click at a recurrence frequency of 10 C/S in evoked responses of the brain stem, firstly in twenty normal individuals, with headphones or in a free area, making it possible to define a range of physiological responses to the different intensities, then in seventeen pathological individuals consisting of six cases of conduction deafness and eleven of perceptive deafness. On the basis of this study, they attempt to demonstrate the value of this type of examination not only in the determination of objective auditory threshold, but also in that of the type of de

[Audiovestibular exploration of the brain stem (author's transl)].

In the context of a postgraduate lecture for specialist physicians, the author describes techniques of objective audiology used on a daily basis in the exploration of the brain stem in the ENT department of Lariboisiere Hospital. The following are reviewed for each of them: --the corresponding anatomical pathways, --the principles and techniques used. The results are illustrated by examples. Increase in the threshold of stapes acoustic reflex above 95 dB on a frequency of 1,000 Hz is highly suggestive of acoustic nerve involvement or the lower part of the brain stem (bar sign). By contrast, th

Clinical applications of the auditory brain stem response.

The auditory brain stem response is a powerful new tool for the detection and quantification of hearing impairment, especially in the pediatric population. It gives exact information about the functional status of the cochlea and brain stem pathways. The technique distinguishes recruiting from nonrecruiting losses, predominantly high frequency from flat losses, and retrocochlear from peripheral disorders. The recent introduction of bone conducted stimuli should soon permit the unambiguous separation of conductive and sensorineural losses.

Möbius syndrome due to brain stem tegmental necrosis.

Möbius syndrome undoubtedly results from a variety of disorders affecting central or peripheral portions of appropriate cranial nerves or their target muscles. Pathological alterations observed in cranial nuclei are most often viewed as aplastic or dysplastic lesions. Two patients with Möbius syndrome with associated facial and skeletal malformations showed mineralized necrotic foci in multiple brain stem nuclei. Prenatal encephalomalacic lesions represent the pathological basis for some cases of congenital static Möbius syndrome.

Radiation of infratentorial and supratentorial brain-stem tumors.

A retrospective survey of 41 patients with unbiopsied thalamic, pontine, and medullary tumors treated with radiation therapy demonstrated 30% to 40% 3-year survival, with seven long-term survivors. Eleven of the 12 patients surviving over 3 years have essentially normal neurological function. Of seven patients in whom histological diagnoses were eventually obtained, four proved to have glioblastomas and three low-grade astrocytomas. Autopsy revealed that all glioblastomatous tumors extended beyond the treatment field; this suggested that larger volumes, perhaps the whole brain, should be irrad
