Pathophysiology of Cerebral Vascular Dysfunction in Pregnancy-Induced Hypertension.

PURPOSE OF REVIEW: To review and summarize what is known about cerebrovascular derangements during preeclampsia. RECENT FINDINGS: Preeclampsia is a devastating disorder of pregnancy with no known cure. Little is known about the pathophysiological mechanisms which lead to the symptoms of the disorder, particularly with regard to individual vascular beds such as the cerebral circulation. Studies suggest that the cerebrovascular dysfunction characteristic of the preeclampsia syndrome is characterized by alterations in cerebral blood flow autoregulation and opening of the blood-brain barrier. Cerebral hemodynamic impairment: methods of measurement and association with stroke risk.

Stenosis or occlusion of the major arteries of the head and neck may cause hemodynamic impairment of the distal cerebral circulation. Hemodynamic factors may play an important role in the pathogenesis of ischemic stroke for patients with cerebrovascular disease. Several neuroimaging methods are currently available for the indirect assessment of the hemodynamic effect of atherosclerotic stenosis or occlusion on the distal cerebrovasculature. Because these methods rely on different underlying physiologic mechanisms, they are not interchangeable. Two basic categories of hemodynamic impairment can

Neurological disorders in Northern Tanzania: A 6-year prospective hospital-based case series.

BACKGROUND: The burden of neurological disorders is large and altered by the HIV epidemic. OBJECTIVES: We describe the pattern of neurological disorders and their association with HIV infection in adult patients attending a consultant hospital in Northern Tanzania. METHODS: In this prospective cross-sectional study, we collected data on adult neurological referrals over a 6-year period between 2007-13. The odds of HIV infection, across neurological categories adjusted for age and sex, was calculated. RESULTS: Of 2037 participants, 54.8% were male and 45.2% were female. The median age of par

[Depression in internal medicine. Cerebral ictus].

Despite the fact that it is one of the main and most common neuropsychological complications of cerebral ictus, depression is often overlooked and at times even mistaken for poor motivation, above all in patients with marked speech disorders. Several factors (organic, psychological, social, etc.) contribute to depression and for this reason simple pharmacological treatment is often not sufficient. The paper discusses the most important data taken from the literature relating to this topic.

Poststroke psychosis in an 8-year-old child with moyamoya disease.

Psychiatric illnesses following cerebrovascular accidents has been a popular area of research in recent times. Though poststroke anxiety and mood disorders including depression have been reported often, reports of poststroke psychosis have been considerably rarer. This case report documents an even rarer occurrence of a psychotic disorder in an 8-year-old child with moyamoya disease following a cerebrovascular accident. The child was managed with low-dose atypical antipsychotic, risperidone and the psychotic symptoms eventually were under control around 3 weeks following the initiation of psyc

Vascular parkinsonism: an update.

Vascular parkinsonism (VP), resulting from cerebrovascular disease, is a rare disorder with a characteristic motor and non-motor clinical profile distinct from sporadic/idiopathic Parkinson disease (PD) and other parkinsonian disorders. It accounts for 3-6% of all parkinsonian syndromes and may overlap with other parkinsonisms. Clinical features of VP are heterogenous and include bilateral rigidity with lower body predominance, bradykinesia, postural instability, shuffling gait, falls, corticospinal symptoms and cognitive impairment, tremor being rare or absent. Migraine and cerebrovascular disease: still a dangerous connection?

Migraine is a common type of headache in young adults, with an estimated prevalence of 4% before puberty and as high as 25% in women by their mid to late 30s. About one third of migraineurs experience transient neurological symptoms known as auras, which characterize a variant known as migraine with aura. Many evidences have shown an increased risk of vascular events in patients affected by migraine, particularly among women and among migraine patients with aura. Potential underlying mechanisms include endothelial dysfunction, hypercoagulability, platelet aggregation, vasospasm, cardiovascular

Pentoxifylline. A review of its pharmacodynamic and pharmacokinetic properties, and its therapeutic efficacy.

Pentoxifylline (oxpentifylline) is an orally active haemorheological agent for the treatment of peripheral vascular disease, cerebrovascular disease and a number of other conditions involving a defective regional microcirculation. Pentoxifylline acts primarily by increasing red blood cell deformability, by reducing blood viscosity and by decreasing the potential for platelet aggregation and thrombus formation. Extensive open and placebo-controlled studies have shown that pentoxifylline 600 to 1200 mg/day for at least 6 weeks is associated with subjective and objective improvements in 60 to 100

Cerebrovascular pathology and misdiagnosis of multiple system atrophy: An autopsy study.

BACKGROUND: Multiple system atrophy (MSA) is a progressive neurodegenerative disease characterized by a combination of dysautonomia, parkinsonism, and cerebellar ataxia. Other disorders can mimic MSA, but it is unknown whether cerebrovascular pathology, so-called "vascular parkinsonism," can mimic MSA. This study aimed to determine the clinicopathological features and red flags for vascular parkinsonism masquerading as MSA. METHODS: Using a brain bank database, we screened 270 patients with an antemortem diagnosis of MSA, who did not have pathologic evidence of MSA, but rather cerebrovascular

Current evidence and emerging strategies in the management of cerebrovascular disease: a systematic review and narrative synthesis of contemporary literature.

BACKGROUND: Cerebrovascular disease is a leading cause of global mortality and disability, with ischemic stroke comprising ∼ 85% of cases. Despite advances in reperfusion therapies, persistent challenges including delayed presentation, treatment inequities, fragmented rehabilitation, and failed neuroprotective translation necessitate a contemporary evidence synthesis. OBJECTIVES: To synthesize current evidence and emerging strategies in cerebrovascular disease management, evaluate diagnostic and pharmacological advances, and identify unresolved controversies and research gaps across the trans

The impact of cerebrovascular disease.

Stroke may be affected by pathologic conditions such as atherosclerosis, vascular injury, coagulopathy, and increased platelet aggregability. Knowledge of the pathophysiology of stroke is critical in order to effectively prevent or treat this disabling state. Cerebrovascular disease is the third leading cause of death in the USA. It is also associated with significant morbidity, leading to extremely high health care costs and lost productivity. Total estimated costs of stroke are estimated to be more than $45 billion annually. This is an especially important issue for patients with diabetes, w

Intracranial Calcification Masquerading as Hemorrhage in a Patient with Multiple Myeloma Presenting with Facial Neuropathy.

Multiple myeloma is an immunoglobulin-producing plasma cell neoplasm that commonly affects the bones, kidneys, the hematopoietic system, and rarely the nervous system. Peripheral nervous system involvement in the form of cranial neuropathy, radiculopathy, and polyneuropathy are common. Compressive myelopathy constitutes the majority of central nervous system disorders followed by cerebrovascular disorders, intracranial plasmacytomas, and leptomeningeal myelomatosis. Cerebrovascular complications such as acute ischemic stroke and transient ischemic attack are not uncommon. Cerebrovascular Steal Phenomenon and Electroconvulsive Therapy: A Case Report and Review of the Literature.

Electroconvulsive therapy (ECT) is a safe and effective treatment for major depressive disorder, but cerebrovascular and cardiovascular complications, although rare, remain the most concerning. This is particularly notable in those with preexisting cerebrovascular disease, which impacts dynamic cerebral autoregulation. In these patients, the increased blood flow to the seizing portions of the brain induced by ECT potentially can reduce cerebral blood flow to ischemic areas, possibly causing adverse neurological events. The authors describe a patient with chronic cerebral ischemic disease, chro

Impact of Sleep Disorders and Disturbed Sleep on Brain Health: A Scientific Statement From the American Heart Association.

Accumulating evidence supports a link between sleep disorders, disturbed sleep, and adverse brain health, ranging from stroke to subclinical cerebrovascular disease to cognitive outcomes, including the development of Alzheimer disease and Alzheimer disease-related dementias. Sleep disorders such as sleep-disordered breathing (eg, obstructive sleep apnea), and other sleep disturbances, as well, some of which are also considered sleep disorders (eg, insomnia, sleep fragmentation, circadian rhythm disorders, and extreme sleep duration), have been associated with adverse brain health.
