Psychopathology and aggressive behaviour in cancer pain perception.

In order to investigate the relationships between psychopathology and aggressive behaviour on the one hand and cancer pain perception on the other, 11 patients who suffered from neoplasm were investigated by means of Symptomatology Check List (SCL-90) by Derogatis, Inventory for assessing different kinds of hostility (QTA) by Buss-Durkee, Scala di Autovalutazione per la Depressione (SAD) by Cassano & Castrogiovanni and Mc-Gill Pain Questionnaire (MPQ.) by Melzack. The results show that when psychopathologic symptoms are present, they lead the patient to feel pain as unbearable, thus arousing v

Pain catastrophizing levels differentiate between common diseases with pain: HIV, fibromyalgia, complex regional pain syndrome, and breast cancer survivors.

OBJECTIVES: Pain catastrophizing is a core psychological factor determining pain experience. We addressed the question of whether patients with different pain syndromes group into different pain catastrophizing phenotypes. METHODS: A total of 727 patients with chronic pain associated with four primary syndromes: Breast cancer (BC) survivors (n = 400), fibromyalgia (FM, n = 52), complex regional pain syndrome (CRPS, n = 155), and HIV (n = 120) were first studied for differences in levels of pain catastrophizing (Pain Catastrophizing Scale, PCS) and pain intensity by analysis of variance. [The role and status of standardized cancer pain management in comprehensive cancer care].

Standardized cancer pain management is an essential component of comprehensive cancer care. Pain is one of the most common and distressing symptoms in palliative care for patients with advanced cancer. Persistent pain severely impairs daily function and quality of life. The implementation of standardized cancer pain management, including standardized assessment, analgesic treatment, adverse effect management, follow-up, and end-of-life care, has significantly improved pain control and quality of life, with the goal of achieving optimal pain relief and preserving dignity throughout the disease

The correlation between pain and function in cancer survivors.

BACKGROUND: Function is critically important to cancer survivors, who are at risk of losing independence and mobility. But relatively, little is known about how pain, a widely prevalent symptom, contributes to functional decline. Understanding this may lead to earlier intervention and improved clinical outcomes. METHODS: Four hundred eighty-eight participants across five cancer centers with rehabilitation clinics completed the PROMIS Cancer Function Brief 3D profile assessment and the numeric pain rating scale (NRS) during ambulatory clinic visits. Key clinical and demographic factors, includ

Advances in Neuroimaging of Breast Cancer Pain: An Overview.

Breast cancer is currently the most common malignant tumor, primarily affecting women, and it frequently leads to chronic pain that significantly impairs physical and mental health. Neuroimaging studies have demonstrated that breast cancer-related pain is associated with specific brain alterations, including changes in activation, connectivity, and structure of pain-processing regions. This review synthesizes findings on functional and structural brain changes related to chronic pain in breast cancer and compares them with non-cancer chronic pain patterns. By integrating recent evidence, it pr

The Use of Parenteral Opioids in Cancer Pain Management.

Opioids should be offered to patients with moderate-to-severe pain related to cancer or active cancer treatment unless contraindicated. Although oral administration of opioids is generally preferable, a parenteral route may be advisable and mandatory in some clinical circumstances. Parenteral administration of opioids may accelerate the achievement of analgesia. The intravenous route fits the need of rapid achievement of analgesia in patients poorly responsive to other opioids and provides a fast analgesia in patients with breakthrough pain, that has a specific temporal pattern requiring a rap

A narrative review of buprenorphine in adult cancer pain.

INTRODUCTION: Adult cancer pain is a disease state battled on a global scale. Proper pain management is essential to prevent health complications and promote patient well-being. Due to the opioid misuse crisis in the United States, providers are looking for alternatives to traditional opioids used for adult cancer pain. Buprenorphine has a unique pharmacologic profile, allowing it to be delivered in noninvasive ways; thus, it offers an alternative to traditional options. Randomized controlled trials have shown improved pain scores with transdermal buprenorphine, and they showed reductions in p

Opioids for cancer pain - an overview of Cochrane reviews.

BACKGROUND: Pain is a common symptom with cancer, and 30% to 50% of all people with cancer will experience moderate to severe pain that can have a major negative impact on their quality of life. Opioid (morphine-like) drugs are commonly used to treat moderate or severe cancer pain, and are recommended for this purpose in the World Health Organization (WHO) pain treatment ladder. The most commonly-used opioid drugs are buprenorphine, codeine, fentanyl, hydrocodone, hydromorphone, methadone, morphine, oxycodone, tramadol, and tapentadol. OBJECTIVES: To provide an overview of the analgesic effic

Corticosteroids for the management of cancer-related pain in adults.

BackgroundOne of the most feared symptoms associated with cancer is pain. Opioids remain the mainstay of pain treatment but corticosteroids are often used concurrently as co- or adjuvant analgesics. Due to their anti-inflammatory mechanism of action, corticosteroids are said to provide effective analgesia for pain associated with inflammation and in the management of cancer-related complications such as brain metastasis and spinal cord compression. However, corticosteroids have a wide range of adverse effects that are dose and time dependent.ObjectivesTo evaluate the efficacy of corticosteroid

A systematic review of oxycodone in the management of cancer pain.

BACKGROUND: Oxycodone is often used as an opioid analgesic for moderate to severe cancer-related pain, but its use varies across Europe. This systematic literature review forms the basis of guidelines for oxycodone use within the European Palliative Care Research Collaborative opioid guidelines project conducted on behalf of the European Association for Palliative Care. OBJECTIVES: The objective of this study was to identify and assess the quality of evidence for the use of oxycodone for cancer pain in adults. METHODS: The Cochrane Database of Systematic Reviews, Cochrane Central Register of

Pain in patients with lung cancer: pathophysiology and treatment.

This review analyses the characteristics of the principal pain syndromes associated with lung cancer, their physiopathology and causes, and provides updated information on available treatments. Pain associated with lung cancer is characterized by multiple expressions, due to either the progression of disease and/or induced by oncological treatment. The analgesic treatment is principally based on the use of opioids. Other than the oral route, which is the preferred one, alternative modalities to administer opioids may be helpful in different clinical circumstances. According to the opioid respo

Morphine analgesia in cancer pain: role of the glucuronides.

Preclinical data and limited studies in humans have suggested that morphine-6-glucuronide (M6G) has analgesic activity and morphine-3-glucuronide (M3G), contributes adversely to the therapeutic effect of morphine. This open point-prevalence study in 103 patients on oral morphine for cancer-related pain investigated the correlations between morphine doses, metabolites, and the degree of pain relief or toxicity. Morphine, M6G, and M3G were assayed by high-performance liquid chromatography on a single blood sample taken between two and four hours after dose. Pain, analgesia, and toxicity were rec

Bone cancer pain and the role of RANKL/OPG.

Cancer-induced bone diseases are common and can have a devastating impact at the end of life. One of the most difficult sequelae of cancer is metastases to the skeleton, an event that results in bone destruction and bone cancer pain. Bone cancer pain is usually progressive as the disease advances, and is particularly difficult to treat. Recently, experimental models of bone cancer pain have been developed and have provided seminal insight in understanding the pathophysiology of bone cancer pain. Animal models of bone cancer provided the finding that bone destruction (osteolysis) is associated

Diphenhydramine as an analgesic adjuvant in refractory cancer pain.

Clinical and animal data suggest that antihistamines may have efficacy in the management of pain. While many mechanisms of action have been proposed for the analgesic action of antihistamines, the exact mechanism is unknown. Controlled clinical trials in different pain models have demonstrated that antihistamines have direct and adjuvant analgesic activity. We report three patients with advanced cancer pain refractory to adjuvants and oral, intravenous, and epidural opioids, who achieved sustained pain relief after the repeated administration of diphenhydramine. Diphenhydramine may be useful i

Role of methadone in the management of pain in cancer patients.

In most cancer patients who are treated with opioid analgesics for pain control, the type of opioid needs to be changed at least once because of the presence of side effects or the need to escalate analgesic doses to toxic levels. Methadone may be an excellent alternative in such patients due to its lack of known active metabolites, high lipid solubility, excellent absorption following oral and rectal delivery, and extremely low cost. However, it has unique problems, such as a long, unpredictable half-life and greater potency than previously assumed. At present, methadone is viewed as a second

[Treatment of cancer pain: the role of co-analgesics].

Opioid analgesics are widely acknowledged as the most important drugs for the treatment of chronic cancer pain. Although these drugs can in most cases control severe pain, even when they are used appropriately, they may produce new symptoms or exacerbate preexisting symptoms, most notably nausea and somnolence. The combination of severe pain, anorexia, chronic nausea, asthenia, and somnolence is a frequent finding in patient with advanced cancer. An adjuvant drug should meet at least one of the following criteria: 1) to increase the analgesic effect of opioids; 2) to decrease their toxicity; 3

New approaches to pain control in patients with cancer.

Pain affects most patients with malignant disease, and the prevalence of severe pain increases in the advanced stages of the condition. One in 5 patients with cancer has uncontrolled pain, even after 10 years of the use of the World Health Organization programme for cancer pain control and its 'three-step ladder' for the rational use of analgesics including morphine. Morphine has long been the 'gold standard' for the treatment of severe cancer pain. However, its side-effects, particularly sedation, cognitive impairment and myoclonus at high doses, have provoked the use of 'opioid rotation' to
