Transcatheter Tricuspid Valve Edge-to-Edge Repair in Cardiac Wild-Type Transthyretin Amyloidosis.

BACKGROUND: Cardiac transthyretin amyloidosis (ATTR-CM) is an infiltrative, restrictive cardiomyopathy leading to heart failure across all stages of the disease. While specific disease-modulating therapies evolve, interventional therapeutic approaches on atrioventricular valve dysfunction have not yet been investigated in this cardiomyopathy and remain subject to personalized strategy. CASE SUMMARY: We present 2 patients with wild-type ATTR-CM who underwent successful transcatheter tricuspid valve edge-to-edge repair (T-TEER) for severe tricuspid regurgitation (TR) 2-4 years after initial dia

Accurate Classification of Non-ischemic Cardiomyopathy.

PURPOSE OF REVIEW: This article aims to review the accurate classification of non-ischemic cardiomyopathy, including the methods, basis, subtype characteristics, and prognosis, especially the similarities and differences between different classifications. RECENT FINDINGS: Non-ischemic cardiomyopathy refers to a myocardial disease that excludes coronary artery disease or ischemic injury and has a variety of etiologies and high incidence. Recent studies suggest that traditional classification methods based on primary/mixed/acquired or genetic/non-genetic cannot meet the precise needs of contemp

Atypical cardiac amyloidosis phenotypes identified at transplant: a case series.

BACKGROUND: Transthyretin amyloidosis (TTR) is increasingly implicated as an aetiology of advanced cardiomyopathy. Typically, both genetic variant (TTRv) and wild-type (TTRwt) amyloidosis present with a restrictive phenotype. We present a series of three patients who were found to have cardiac amyloidosis on explant following heart transplant (HT) who had atypical, non-restrictive phenotypes. CASE SUMMARY: All three patients were men, three were Black, and only one had an alternative pre-HT explanation for their advanced, dilated cardiomyopathy. Pre-HT transthoracic echocardiograms were notab

Dilated Cardiomyopathy With Mid-Range Ejection Fraction at Diagnosis: Characterization and Natural History.

Background Limited data are available on mid-range ejection fraction (mrEF) patients with dilated cardiomyopathy. We sought to define the characteristics, evolution, and long-term prognosis of dilated cardiomyopathy patients with mrEF at diagnosis. Methods and Results We analyzed all dilated cardiomyopathy patients consecutively evaluated in the Trieste Heart Muscle Disease Registry from 1988 to 2013. mrEF and reduced ejection fraction (rEF) were defined as baseline left ventricular (LV) ejection fraction values between 40% and 49% and <40%, respectively. All-cause mortality or heart transplan

Animal models of primary myocardial diseases.

Feline and canine cardiomyopathies (primary myocardial diseases) were reviewed and divided into three groups based on the clinical, hemodynamic, angiocardiographic, and pathologic findings: (1) feline and canine hypertrophic cardiomyopathy, (2) feline and canine congestive (dilated) cardiomyopathy, and (3) feline restrictive cardiomyopathy. All three groups consisted predominantly of mature adult male cats and dogs. Cardiomyopathy in the hamster and turkey was also reviewed. The most common presenting signs were dyspnea and/or thromboembolism in the cat, systolic murmurs with gallop rhythms on

Classification of cardiomyopathies: bringing order to complexity.

Cardiomyopathy classification remains challenging due to their extraordinary clinical, morphological, and genetic heterogeneity. As diagnostic technologies evolve, so too must the frameworks by which we conceptualize and communicate these diseases. Since the 2008 ESC morphofunctional classification and the genotype-phenotype integrated MOGE(S) system proposed in 2013, substantial advances in imaging and genetics have prompted a revised 2023 ESC phenotype-first model. The five current phenotypes-dilated cardiomyopathy (DCM), hypertrophic cardiomyopathy (HCM), restrictive cardiomyopathy (RCM), a

[Pathology of the "primary" cardiomyopathies (author's transl)].

The definition and clinical classification of the cardiomyopathies which the author uses are briefly mentioned, macroscopic, histologic, histochemical and electron microscopic appearances of the three clinical groups are described in detail. Classification is made into: 1. Congestive cardiomyopathy, 2a. hypertrophic cardiomyopathy with obstruction, 2b. hypertrophic cardiomyopathy without obstruction, 3. obliterative or restrictive cardiomyopathy. Löffler's endocarditis parietalis fibroplastica and endomyocardial fibrosis are examples which are compared with eosinophilic myocarditis and patholo

Noninvasive Diagnostic Modalities in an Isolated Case of Cardiac Amyloidosis.

Amyloidoses are a family of inherited or acquired disorders characterized by the deposition of insoluble extracellular protein fibrils in various organs and tissues, thereby impairing their function. Amyloidoses are typically misfolded proteins, and on rare occasions, can deposit in the myocardium resulting in an infiltrative/restrictive cardiomyopathy. Restrictive cardiomyopathy is an underdiagnosed cause of congestive heart failure (CHF) with preserved ejection fraction, atrial and ventricular arrhythmias along with conduction defects. In elderly patients, as with this study, cardiac amyloid

Identification of a Novel Titin Variant Underlying Myocardial Involvement in Neurofibromatosis Type 1.

Because of the rare co-occurrence, it remains a question whether cardiomyopathy is a true association of neurofibromatosis type 1. A boy with café-au-lait spots manifested restrictive cardiomyopathy. Whole exome sequencing confirmed the genetic diagnosis of neurofibromatosis and further identified a novel titin (TTN) missense variant. The significance of the variant is supported by its de novo origin, in silico predictions, and evolutionary conservation. Modern genetics raises an intriguing explanation for the unexpected phenotype and adds to the evolving role of TTN variants in cardiomyopathy

Inherited Cardiomyopathies and the Role of Mutations in Non-coding Regions of the Genome.

Cardiomyopathies (CMs) are a group of cardiac pathologies caused by an intrinsic defect within the myocardium. The relative contribution of genetic mutations in the pathogenesis of certain CMs, such as hypertrophic cardiomyopathy (HCM), arrythmogenic right/left ventricular cardiomyopathy (ARVC) and left ventricular non-compacted cardiomyopathy (LVNC) has been established in comparison to dilated cardiomyopathy (DCM) and restrictive cardiomyopathy (RCM). The aim of this article is to review mutations in the non-coding parts of the genome, namely, microRNA, promoter elements, enhancer/silencer e

Impact of the 2016 revision of US Pediatric Heart Allocation Policy on waitlist characteristics and outcomes.

US Pediatric Heart Allocation Policy was recently revised, deprioritizing candidates with cardiomyopathy while maintaining status 1A eligibility for congenital heart disease (CHD) candidates on "high-dose" inotropes. We compared waitlist characteristics and mortality around this change. Status 1A listings decreased (70% to 56%, P < .001) and CHD representation increased among status 1A listings (48% vs 64%, P < .001). Waitlist mortality overall (subdistribution hazard ratio [SHR] 0.96, P = .63) and among status 1A candidates (SHR 1.16, P = .14) were unchanged. CHD waitlist mortality trended be

Endomyocardiofibrosis: A Systematic Review.

Endomyocardiofibrosis was described first time in Uganda as an infrequent restrictive cardiomyopathy with a poor prognosis, characterized by fibrosis of the ventricular subendocardium and severe restrictive physiology leading to difficult therapeutic management and frequently associated with hypereosinophilic syndrome. Its higher prevalence in the tropics and its relationship in some cases with hypereosinophilic endocarditis has led to the search for genetic, infectious, autoimmune and nutritional causes, but its etiology remains unclear. It is a rare cardiomyopathy, difficult to diagnose and

Inclusion of electric disturbance type cardiomyopathy in the classification of cardiomyopathy: a current review.

There are cases of cardiomyopathy which are not easily classifiable into hypertrophic, dilated or restrictive type. They often show ventricular arrhythmia, right or left bundle branch block, intraventricular conduction disturbance, atrioventricular conduction disturbance, and sinus node dysfunction (sick sinus syndrome). Endomyocardial biopsy in such cases reveals advanced myocardial disease, for which we propose the simpler term "electric disturbance type cardiomyopathy (ECM)". In our consecutive series of studies of cardiomyopathy employing endomyocardial biopsy in 573 cases, 264 (46%) were
