Hypermagnesemia (6.95 mmol/l) and respiratory arrest occurred to a 20-year-old female (G3P2002) at 26 weeks of gestation during tocolytic treatment with MgSO4.7H2O (density greater than plasmalyte) injected into an i.v. infusion bag containing 1 l of plasmalyte without mixing. The patient was rescued with calcium gluconate and normal pregnancy continued. It is important to adequately mix an i.v. solution after adding a drug particularly when the drug-containing solution has greater density than the parent i.v. solution.

Calcium is a dynamic mineral. Recent discoveries designate that low intake of calcium generates deficiencies and path to other diseases. Hypocalcemia persists as a problem after thyroidectomy. We reviewed our experience with 245 thyroidectomies to define the spectrum of hypocalcemia, elucidate the mechanisms of hypocalcemia, and formulate a rational basis for its management. Using calcium salts in management of amlodipine overdose is challenging. A 25-year-old male with known history of adult polycystic kidney disease presented with hypotension, tachycardia, and intact neurological status after ingestion of 450 mg of amlodipine. Immediately, normal saline infusion and norepinephrine were initiated. Two grams of calcium gluconate was injected, followed by intravenous infusion of 1.16 mg/kg/h.
