Heart transplantation procedures, which present a number of challenges not generally seen in perioperative nursing practice, provide important knowledge for perioperative nurses because they demonstrate the importance of organization, communication, collaboration, and resourcefulness among surgical team members. On Friday, Aug 13, 1993, the Virginia Heart Center at Fairfax Hospital, Falls Church, implanted donor hearts in three adult recipients in a single day. The day was even more challenging because of elective coronary artery bypass graft procedures performed on three other patients. Succe

PURPOSE: To evaluate the perioperative complications associated with surgical correction in neuromuscular scoliosis and to identify the risk factors associated with these complications.
METHODS: A retrospective review of the hospital charts of patients with neuromuscular scoliosis who underwent surgical correction at a medical center was performed.
RESULTS: Data was available on a total of 175 patients. The overall perioperative complication rate was 33.1% (96 complications in 58 patients). Complications were subdivided into pulmonary issues (19.4%), wound and implant infections (9.7%), card

Improved surgical and medical management has led to an increase in survival after staged univentricular palliative procedures. Subsequently, this improved survival has led to an increase in the number of patients who will present for noncardiac surgical interventions with Fontan physiology. A comprehensive understanding of normal Fontan physiology and the perturbations that the proposed surgical procedure will likely have is necessary to care for and design a comprehensive anesthetic plan that takes into account the effects of anesthetic agents, ventilation strategies, cardiovascular drugs, an

In total arch-replacement surgery, careful attention must be paid to the presence of anomalous arch vessels, because their presence may require surgeons to change the brain-protection strategy during deep hypothermic circulatory arrest and selective cerebral perfusion. The anomaly we most often encounter is an isolated left vertebral artery (ILVA). We describe a case involving a straightforward but physiological way of reconstructing the ILVA in which a cuff is hollowed together with the left subclavian artery.

Several surgical techniques have been described to channel the right pulmonary venous return to the left atrium in the Scimitar syndrome which includes direct reimplantation, graft interposition, or repair. Because these techniques can result in pathway obstruction, Lugones has devised a technique which uses in situ pericardium to construct a wide tunnel connecting the pulmonary venous return to the left atrium. We report the use of Lugones technique in an adult with Scimitar syndrome.

The geriatric patient has a high incidence of foot deformities and may be considered a good surgical candidate if healthy and psychologically ready for the surgical procedure. Foot problems in the geriatric population that may require surgical intervention can be grouped into approximately five categories, including (1) localized orthopedic disorders (ie, bunions, hammer toes, and so on); (2) skin and nail problems (ie, onychomycosis); (3) degenerative and inflammatory arthritis; (4) diabetic foot disorders; and (5) neuromuscular disease. However, age-related changes in cardiovascular, pulmona

Approximately 0.6% of the United States population identifies as transgender or gender nonbinary. The term transgender refers to a person who experiences incongruence between their sex at birth and their gender identity. The care of the transgender patient is complex, often requiring the expertise of various medical and surgical specialties. The prevalence of gender-affirming surgery is increasing as societal acceptance and cultural norms are shifting. While gender-affirming procedures include a variety of body systems, airway and facial procedures are of particular importance to anesthesia pr

We report two cases of perioperative haemorrhagic shock after accidental puncture of trunc vessels, during emergency shunt procedures in children who suffer of severe intracranial hypertension. In both cases it's the peritoneal internalization of the shunt with Portnoy trocar which is responsible of these deep vascular wounds. Evolution was favourable in both cases with cardiovascular resuscitation, transfusion and surgical haemostatic correction. Few days later, an internal shunt was performed in the second patient, while the first patient did not need shunt anymore. These two accidents lead

Single stage repair of syndromes of coarctation and interruption of the aortic arch is a routine procedure in many surgical centres with good immediate results. The classical technique of aortic repair is based on the principles of Crafoord's extended resection anastomosis. Recoarctation is not an unusual long-term complication. A technique of enlarging angioplasty of the aorta using a patch of pulmonary artery has been developed and used in 22 neonates with obstruction of the aortic arch associated with ventricular septal defect with an average age and body weight of 15 days and 2.9 Kg respec

Early ambulation following cardiovascular and thoracic surgery (CVTS) is associated with improved postoperative outcomes. However, barriers such as patient debility, high acuity, hemodynamic instability, cardiopulmonary complications, or resource limitations (e.g., intensive care unit setting, specialized equipment, or staff training) often prevent or delay early ambulation. Overground robotic exoskeletons (ORE) may facilitate earlier and higher-dose ambulation, yet their role in acute CVTS recovery remains uninvestigated. To evaluate the safety, feasibility, and preliminary outcomes of ORE-as

The history of cardiothoracic surgery at Baylor College of Medicine begins in 1948, when Michael E. DeBakey was recruited as chair of the Department of Surgery. With the addition of Denton A. Cooley, E. Stanley Crawford, and other surgical legends, Baylor College of Medicine quickly rose to the forefront of cardiothoracic surgery. Highlights in its history include treatment of aortic aneurysm and dissection, development of synthetic aortic substitutes, early experience with cardiac replacement valves, the first successful use of coronary artery bypass grafts to revascularize the heart, and the

A single-donor fibrin sealant system was used in 689 thoracic and cardiovascular surgical procedures over the 4-year period between April 1, 1985, and March 31, 1989. An excellent overall success rate (646/689, 94% effective) was achieved with specific applications, including reduction of leakage of air (29/33, 88% effective), blood (595/634, 94% effective), and fluid (14/14, 100% effective), as well as positioning of anatomical structures such as coronary bypass grafts (8/8, 100% effective). Application methods included use of spray bottles (477/497, 96% effective), syringes (165/186, 89% eff

The ideal surgical approach is unclear in adult patients with coarctation of the aorta that is associated with other cardiovascular pathologies that require intervention. Standard median sternotomy allows simultaneous, coronary revascularization surgery, valve replacement and repair of aortic coarctation. However the collateral circulation and the anatomy of the mammary arteries must be determined, to avoid possible complications. We report a case of a 69 year-old man with aortic coarctation, aortic stenosis, coronary artery disease and internal mammary artery dilatation who underwent concomit

Hypertension is a frequently encountered abnormality in patients being prepared for surgical procedures. This condition complicates anesthetic and postoperative management, but careful monitoring and treatment allow hypertensive patients to tolerate surgery safely. Particular attention should be directed toward continuing antihypertensive medicine until the time of the surgical procedure or initiating treatment before it, monitoring the blood pressure frequently after the operation, and controlling postoperative hypertension with one of many parenteral agents available. The possibility of the

BACKGROUND: The surgical approach to children with complex cardiovascular and pulmonary anomalies is still controversial. Staged operations through multiple incisions are often performed in this setting.
OBJECTIVE: The different applications and clinical advantages of a bilateral thoracosternotomy approach to complex cardiothoracic disease requiring surgical repair were reviewed retrospectively.
METHODS: Between January 1993 and June 1995, 33 patients, aged between 2 months and 17 years (mean 7.8 +/- 5.3) underwent surgical treatment of complex cardiovascular or pulmonary disease using a cla

PURPOSE OF REVIEW: Transcatheter valve therapies for mitral and tricuspid regurgitation are alternative methods to more invasive surgical treatment in candidates with high or prohibitive surgical risk. Echocardiography is the primary imaging modality for patient selection, pre-procedural planning, intra-procedural guidance, and post-procedural follow-up.
RECENT FINDINGS: This article outlines the applications of transcatheter valve repair therapy for mitral and tricuspid regurgitation based on the current cardiovascular guidelines and a growing body of evidence. In this review, we provide a s

Successful implementation of automated blood sampling (ABS) into a telemetry instrumented canine cardiovascular model provides simultaneous cardiovascular assessment of novel compounds while collecting multiple blood samples for analysis of drug level, cytokines, and biomarkers. Purpose-bred male Beagle dogs (n = 36) were instrumented with a dual-pressure telemetry transmitter and vascular access port. Modifications to acclimation practices, surgical procedures, and housing were required for implementation of ABS in our established cardiovascular canine telemetry colony. These modifications ha

Double outlet right ventricle (DORV) is a heterogeneous group of congenital heart diseases that require individualized surgical approach based on precise understanding of the complex cardiovascular anatomy. Physical 3-dimensional (3D) print models not only allow fast and unequivocal perception of the complex anatomy but also eliminate misunderstanding or miscommunication among imagers and surgeons. Except for those cases showing well-recognized classic surgical anatomy of DORV such as in cases with a typical subaortic or subpulmonary ventricular septal defect, 3D print models are of enormous v
