Suppression of serum thyrotropin with thyroxine in patients with Graves' disease: effects on recurrence of hyperthyroidism and thyroid volume.

Based on findings that thyroxine may have a beneficial effect on the recurrence of Graves' hyperthyroidism, we prospectively studied the effects of a TSH suppressive treatment with thyroxine on the course of Graves' disease in fifty patients with recent onset of hyperthyroidism. After the normalization of serum tri-iodothyronine (T3) and thyroxine (T4) concentrations, one group of patients was randomly assigned to a combined treatment with carbimazole and a TSH suppressive dose of T4 for 12 months, followed by another 12 months of TSH suppressive therapy alone. The other group of patients also

Anaplastic carcinoma of the thyroid in a patient receiving radio-iodine therapy for amiodarone-induced thyrotoxicosis.

We describe a case of amiodarone-induced thyrotoxicosis following heart valve replacement in a patient with a multinodular goitre. He responded to medical therapy with carbimazole and potassium perchlorate. Subsequent therapy with radio-iodine was followed 30 weeks later by the development of anaplastic thyroid carcinoma. It is possible that amiodarone caused a low uptake of radio-iodine by the thyroid. It is not known whether this possible low dose exposure to ionising radiation played a part in the development of the anaplastic carcinoma.

Antithyroid drug induced a granulocytosis: what still we need to learn?

Antithyroid drugs (ATDs) induced agranulocytosis is a rare but life threatening condition. We report a 29 years Filipino female diagnosed as having hyperthyroidism with normal base line blood counts, liver and renal profile. She was started on maximum 60 mg (20mg TID) oral dose of carbimazole since one month by her treating physician. Exactly after one month of treatment she presented to emergency room (ER) with fever, sore throat and generalized weakness for several days.

Iodine Deficiency-Induced Thyrotoxicosis Mimicking Graves' Disease: A Case of Triiodothyronine (T3)-Predominant Hyperthyroidism Without Goiter.

Triiodothyronine (T3)-predominant thyrotoxicosis without goiter is uncommon, with differential diagnoses including Graves' disease, thyroiditis, and iodine deficiency-related autonomy. Diagnostic certainty can be challenging in resource-constrained settings. We describe a 41-year-old man who presented with palpitations, heat intolerance, and weight loss and reported exclusive use of non-iodized rock salt for three years. Examination revealed tachycardia and tremor without goiter or orbitopathy. Thyroid function tests showed suppressed thyroid-stimulating hormone, elevated free T3, normal free

Radioiodine treatment of Basedow's disease: interference and influence factors, risk estimation.

I-131 treatment of Basedow's disease is effective and safe. Relatively high energy doses to the thyroid in the range of 150-200 Gray are needed in endemic goiter areas, where immunogenic hyperthyroidism may coincide with functional autonomy. Induction of euthyroidism by low doses of antithyroid drugs (methimazole, carbimazole) before I-131 treatment prevents complications and has no significant influence on outcome of I-131 therapy. As compared to alternative therapeutic modalities, the risks of I-131 are low.

Platysmal myoclonus in subclinical hyperthyroidism.

Hyperthyroidism is associated with various movement disorders, such as chorea and tremors. We report on a young Chinese woman with an unusual presentation of myoclonus, involving both platysmal muscles, in association with subclinical hyperthyroidism. The myoclonus was preceded by symptoms of hyperthyroidism, namely weight loss, menstrual disturbances, and heat intolerance. The movements abated with clonazepam and hyperthyroidism was treated with carbimazole. The myoclonus recurred briefly when she stopped taking clonazepam, but she has since remained well and euthyroid.

Successful radioiodine treatment in a 3 year old child with Graves' disease following antithyroid medication induced neutropenia.

A 3 year old child with Graves' disease and mitral valve prolapse became neutropenic on carbimazole therapy. She was switched to propylthiouracil but the neutropenia recurred. She was treated with radioiodine but required two doses of 113 MBq and then 198 MBq five months later before becoming hypothyroid. The mitral valve prolapse resolved when she was euthyroid on thyroxine replacement. Antithyroid drugs, surgery, and radioiodine all have a place in the management of the thyrotoxic child.

Thyrotropin receptor antibodies associated with post-operative relapse of thyrotoxicosis in a pregnancy complicated by neonatal thyrotoxicosis.

This case describes the clinical, biochemical and immunological features associated with relapse of thyrotoxicosis during pregnancy in a patient who had recently undergone a subtotal thyroidectomy for Graves' disease. The baby, shortly after birth, showed clinical and biochemical features of thyrotoxicosis which responded to carbimazole therapy. Thyrotropin receptor antibodies and thyroid stimulating antibodies were present in the blood of the mother and baby. The clinical course of the neonatal thyrotoxicosis correlated with the TSH receptor antibody levels.

Thyroid storm presenting as jaundice and complete heart block.

Thyroid storm is a difficult diagnosis in "apathetic" variant of hyperthyroidism. The clinical features may not be evident. Abnormal atrioventricular (AV) conduction, such as complete heart block, in thyrotoxicosis is uncommon. We report a case of a 16-year-old girl presenting with fever, jaundice, heart failure and complete heart block in whom the diagnosis of thyroid storm was initially missed because of the unusual presenting features. Prompt resolution of the conduction abnormality occurred when treatment with carbimazole, intravenous iodide and dexamethasone was instituted.

Thionamides inhibit the transcription factor nuclear factor-kappaB by suppression of Rac1 and inhibitor of kappaB kinase alpha.

Thionamides, inhibitors of the thyroid peroxidase-mediated iodination, are clinically used in the treatment of hyperthyroidism. However, the use of antithyroid drugs is associated with immunomodulatory effects, and recent studies with thionamide-related heterocyclic thioderivates demonstrated direct anti-inflammatory and immunosuppressive properties. Using primary human T-lymphocytes, we show that the heterocyclic thionamides carbimazole and propylthiouracil inhibit synthesis of the proinflammatory cytokines tumor necrosis factor (TNF)alpha and interferon (IFN)gamma. In addition, DNA binding o

[Clinical aspects, diagnosis and drug therapy of hyperthyroidism].

Graves' disease and toxic uni- or multinodular goiter are the most frequent causes of hyperthyroidism. Graves' disease is caused by thyroid stimulating immunoglobulins which are directed against the TSH receptor of thyroid follicular cells. Graves' disease affects more females than males and is associated with diffuse goiter and a rapid appearance of symptoms and signs of hyperthyroidism. Patients with Graves' disease are on average younger than patients with toxic nodular goiter. The diagnosis of Graves' disease is usually easy, particularly if signs of endocrine opthalmopathy are present. To

Normal suppressive T cell function of Epstein-Barr virus-induced B cell activation in Graves' disease.

Several studies have demonstrated abnormalities of T cell regulation of Epstein-Barr virus (EBV)-induced B cell activation in systemic autoimmune diseases, such as rheumatoid arthritis, systemic lupus erythematosus, and systemic sclerosis. To investigate whether this abnormality is a common feature of other autoimmune diseases, we studied 10 EBV-immune normal subjects and 22 EBV-immune patients with Graves' disease (GD); 11 had newly diagnosed hyperthyroidism, and 11 had received carbimazole treatment for hyperthyroidism for at least 6 months. Peripheral B lymphocytes infected with EBV were cu

Grave's disease as a manifestation of immune reconstitution inflammatory syndrome in an HIV-infected child on highly active antiretroviral therapy: A case report.

INTRODUCTION: Immune reconstitution inflammatory syndrome (IRIS) can unmask autoimmune disease after antiretroviral therapy (ART), and Graves' disease has been reported as a late autoimmune manifestation, though pediatric cases are exceptionally rare. CASE PRESENTATION: A 9-year-old Ethiopian boy with vertically acquired HIV, diagnosed at age 6 during an acute illness, had presented at that time with profound immunosuppression (CD4 23 cells/mm³, HIV RNA ∼150,000 copies/mL) and was started on combination ART. He achieved sustained virologic suppression and marked immune recovery (CD4 >1800 cel

Metabolism of 35S-labelled antithyroid drugs in man.

Differences in the metabolic fate of antithyroid drugs influence the optimal frequency of administration and their therapeutic efficacy. (35)S propylthiouracil differed from the (35)S imidazoles (carbimazole and methimazole) in the more rapid absorption and excretion and the shorter biological half-life in the plasma of the former. Renal function may have a more important influence on the biological half-life of the drugs than thyroid status. Further work is required to determine the optimal frequency of administration for each compound.

Conversion of Primary Hypothyroidism to Hyperthyroidism: A Case Report.

A 51-year-old Caucasian male developed Graves' thyrotoxicosis following long-standing treatment for hypothyroidism. After a short period of treatment with carbimazole, he developed agranulocytosis and required total thyroidectomy. In this relevant case report, we review several pathogenetic mechanisms that explain the transformation of autoimmune hypothyroidism into Graves' disease and the possible approaches to the management of agranulocytosis secondary to antithyroid medications. Further studies are required to determine the best way to manage severe thyrotoxicosis when agranulocytosis deve

Thyrotoxicosis: changing trends in treatment.

A review of treatment of thyrotoxicosis over a 14 year period has led to three modifications in our approach. Firstly, we now treat fewer patients with a prolonged course of carbimazole, recommending surgery as primary treatment in more patients, particularly the young and middle aged severe thyrotoxics. Secondly, we use conventional therapeutic doses of radioiodine very much less frequently, because of the unpredictable response, and when using radioiodine attempt thyroid ablation more frequently. Thirdly, conventional 'ablative' doses are frequently too low.

Childhood thrombocytopenia associated with Graves disease is distinct from idiopathic thrombocytopenic purpura.

The authors report a case of thrombocytopenia in a 10-year-old Chinese girl who was also found to have Graves disease 6 months later. The clinical, hematologic, and bone marrow features were indistinguishable from chronic idiopathic thrombocytopenic purpura, but the patient's platelet counts were normalized concurrently with the control of thyrotoxicosis with carbimazole treatment. Childhood thrombocytopenia associated with autoimmune thyroid disease is a rarely reported condition and its management differs from that of idiopathic thrombocytopenic purpura.

[The use of antithyroid drugs should be reduced as much as possible in the first trimester].

The treatment of choice for hyperthyroidism in pregnancy is antithyroid drugs, but the potential risk of birth defects is of major concern. For the use of thiamazole and carbimazole, there is consistent evidence of an increased risk of birth defects, which are often severe. For the use of propylthiouracil, the evidence is less clear. These birth defects may be less severe, and a Danish study which included all birth defects diagnosed before the age of two years showed an increased risk of birth defects in the face and neck region and in the urinary system after the use of propylthouracil.
