Impact of fatty acids on human UDP-glucuronosyltransferase 1A1 activity and its expression in neonatal hyperbilirubinemia.

While breast milk has been known as a cause of neonatal hyperbilirubinemia, the underlying mechanism of breast milk-induced jaundice has not been clarified. Here, the impact of fatty acids on human UDP-glucuronosyltransferase (UGT) 1A1--the sole enzyme that can metabolize bilirubin--were examined. Oleic acid, linoleic acid, and docosahexaenoic acid (DHA) strongly inhibited UGT1A1 activity. Forty-eight hours after a treatment with a lower concentration of DHA (10 mg/kg), total bilirubin significantly increased in neonatal hUGT1 mice, which are human neonatal jaundice models. In contrast, treatm

Human milk exosomal-miR-144-5p alleviates neonatal necrotizing enterocolitis by regulating the TLR4/NF-κB pathway.

BACKGROUND: Human breast milk-derived exosomes have been shown to prevent necrotizing enterocolitis (NEC). However, the mechanism remains unclear. This study aims to examine the role of miR-144-5p in repairing the damage of the tight junction barrier caused by NEC and its underlying mechanism. METHODS: Differentially expressed exosome-derived miRNAs from term and preterm breast milk were identified through miRNA sequencing. Subsequently, the biological role and mechanism of the miRNA were studied both in vitro and in vivo. RESULTS: We found that exosomal miR-144-5p enhances the viability of

Microbiota-targeted maternal antibodies protect neonates from enteric infection.

Although maternal antibodies protect newborn babies from infection1,2, little is known about how protective antibodies are induced without prior pathogen exposure. Here we show that neonatal mice that lack the capacity to produce IgG are protected from infection with the enteric pathogen enterotoxigenic Escherichia coli by maternal natural IgG antibodies against the maternal microbiota when antibodies are delivered either across the placenta or through breast milk. By challenging pups that were fostered by either maternal antibody-sufficient or antibody-deficient dams, we found that IgG derive

Continuous intragastric milk feeds in infants of low birth weight.

In a feeding trial 66 infants of low birth weight received continuous intragastric milk feeds from the fourth hour of life, starting with 60 ml/kg/24 hr and reaching a maximum of 300 ml/kg/24 hr on the ninth day. Each infant received only full-strength milk, which was either expressed human breast milk or SMA-S26 (a proprietary low-protein adapted cows' milk) or half-cream Regal milk (partly-skimmed evaporated cows' milk). For various reasons 10 babies had to be withdrawn, and the final assessment was made on the 56 who completed the trial successfully.Persistent vomiting was a problem in only

Maternal Gestational Hypertension Is Associated with Reduced TSG-6 in Human Milk: Potential Implications for Neonatal Immunological Support.

Tumor necrosis factor alpha-stimulated protein 6 (TSG-6) is a potent anti-inflammatory mediator secreted by mesenchymal stem cells (MSCs). In human milk (HM), these cells (human milk mesenchymal stem cells, HM-MSCs) contribute to postnatal programming of the neonatal gastrointestinal immune axis. This study assessed how maternal gestational hypertension and maternal age affect the cytoprotective TSG-6 pool and its associated immunomodulatory microRNA signature in early milk. Early colostrum, 2-4 days postpartum, from healthy mothers and hypertensive patients was analyzed. TSG-6 gene expression

Adjustable fortification of human milk fed to preterm infants: does it make a difference?

BACKGROUND: Inadequate nutrition leading to growth failure is common among premature infants. Although fortified breast milk (breast milk plus commercially prepared fortifier) is the preferred feeding, nutrient intakes achieved with fortified breast milk fall short of meeting nutrient needs. This is mainly due to inadequate protein content of fortifiers and variability in composition of expressed breast milk. OBJECTIVE: A new adjustable fortification regimen has been designed to ensure that protein needs of premature infants are met at all times. The new regimen encompasses increasing the amo

Lactation anaphylaxis: report of a rare case with recurrent postpartum anaphylaxis.

Lactation anaphylaxis is extremely rare and has been scarcely reported in the literature. Breast feeding and/or milk expression immediately induces life-threatening anaphylactic reactions, including generalized urticaria, angioedema, respiratory symptoms, and hypotension. Six English-language case reports have described the clinical course in detail. The present report describes a case involving a 24-year-old woman with no history of allergic reactions or anaphylaxis who experienced anaphylactic reactions three times immediately after milk expression. Lactation anaphylaxis was suspected when a

Human breast milk-derived exosomes protect against intestinal ischemia and reperfusion injury in neonatal rats.

BACKGROUND: Intestinal ischemia and reperfusion (IR) injury like that seen in midgut volvulus can be life-threatening in the pediatric population. Human breast milk-derived exosomes (HMDEs) can prevent intestinal inflammation in experimental necrotizing enterocolitis and other intestinal diseases. The aim of this study is to investigate the effects of HMDEs on intestinal damage related to IR injury. METHODS: Exosomes were isolated from human breast milk by ultracentrifugation then confirmed by Nanoparticle tracking analysis and detection of exosome membrane markers. 2-weeks old Sprague Dawley

Activin, inhibin and the human breast.

Activins and inhibins are growth factors involved in cell differentiation and proliferation. Human breast tissues such as normal mammary tissue, fibroadenoma, and breast cancer express inhibin and activin mRNA and proteins. Activin A and its binding protein, follistatin, are also present in human milk during the first week of lactation. Using immunohistochemistry, we have observed that the inhibin/activin alpha, betaA, and betaB subunits are present in normal breast tissue regardless of menstrual cycle phase or menopause, as well as in fibrocystic disease, and breast tumors. The mRNAs encoding

Local complement genes expression in the mammary gland: effect of gestation and inflammation.

Complement components in breast milk may enhance the local immune response in the gut of infants. In this study, we investigated the expression of complement genes in the mammary gland and attempted to determine possible regulatory mechanisms. We have studied the expression of C3, C4, factor B, and HLA-DRalpha mRNA by in situ hybridization in gestational mammary gland specimens and compared these findings to those in breast tissue affected with an inflammatory process, lactating adenoma or idiopathic gynecomastia. In normal resting breast, only C4 mRNA was noted in some ductal epithelium. In g

Isolation of a lactoferrin cDNA clone and its expression in human breast cancer.

A cDNA library constructed from mRNA from a human breast carcinoma metastasis was screened with a polyclonal antibody to deglycosylated human milk fat globule membrane, resulting in the isolation of eight clones from a total of 10(5) plaques. One of these (J16) was identified as lactoferrin. It was highly expressed (as a 2.5 Kb mRNA) in lactating breast and in both normal resting tissue taken from adjacent to carcinoma or from reduction mammoplasties. Immunoreactive lactoferrin was localised to ductal cells and their secretions in both normal and mildly hyperplastic ducts. In a normal tissue s

Case Report of Dysphoric Milk Ejection Reflex.

Dysphoric milk ejection reflex (D-MER) is a dysregulation of emotions (dysphoria) that can occur during the milk ejection reflex and during breastfeeding or expressing breast milk. Symptoms of D-MER present suddenly and can include hopelessness, sadness, nervousness, irritability, nausea, dread, palpitations, and a hollow feeling in the stomach. Although D-MER was first reported in 2007, it remains understudied to date and should not be confused with postnatal depression or anxiety disorders. Knowing that they are experiencing symptoms of a named condition is very supportive for women who expe

Effect of fortifying breast milk on gastric emptying.

A study was performed to determine if the addition of a fortifier to expressed breast milk (EBM) affected gastric emptying in low birthweight infants. Using ultrasonography, the gastric emptying of EBM alone was compared with that containing a fortifier, in a blind, crossover study. Twenty two low birthweight infants were studied: median (range) gestation 31.5 weeks (28-37); birthweight 1495 g (1000-2480 g). The gastric antral cross-sectional area (ACSA) was measured by ultrasonography before each feed and then sequentially after its completion until the ACSA returned to its pre-feed value. Th

The Benefits and Biological Risks in Breastfeeding and Sharing Human Milk: Have We Got it Right?

BACKGROUND: Implementation of the strong recommendation by the World Health Organization (WHO), the United Nations Children's Fund (UNICEF), and others, to increase use of donor human milk in cases where breastfeeding cannot be provided, requires a flexible, knowledge-based and risk-informed approach. METHODS: Based on an extensive literature review, we present the evidence for potential biological risks of breastmilk in three scenarios: fresh, unmodified milk from breast to baby (breastfeeding and wet-nursing); expressed and unmodified milk stored at < 4 ºC until just before consumption; and

Sustaining Breastmilk Expression and Direct Feeding at Breast for Very Low Birth Infants: A Qualitative Exploration of Parental Perspectives.

In Ireland, while most women express milk for their preterm very low birthweight (VLBW) infants, only a few are discharged home exclusively receiving mothers' own milk, and even fewer are engaged in direct feeding at breast (DFAB). This is in the context of Ireland having one of the lowest breastfeeding initiation and sustenance rates in the world. This qualitative descriptive study aimed to explore parents' experiences and perspectives on sustaining breastmilk expression and DFAB for VLBW infants. Semi-structured interviews were conducted with nine mothers and three partners, and the data wer

Biomarker-based text messages to promote lactation success in mothers of critically Ill infants: a randomized controlled pilot study.

OBJECTIVE: Infrequent breast pumping limits mother's own milk production in mothers of infants admitted to the neonatal intensive care unit. We aimed to determine the feasibility and benefit of biomarker-based personalized text messages on pumping frequency and milk sodium levels. A secondary aim examined lactation outcomes. STUDY DESIGN: In this randomized controlled pilot study, 51 mothers were randomized to receive personalized text messages regarding pumping frequency or standard care. RESULTS: There were no differences in pumped milk volume or sodium level, however, there was a trend to

90K (Mac-2 BP) in human milk.

Human 90K, also known as Mac-2 binding protein (Mac-2 BP), is a secreted glycoprotein which is widely expressed, binds to the human macrophage-associated lectin Mac-2, and may have a role in host defence. We have measured the concentrations of 90K in human breast milk from eight healthy mothers delivering mature healthy infants after full-term pregnancy using a specific immunoenzymatic assay. Maximal 90K concentrations were observed on days 2-3 post-partum, and ranged from 13.4 to 79.2 microg/ml. The concentrations of 90K in the milk had no correlation with those in the maternal blood.

Maternal Alkaline-Ionized Mineral Complex Supplementation Regulates Intestinal Epithelial Development and Promotes Glucose and Lipid Metabolism Through the Wnt/Notch Pathway in Piglets.

Breast milk is the sole nutritional source for infants during lactation, but the immaturity of infants' intestinal function elevates the risk of infant malnutrition. As a nutritional additive, maternal Alkaline Ionized Mineral Complex (AIMC) has great application potential in livestock production. This study investigated maternal AIMC supplementation on offspring glucose and lipid absorption using a piglet model. Thirty sows at 107 days of gestation were randomly divided into the Con group (basal diet) or the AIMC group (basal diet + 30 g/head/day AIMC) and fed until piglets were weaned (total
