Muencke syndrome with cleft lip and palate.

Muencke syndrome results from mutations in the FGFR3 gene, and although it is well recognized that the clinical presentation is variable, the important key finding includes coronal synostosis. We present a family where a mother with proven FGFR3 Pro250Arg mutation gave birth to identical twins both of whom had craniosynostosis but had coexisting bilateral cleft lip and palate. We believe that this is the first description of clefting occurring in conjunction with Muencke syndrome and so further extends the range of phenotypic variation that can occur in this syndrome.

Surgical correction of cleft lip and palate.

Surgical cleft repair aims to restore function of the oro-nasal sphincter and oro-nasal soft tissues and re-establish the complex relationship between perioral and perinasal muscle rings without compromising subsequent mid-facial growth and development. Here we review the surgical anatomy of this region, optimal timing for surgical repair and current thinking on the use of surgical adjuncts. In addition, an overview of current surgical techniques available for the repair of cleft lip, cleft palate and velopharyngeal insufficiency is presented. Finally, we briefly discuss nasal revision surgery

Cleft lip and palate care in Romania.

Vanessa Martin travelled with a surgical team to Romania over a period of 12 years to support nurses and improve practice in the treatment of cleft lip and palate. She recounts her experiences and the changes that took place during that time.

Modification of the repair of a unilateral cleft lip.

The ideally repaired cleft lip should provide a symmetrical Cupid's bow, philtrum, and minimal scar. In the appearance of the upper lip, the philtrum plays a key role. The most popular method for unilateral cleft lip repair is the rotation advancement technique introduced by Millard. This technique requires the rotation of the non-cleft-side flap in a unilateral cleft lip. As the vertical discrepancy between the peaks of Cupid's bow is increased, the scarring becomes more evident. Also, where it crosses the philtral column in the oblique extension of the upper lip, it becomes apparent for the

Gastroschisis associated with cleft lip and palate.

Gastroschisis is a common congenital abdominal wall defect. Rarely, it is associated with extraabdominal midline congenital anomalies. Oral clefts are the commonest craniofacial congenital midline defects. They can be associated with other midline defects like omphalocele. We believe these are the first 3 case reports of gastroschisis and cleft palate occurring in the same patient.

[Cleft lip and palate surgery in India].

10 cleft lip and palate temporary facilities were set up 1994-2003 in a Christian Hospital in Central India. 725 patients were operated upon. An analysis of the operations in regard to lip closure, closure of the soft and the hard palate, secondary cleft operations and cleft-related surgery is given. Differing therapy concepts in developed countries are discussed and compared to experiences of other third world volunteer medical groups.

Secondary lip correction in unilateral clefts.

Although all surgeons try to achieve the best possible result in the primary operation of a cleft, a certain percentage of cases might still need secondary corrections. The causes include not only lack of experience of the surgeon, but also the reaction of a child with its individual scar formation, disturbance in the healing of the wound, or the growth of the face, any of which might give an unfavorable result. In secondary operations we must distinguish between minor and major problems. Sometimes different parts of the cleft area will need correction and it is difficult to separate the probl

Cleft lip and palate: a review for dentists.

The goals of primary closure of cleft lip and palate include not only re-establishing normal insertions for all of the nasolabial muscles but also restoring the normal position of all the other soft tissues, including the mucocutaneous elements. Conventional surgical wisdom, which recommends waiting until growth is complete before undertaking surgical correction of the postoperative sequelae of primary cheiloplasty, carries with it many disadvantages. If, after primary surgery of the lip, orolabial dysfunctions remain, they will exert their nefarious influences during growth and will themselve

[Associated anomalies in lip-maxillopalatal clefts].

BACKGROUND: Retrospective studies of cleft lip and palate patients suggest a multifactorial aetiology for this condition. Many patients exhibit multiple defects, often removed from the orofacial region. The frequency and location of such coexistent abnormalities vary between studies. PATIENTS: A retrospective case-note study of 1,737 individuals with orofacial cleft, treated between 1974 and 1998 at our centre, was undertaken to assess the frequency of associated malformations and syndromes. RESULTS: Associated malformations were found to be present in 33% of all cases investigated. In nearl

A model for the cleft lip nasal deformity.

The underlying pathology of the cleft lip nasal deformity has yet to be fully realized, and cleft lip rhinoplasty continues to challenge the reconstructive surgeon. A new model is proposed, which is composed of elements that represent known anatomical structures of the nose. These structures are considered elemental to the mechanism of the primary cleft lip nasal deformity. The lobule is reduced to four arches. Five points on the skull provide foundations for these arches, which react interdependently to extrinsic forces and positional change. When certain changes are imposed on the model, pre

[The child with lip, maxillary, palatal cleft].

Clefting of lip, alveolus and palate may occur in multiple variations. It causes aesthetic and functional detractions. Soft palate clefts may result in hearing-, speech- and swallowing-disorders. Therefore the otolaryngologist is a very important member in the interdisciplinary team directory. A cleft-palate child belongs to an interdisciplinary consulting hour in special hospitals, where different medical specialties are involved. Most important for a sufficient medical rehabilitation are maxillofacial surgery, otolaryngology, paediatrics, plastic surgery, speech therapy, psychology and human

Prospectives in cleft lip and palate repair.

A recent focus on fetal surgery has raised the possibility of fetal cleft lip and palate repairs. Because of the fetus's marked plasticity, fetal repairs may have results that more closely approximate normality. Furthermore, the impact on cost management could be dramatic with a decreased need for extensive postoperative care, orthodontia, and speech therapy. This article considers the future prospects of intrauterine cleft lip repair as the next generation of surgical advances.

The management of the cleft lip and palate patient.

We believe that early closure of the hard palate defect with a palatal appliance and surgical closure of the lip and soft palate according to the rule of tens are very important. Delaying the hard palate closure allows for maximal growth of the palatal shelves. We also feel that pressure equalizing tubes placed in the ears at the initial surgical procedure allows for more normal function of the middle ear, less middle ear infections, better hearing, and ultimately better speech. We feel that early closure of the soft palate may decrease velopharyngeal incompetence by normalizing the position o

Congenital median clefts of the upper lip.

This is a report of 12 cases of congenital median cleft deformity of the upper lip seen in the Department of Plastic Surgery, Singapore General Hospital. This deformity is extremely rare and there are probably not more than 100 cases reported in the literature. The classification, nomenclature, pathogenesis and embryological development of median clefts are still not clearly understood. An analysis of the 12 cases is made with the view to clarify the nomenclature and classification of median clefts. An attempt is also made to explain the embryogenesis of this deformity and its common associati
