The State of Surrogacy in New York: A New National Prototype, New Patrons, New Perils?

Four decades after the Baby M case that led to the prohibition of commercial surrogacy in New York, much has changed in the infertility industry. Advanced technologies including the advent of gestational carrier pregnancies had made it easier and more efficient to create IVF embryos at a distance and over time, accelerating a boom in cross-border, reproductive services and allowing compensated surrogacy to flourish in a growing number of surrogacy-friendly states and beyond. For international couples, the USA has become a hot spot for "circumvention tourism" given its first-rate medical care,

Patients of doctors further from medical school graduation have poorer outcomes.

CONTEXT: There is an apparent contradiction between the findings of studies indicating that patient outcomes are better when physicians have a greater volume of practice and those that find outcomes to be worse with increased time since training, which implies greater volume. OBJECTIVES: This study was designed to estimate the adjusted relationships between physicians' characteristics, including recent practice volume and time since medical school graduation, and patient outcomes. METHODS: This is a retrospective observational study based on all Pennsylvania hospitalisations over 7 years for

Contradictions and dilemmas within the practice of immigration medicine.

OBJECTIVES: To identify, explore and critique features of how practices associated with immigration medicine are socially organized. Specifically, how the work of designated medical practitioners (DMP) - physicians who conduct immigration medical examinations of prospective immigrants to Canada as contractors to the Canadian government department of Citizenship and Immigration Canada - is organized to occur in interactions with applicants who are diagnosed with the human immunodeficiency virus during the immigration medical examination. METHODS: Findings from a theoretically informed empirica

The potential of the internet for alternative caring practices for health.

The practices of health care in late modernity are informed by competing visions of the ideal human and the nature of care. Western societies typically characterise the ideal human as independent and self-reliant. The resultant welfare systems provide temporary havens away from the everyday, competitive spaces of capitalist societies, termed here the enclave model. Social scientists problematise this model on several grounds: the construction of pathologised and medicalised body forms; the neglect of caring practices that are gendered, invisible and primarily private; the de-politicisation of

From rugby to basketball: a comparative analysis on the implementation of mixed ability.

INTRODUCTION: People with disabilities continued to face numerous barriers to participation in mainstream sports clubs. In this regard, the Mixed Ability (MA) model emerged as a response to the limited availability of inclusive opportunities that enabled involvement in these contexts and, consequently, the exercise of individual rights. Therefore, the aim of this study was to analyse the application of the MA model in basketball and to compare it with rugby, a discipline with a more consolidated trajectory in this field. Furthermore, it sought to assess how participation influenced the percept

Paul Kaznelson, Immune Thrombocytopenic Purpura, and Splenectomy: The Tragic Story of a Medical Prodigy.

In 1916 Paul Kaznelson (1892-1959), a 26-year-old medical student at German University in Prague, was the first to describe splenectomy for what would be recognized today as immune thrombocytopenic purpura (ITP). From his physical examination, microscopy of a blood smear, and what was then known about the life cycle of platelets, he concluded that his patient's bleeding diathesis and enlarged spleen were linked: she had no platelets because they were being destroyed by the organ. He convinced the chief of surgery to remove the patient's spleen, a landmark operation that succeeded beyond "the w

Health inequities and social determinants of health in refugee and immigrant communities.

This article evaluates and elucidates the intersections across social and economic determinants of health and social structures that maintain current inequities and structural violence with a focus on the impact on imMigrants (immigrants and migrants), refugees, and those who remain invisible (e.g., people without immigration status who reside in the United States) from Black, Indigenous, and People of Color communities. Psychology has a history of treating individuals and families without adequately considering how trauma is cyclically and generationally maintained by structural violence, ine

A qualitative assessment of structural barriers to prenatal care and congenital syphilis prevention in Kern County, California.

Congenital syphilis is the result of placental transmission from mother to fetus of Treponema pallidum. Although congenital syphilis is preventable through timely treatment, the rate of new infections in the United States (US) has increased each year since 2013, and is increasing at a noticeably greater pace in California (CA). Most research into congenital syphilis has focused on individual psychosocial and behavioral factors that contribute to maternal vulnerability for syphilis. The aim of this study was to evaluate structural barriers to prenatal care access and utilization and congenital

Alfred Nobel and His Prizes: From Dynamite to DNA.

Alfred Nobel was one of the most successful chemists, inventors, entrepreneurs, and businessmen of the late nineteenth century. In a decision later in life, he rewrote his will to leave virtually all his fortune to establish prizes for persons of any nationality who made the most compelling achievement for the benefit of mankind in the fields of chemistry, physics, physiology or medicine, literature, and peace among nations. The prizes were first awarded in 1901, five years after his death. In considering his choice of prizes, it may be pertinent that he used the principles of chemistry and ph

Medical education for "Dreamers": Barriers and opportunities for undocumented immigrants.

Medical schools should amend their admissions policies to welcome applications from qualified undocumented immigrants, often called "Dreamers." The recent creation of the Deferred Action for Childhood Arrivals (DACA) program of the U.S. Citizenship and Immigration Service removes the key obstacles to securing a license and residency eligibility for such medical school graduates. As a result, to deny application to Dreamers of DACA status represents a kind of unjustified discrimination and violates the basic ethical principle of the equality of human beings. In addition, the medical profession'

Measles, mumps, rubella, and varicella seroprevalence in health care workers: A cross-sectional study from Tan Tock Seng Hospital and the National Centre for Infectious Diseases, Singapore.

OBJECTIVES: We assessed measles, mumps, rubella (MMR) and varicella seroprevalence and factors associated with seropositivity among our institutions' health care workers (HCWs). METHODS: This cross-sectional enzyme-linked immunosorbent assay (ELISA) seroprevalence study of HCWs' residual sera from 2020 assessed factors associated with seropositivity using logistic regression and tested performance characteristics of an "immunity" criterion (composite of past vaccination and serology results) against seropositivity. Measles non-seropositive sera were re-tested with plaque neutralization reduct

Substantial variation in the acceptance of medically complex live kidney donors across US renal transplant centers.

Concern exists about accepting live kidney donation from 'medically complex donors'--those with risk factors for future kidney disease. This study's aim was to examine variation in complex kidney donor use across US transplant centers. We conducted a retrospective cohort study of live kidney donors using organ procurement and transplantation network data. Donors with hypertension, obesity or estimated glomerular filtration rate (eGFR) <60 mL/min/1.73 m(2) were considered medically complex. Among 9319 donors, 2254 (24.2%) were complex: 1194 (12.8%) were obese, 956 (10.3%) hypertensive and 392 (
