Ultrasound-Guided Interscalene Catheter Complicated by Persistent Phrenic Nerve Palsy.

A 76-year-old male presented for reverse total shoulder arthroplasty (TSA) in the beach chair position. A preoperative interscalene nerve catheter was placed under direct ultrasound-guidance utilizing a posterior in-plane approach. On POD 2, the catheter was removed. Three weeks postoperatively, the patient reported worsening dyspnea with a subsequent chest X-ray demonstrating an elevated right hemidiaphragm. Pulmonary function testing revealed worsening deficit from presurgical values consistent with phrenic nerve palsy. The patient decided to continue conservative management and declined fur

The effect of the sympathetic and sensory nervous system on active eustachian tube function in the rat.

CONCLUSION: We propose that simultaneous activation of the sensory and sympathetic nervous system may adversely affect eustachian tube function, which may have a role in the genesis of Ménière's disease. OBJECTIVE: We have determined the distribution of sympathetic and nociceptive sensory axons in the mucosa of the rat eustachian tube and investigated whether sympathetic or nociceptive neurons influence the function of the eustachian tube. MATERIALS AND METHODS: We tested whether the ability of a rat to equalize air pressure in the middle ear during evoked swallowing was altered by activatio

The Double-Deck Cervical Paraspinal Interfascial Block: Combined Cervical Cervicis Plane and Lower Cervical Erector Spinae Plane Blocks for C1-C2 Fusion Under SSEP/MEP Monitoring-A Case Report.

Analgesia for craniovertebral junction (CVJ) and high cervical posterior surgery is challenging because surgical exposure disrupts multilevel posterior cervical musculature and irritates the upper cervical dorsal rami, particularly C2-C3. Procedures performed with somatosensory evoked potential (SSEP) and motor evoked potential (MEP) monitoring also favor posterior, motor-sparing regional techniques. We describe a "double-deck" strategy combining an upper cervical cervicis plane (CCeP) block with a lower cervical erector spinae plane (ESP) block for posterior C1-C2 fusion performed under multi

The effect of bupivacaine on postoperative pain following thyroidectomy: a systematic review and meta-analysis.

INTRODUCTION: Thyroid surgery, which is usually followed by moderate postoperative pain, has gained increasing attention in recent years. A systematic review and meta-analysis was conducted to assess the effect of prophylactic bupivacaine on postoperative pain following thyroidectomy. EVIDENCE ACQUISITION: We searched the PubMed, Web of Science, Embase, and Cochrane Library databases for specific keywords. RevMan 5.0 and Stata 12.0 software were used to perform meta-analyses. The endpoints were postoperative pain, rescue analgesic requirement, and postoperative nausea and vomiting (PONV) duri

Ultrasound-guided bilateral superficial cervical block and preemptive single-dose oral tizanidine for post-thyroidectomy pain: a randomized-controlled double-blind study.

PURPOSES: The postoperative analgesic effect of tizanidine has not yet been evaluated sufficiently. The role of bilateral superficial cervical plexus block (BSCPB) for postoperative analgesia after thyroidectomy remains questionable. We aimed to evaluate the analgesic effect of combined use of BSCPB and a single-dose oral tizanidine in patients undergoing elective thyroid surgery. METHODS: Sixty patients undergoing thyroidectomy were randomized into 3 groups. The control group (Group C, n = 20) received BSCPB with 0.9% saline plus oral placebo. The superficial cervical group (Group SC, n = 20

[POSTOPERATIVE CONTROLLED ANALGESIA IN PATIENTS WITH HEAD AND NECK CANCER].

Acute postoperative pain is still an urgent problem in surgery. Today there are many drugs for pain relief, but at the same time, the adequacy of postoperative analgesia for the subjective assessments of patients does not exceed 50%. Therefore, taking into account the pathophysiology of acute pain syndrome, it is necessary to inject an anesthetic directly near the operating field, which can be achieved by long-term conduction anesthesia. So, in the area of the head and neck, there are several techniques (G. Brown, J. Bershet, V. M. Uvarov, J. S. Vaisblat, V. A. Dubov and V. D. Dunaevsky, P. Yu

Motor evoked potentials for femoral nerve protection in transpsoas lateral access surgery of the spine.

Detecting potential intraoperative injuries to the femoral nerve should be the main goal of neuromonitoring of lateral lumber interbody fusion (LLIF) procedures. We propose a theory and technique to utilize motor evoked potentials (MEPs) to protect the femoral nerve (a peripheral nerve), which is at risk in LLIF procedures. MEPs have been advocated and widely used for monitoring spinal cord function during surgical correction of spinal deformity and surgery of the cervical and thoracic spine, but have had limited acceptance for use in lumbar procedures. This is due to the theoretical possibili

[Study of the proximal conduction of the arms using SEP and MEP].

The proximal conduction of the upper limb has been studied with the somatosensory evoked potentials (SEP) and motor evoked potentials (MEP) in 40 controls which provided normal data. The SEP allowed the sensory conduction study of the Erb/C7 segment with N9/N13 delay determination after the median nerve stimulation at the wrist, and after the ulnar nerve stimulation at the wrist (20 cases) or above the elbow (20 cases). The N9/N13 delay was 3.24 ms +/- 0.35 for the median nerve, 3.30 ms +/- 0.52 for the ulnar nerve after stimulation at the wrist and 3.30 ms +/- 0.40 after stimulation at the el

Parasympathetic neurons in the cranial medial ventricular fat pad on the dog heart selectively decrease ventricular contractility.

We hypothesized that selective control of ventricular contractility might be mediated by postganglionic parasympathetic neurons in the cranial medial ventricular (CMV) ganglion plexus located in a fat pad at the base of the aorta. Sinus rate, atrioventricular (AV) conduction (ventricular rate during atrial pacing), and left ventricular contractile force (LV dP/dt during right ventricular pacing) were measured in eight chloralose-anesthetized dogs both before and during bilateral cervical vagus stimulation (20-30 V, 0.5 ms pulses, 15-20 Hz). Seven of these dogs were tested under beta-adrenergic

[Cervical epidural anesthesia].

Cervical epidural anaesthesia (CEA) results in an effective sensory blockade of the superficial cervical (C1/C4) and brachial plexus (C5/T1-T2). It is used both intraoperatively and in the treatment of postoperative or chronic pain. The approach to the epidural space at the C7-T1 interspace is not technically difficult. Patients are placed in the sitting position, increasing the negative pressure in the epidural space, with the head flexed on the thorax, in order to open the lowest cervical interspace. A 18-gauge Tuohy needle is inserted by a midline approach into the C6-C7 or C7-T1 interspace

Treatment of iatrogenic brachial plexopathy persisting for 3 years due to cervical foraminotomy using Korean medicine: A case report (CARE-compliant).

BACKGROUND: Extrinsic or iatrogenic brachial plexopathy often resolves spontaneously within six months. Although steroid therapy and rehabilitation interventions are employed in the management of brachial plexopathy, their efficacy remains limited with insufficient supporting evidence. Moreover, therapeutic options to improve symptoms after the natural recovery period are limited. CASE PRESENTATION: A 49-year-old Asian woman was admitted with a three-year history of right upper extremity weakness and pain. The patient was diagnosed with sequelae of iatrogenic right brachial plexus injury due

The numb arm and hand.

Trauma and compression along the course of the median, ulnar or radial nerve from the brachial plexus to the fingers may cause pain, weakness, numbness or tingling of the upper extremity. Diabetes, smoking, alcohol consumption, rheumatoid arthritis and hypothyroidism are risk factors for nerve entrapment, although these disorders typically produce bilateral symptoms. Carpal tunnel syndrome, the most common nerve entrapment condition, results from median nerve compression at the wrist. The diagnosis is suggested by decreased pain sensation and numbness in the thumb and index and middle fingers;

Dexmedetomidine during carotid endarterectomy: a registry on safety and satisfaction of patients and operators (DexTEA).

BACKGROUND: From an anesthesiologic perspective, carotid endarterectomy (CEA) is an operation that is increasingly being performed with locoregional techniques and sedation. The aim of this observational study was to compare dexmedetomidine with midazolam and fentanyl during CEA in terms of perioperative adverse events, patient, anesthetist, and surgeon satisfaction, as well as the possible reduction in clamping time and the number of times additional local anesthesia is needed. METHODS: Sixty patients listed for CEA were enrolled and two types of sedation were used, both protocols widely use

Awake patient monitoring to determine the need for shunting during carotid endarterectomy.

BACKGROUND: The indications for shunt placement to prevent cerebral ischemia during carotid endarterectomy have been controversial. Some investigators have recommended empiric shunting for patients presumed to be at higher risk for cerebral ischemia with a recent stroke or severe stenosis or occlusion of the contralateral internal carotid artery. METHODS: Carotid endarterectomy was performed in 81 cases with cervical block anesthetic, monitoring the awake patient for the development of cerebral ischemia (unresponsiveness or paralysis) during carotid clamping. The need for shunting (based on a

Anatomical consideration of the occipital cutaneous nerves and artery for the safe treatment of occipital neuralgia.

There is no standardized approach to the greater occipital nerve (GON) block technique for treating occipital neuralgia. The aim of the present study was to validate the previously-suggested guidelines for conventional injection techniques and to provide navigational guidelines for safe GON block. The GON, lesser occipital nerve (LON) and occipital artery (OA) were carefully dissected in the occipital region of embalmed cadavers. Using a 3 D digitizer, the GON, LON, and OA were observed on the two reference lines. The distances between the landmarks were recorded and statistically analyzed. On

[Percutaneous cervical cordotomy in the therapy of incidence pain].

BACKGROUND: Incident pain does not respond to opioid treatment and it is not easily relieved with other therapeutic strategies (local intrapleural or spinal analgesia, phenol blocks etc.). For this reason cervical percutaneous cordotomy at C(1)-C(2) interspace is the only effective antalgic therapy in patients whose life expectancy is more than three to six months. METHODS: This study is a rectrospective review of 22 patients with cancer and incident pain from brachial, lumbar-sacral plexus injury and gluteal ulcer. RESULTS: Cordotomy provided excellent contralateral side pain relief in 21 p

Subclavian-to-internal carotid artery bypass with carotid endarterectomy for total occlusion of the common carotid artery under awake neurologic monitoring: a case series.

BACKGROUND: Total occlusion of the common carotid artery (CCA) from its origin is rare, occurring in less than 1% of brachiocephalic stenoses. Surgical revascularization is technically demanding, especially with tandem severe ipsilateral internal carotid artery (ICA) stenosis. Procedures under regional anesthesia with continuous neurologic monitoring are rarely described. OBJECTIVE: To evaluate outcomes of subclavian-to-internal carotid artery bypass with carotid endarterectomy (CEA) in symptomatic patients with total CCA occlusion and severe ipsilateral ICA stenosis under regional anesthesia

The impact of the pore position of a self-coiling catheter for continuous interscalene brachial plexus block on postoperative pain in patients undergoing arthroscopic rotator cuff repair surgery: a prospective observational study.

This prospective observational study aimed to investigate the usefulness of the self-coiling catheter for continuous interscalene brachial plexus block (CISB) in patients undergoing arthroscopic rotator cuff repair (ARCR). In 22 patients, the self-coiling tip of catheter was placed anterior to the C5 and C6 roots under ultrasound guidance. The primary outcome is the relationship between the distance from the distal side pore of the catheter to the C5/C6 nerve roots and the visual analogue scale (VAS) of postoperative pain. The distance was recorded as a positive value when the distal pore was

Utility of Preoperative Electrodiagnosis Together with Peripheral Nerve High-Resolution Ultrasound: A Complex Case Report of Neurofibromatosis Type I.

Our case report underscores the importance of electroneuromyography (ENMG) combined with peripheral nerve high-resolution ultrasound (HRUS) in the evaluation of neurofibromatosis type 1 (NF1). A 49-year-old woman affected by NF1 came to our attention because of new-onset left arm weakness and atrophy. Debulking of a cervicothoracic C7-T1 neurofibroma had been performed 8 years earlier. On current admission, magnetic resonance imaging disclosed increased lesion volume that was thought to cause the neurologic deficits by compressing the C8 root. Findings from intraoperative neurophysiologic moni
