Recovery of ceftazidime-resistant Klebsiella pneumoniae from pediatric liver and intestinal transplant recipients.

The purpose of this report was to determine the frequency of colonization and bacteremia with ceftazidime-resistant Klebsiella pneumoniae among pediatric candidates for and recipients of liver (LTx) and/or intestinal (ITx) transplantation. Between January and December 1994, surveillance stool cultures obtained from 51 children on the abdominal transplant service were planted on a selective medium containing 2 microg/ml of ceftazidime. Isolates of K. pneumoniae which grew on the selective medium were screened for extended-spectrum beta-lactamase (ESBL) production by the double-disk synergy test

Ceftazidime-Avibactam-Induced Neurotoxicity Manifesting as Seizure in an Older Adult: A Case Report.

This single-case report describes severe neurotoxicity manifesting as seizure in an older, critically ill patient with acute kidney injury during treatment with ceftazidime-avibactam (CZA-AVI), a combination antibiotic used for multidrug-resistant Gram-negative infections. Although generally safe, neurotoxicity from CZA-AVI, while uncommon, can be severe. The patient, a 78-year-old woman with multiple comorbidities and acute kidney injury, developed impaired consciousness progressing to generalized seizures a few days after initiating CZA-AVI. Electroencephalography confirmed seizure activity.

Synergistic mode of action of a ceftazidime-hexa-arginine conjugate with resistance-overcoming properties.

UNLABELLED: We previously described that a non-toxic ceftazidime-hexa-arginine conjugate (CTZ-R6) surpassed the antibacterial potency of ceftazidime (CTZ) against multi-resistant bacteria by up to 1,000-fold and effectively reduced the bacterial load in murine bacteremia. However, the mechanistic understanding of its resistance-overcoming activity remained elusive. Here, we report on the mode of action of CTZ-R6 in gram-positive and gram-negative bacteria. Against a panel of multi-resistant Enterobacteriaceae and non-fermenters, CTZ-R6 outperformed ceftazidime. The conjugate synergized with av

An Application of a Physiologically Based Pharmacokinetic Approach to Predict Ceftazidime Pharmacokinetics in a Pregnant Population.

Physiological changes during pregnancy can alter maternal and fetal drug exposure. The objective of this work was to predict maternal and umbilical ceftazidime pharmacokinetics during pregnancy. Ceftazidime transplacental permeability was predicted from its physicochemical properties and incorporated into the model. Predicted concentrations and parameters from the PBPK model were compared to the observed data. PBPK predicted ceftazidime concentrations in non-pregnant and pregnant subjects of different gestational weeks were within 2-fold of the observations, and the observed concentrations fel

A Selective Culture Medium for Screening Ceftazidime-Avibactam Resistance in Enterobacterales and Pseudomonas aeruginosa.

The SuperCAZ/AVI medium was developed for screening ceftazidime-avibactam (CZA) resistance among Gram-negative bacteria (Enterobacterales and Pseudomonas aeruginosa). It was evaluated using 50 CZA-susceptible and 42 CZA-resistant Gram-negative isolates. Its sensitivity and specificity of detection were 100%. Excellent performance of the medium was also observed by testing spiked stools, with the lower limit of detection ranging from 101 to 102 CFU/ml. This screening medium provides the opportunity to detect CZA-resistant isolates regardless of their resistance mechanisms.

Variations in ceftazidime and amoxicillin-clavulanate susceptibilities within a clonal infection of Burkholderia pseudomallei.

A patient with a clonal infection of Burkholderia pseudomallei had subpopulations with ceftazidime and amoxicillin-clavulanate susceptibilities that differed among the clinical specimens. Resistance was associated with a novel Cys69Tyr substitution in the Ambler class A beta-lactamase. Susceptibility testing of multiple colony variants from different sites should be performed for patients with culture-confirmed melioidosis.

Effect of ceftazidime, amikacin and ciprofloxacin on biofilm formation by some enterobacterial clinical isolates.

The effect of sub-minimal inhibitory concentrations of antimicrobial agents on the biofilm formationto polystyrene by Escherichia coli, Proteus vulgaris, Providencia stuartii, and Morganella morganii was investigated by examining eight clinical strains. All the isolates tested were efficient biofilm-forming strains in the microtiter plate assay, with crystal violet staining (OD595 nm) ranging from 0.13 +/- 0.01 for P. stuartii ER21870 to 1.23 +/- 0.02 for P. vulgaris ER50120. The biofilm formation of the majority of the strains was affected in the presence of ceftazidime or ciprofloxacin: biof

Post-antibiotic and synergic effects of fluoroquinolones and ceftazidime in combination against Pseudomonas strains.

Infectious complications are still the leading cause of morbidity and mortality in seriously ill patients [1-3]. To combat the resistance of (mainly Gram-negative, non-fermentative) bacteria, e.g. Pseudomonas aeruginosa, to a wide spectrum of antibiotics, drug combination therapy has been widely adopted as standard clinical practice since the late 1990s. beta-Lactam combinations are not optimal and the potential of nephrotoxicity and ototoxicity from aminoglycosides has caused clinicians to evaluate new possibilities, such as combinations of fluoroquinolones and beta-lactams. We examine here t

Cefuroxime resistance in Klebsiella pneumoniae. Susceptibility to cefotaxime and ceftazidime despite production of ESBLs.

The production of beta-lactamases, the outer membrane protein (OMP) patterns, some clinical impacts and the prevalence of resistance among cefuroxime-resistant Danish clinical isolates of Klebsiella pneumoniae were investigated. Fifteen resistant and five susceptible strains were collected from 14 patients during 1991-1994. Isolates from five patients produced extended-spectrum beta-lactamases (ESBLs). Cefuroxime resistance was accompanied by a 10-fold elevation of ciprofloxacin minimal inhibitory concentration (MIC), and for some isolates by an alteration of the OMP pattern. The relationship
