Building research capacity and capability to enhance the quality of living and dying addressing advancing frailty through integrated care: the ALLIANCE partnership.

BACKGROUND: Frailty affects around 10% of people aged over 65 years, increasing to 65% of those over 90 years. This number is increasing. Older people with frailty are projected to become the largest future users of care services as they near end of life. Living with frailty increases vulnerability to sudden deterioration, fluctuating capacity and mortality risk. This leads to complex needs, requiring integrated care, and an approach orientated towards living with, as well as dying from, advancing frailty. However, accessing care in a timely manner can be difficult. AIMS: To develop a sustain

Applying a mutual capacity building model to inform peer provider programs in South Africa and the United States: A combined qualitative analysis.

BACKGROUND: Globally, there is a treatment gap for substance use treatment and harm reduction services. As peer-delivered interventions expand to address this gap in both high-income countries (HICs) and low and middle-income countries (LMICs), they provide an opportunity to examine mutual capacity building, the bidirectional exchange of ideas between distinct settings to address common challenges. METHODS: The aim of this study was to explore the perspectives of patients in Cape Town and Baltimore about the acceptability and feasibility of a peer-delivered intervention using a combined quali

A Biological Approach to Building Resilience and Wellness Capacity Among Police Exposed to Posttraumatic Stress Injuries: Protocol for a Randomized Controlled Trial.

BACKGROUND: Law enforcement officers are routinely exposed to hazardous, disturbing events that can impose severe stress and long-term psychological trauma. As a result, police and other public safety personnel (PSP) are at increased risk of developing posttraumatic stress injuries (PTSIs) and disruptions to the autonomic nervous system (ANS). ANS functioning can be objectively and noninvasively measured by heart rate (HR), heart rate variability (HRV), and respiratory sinus arrhythmia (RSA). Traditional interventions aimed at building resilience among PSP have not adequately addressed the phy

Creating an Outpatient, Local Anesthetic Hand Operating Room in a Resource-Constrained Ghanaian Hospital Builds Surgical Capacity and Financial Stability.

INTRODUCTION: Expanding surgical capacity is very difficult in resource-constrained countries. Financial input and additional physical operating room space are needed. The surgical patient volume exists, but the lack of operating room time causes postponement of cases. Hand surgery is particularly important as it improves patient function and allows for a timely return to the workforce after injury. Some hand surgery cases may be performed under local anesthesia with a very basic instrument set in a procedure room. This arrangement eliminates the need and financial burden of an equipped operat

Building Surgical Capacity Sustainably in Resource-Constrained Ghanaian Hospital: A 7-Year Update on the Creation of an Outpatient Local Anesthetic Operating Room.

BACKGROUND: The Komfo Anokye Teaching Hospital (KATH) in Ghana serves a population of over 5.4 million people. The number of operating rooms creates a bottleneck to accessing surgical care. In a previously published paper, the plastic surgery department at KATH sought to utilize a wide-awake local anesthesia no tourniquet (WALANT) technique to perform hand surgery as a solution to improving surgical capacity. This technique, popularized by Donald H. Lalonde, was first utilized in February 2017 at KATH. We aim to provide an update on the utilization of this procedure room created almost a decad

Capacity-building strategies that support correctional and justice health professionals to provide best-evidenced based healthcare for people in prison: a systematic review.

BACKGROUND: The United Nations (UN) 2015 'Mandela Rules' stipulates that people in prison will have access to equivalent healthcare to other community members. This expectation has challenged prisons in high-income countries to strengthen healthcare delivery to better meet the needs of the growing number of incarcerated First Nations and older, frailer people, many with complex healthcare needs. Yet little is known about correctional and justice health professionals' ('prison workforces') capacity to identify and support people in prisons with complex healthcare needs. AIM: To identify the po

Building rehabilitation care capacity for older adults living in long-term care through family/care partner and physical therapist engagement: a scoping review.

BACKGROUND: Older adults living in long-term care homes often experience impairment in mobility and function requiring physical therapy rehabilitation. Maintaining residents' physical function can promote quality of life, reduce pain and skin breakdown, and support the provision of personal care. Family/care partners notably contribute to resident well-being. Understanding engagement between physical therapists and family/care partners is essential to advancing resident rehabilitation and health outcomes, particularly as staffing capacity, time, and access to rehabilitation services become inc

Building capacity for HIV and implementation science among students in the United States: the stimulating training and access to HIV research experiences (STAR) program.

BACKGROUND: Expanding HIV research capacity among the global majority (individuals identifying as Black/African American, American Indian and Alaska Native, Asian, Native Hawaiian and Other Pacific Islander, Multiracial, and Hispanic/Latino) is important. However, achieving national goals to increase the pool of implementation science and HIV early-stage investigators from underrepresented backgrounds remains elusive, largely due to limited investment in training and mentoring these individuals. To address this issue, we launched the Stimulating Training and Access to HIV Research Experiences

Building research capacity to adapt and develop Patient-Reported outcome measures in low- and middle-income countries: results from a psychometrics workshop in Tanzania.

BACKGROUND: Effective antiretroviral treatment has transformed HIV into a manageable chronic condition. In Tanzania, about 1.7 million people are living with HIV (PLWH), with 79% achieving viral suppression. Patient-Reported Outcome Measures (PROMs) evaluate key outcomes that matter most for PLWH and are an essential tool to evaluate and improve health system performance. Research is needed to adapt and test PROMs to increase their use in HIV care in many low- and middle-income countries including Tanzania. Since 2019, Northwestern University and Muhimbili University of Health and Allied Scien
