Fitz-Hugh-Curtis syndrome in adolescent and young adult females: Utility of a decision rule.

BACKGROUND: The diagnosis of Fitz-Hugh-Curtis syndrome (FHC) is often missed or delayed in patients with right upper quadrant pain (RUQ). OBJECTIVE: To develop a decision rule that predicts FHC in females with RUQ pain based on a constellation of historical features, physical examination findings and laboratory results. METHODS: We conducted a prospective study to test the utility of our FHC decision rule in sexually active females, aged 13-20 years, with RUQ pain who were seen in an urban ED over 57 months. The decision rule was based on 4 features: 1. Presence of pleuritic chest pain, 2. T

A multimodal intervention reduces computed tomography use in blunt head injury.

BACKGROUND: Computed tomography (CT) is frequently used in emergency departments (EDs) for the evaluation of blunt head injury. Unnecessary CT utilization leads to excessive cost, radiation exposure, and operational inefficiencies. Despite validated clinical decision rules, head CT utilization continues to rise, and the need remains for effective implementation strategies. METHODS: We performed a retrospective pre-post analysis after implementing a multi-modal intervention at three EDs within an academic health system. The intervention consisted of a guideline supporting a choice between the

Exploring Paramedics' Perspectives on Paediatric Head Injury in Prehospital Care: Qualitative Study.

BACKGROUND: Paediatric head injury is a common reason for emergency calls. While most cases are mild, a small proportion deteriorates rapidly. Paramedics are often the first point of clinical contact, yet the absence of paediatric-specific tools, infrequent exposure and emotionally charged environments contributes to uncertainty. Paramedic perspectives in this context remain under-represented in the literature. OBJECTIVE: To explore paramedics' experiences, challenges and decision-making in the prehospital assessment of children with suspected head injuries and explore perceptions of existing

Prevalence of key bacterial pathogens among children with pharyngotonsillitis in The Gambia - a case-control study.

OBJECTIVES: Causes of pharyngotonsillitis in Africa, beyond Streptococcus pyogenes, are poorly understood. We aimed to describe the prevalence of S. pyogenes, Fusobacterium necrophorum, and Streptococcus dysgalacatiae subspecies equisimilis (SDSE) in children in The Gambia and evaluate the usefulness of clinical decision rules, originally developed for S. pyogenes or beta-haemolytic streptococci, for their prediction. METHODS: We conducted a case-control study on causes of pharyngotonsillitis in children (0-15 years). Samples from age-matched cases and asymptomatic controls were included from

Diagnostic management of suspected acute pulmonary embolism in the postpartum period.

BACKGROUND: Pregnancy and the postpartum period increase the risk of venous thromboembolism including pulmonary embolism (PE), which is an important cause of maternal morbidity and mortality. While diagnostic strategies for suspected PE in pregnancy have evolved, postpartum patients remain underrepresented in research. OBJECTIVES: This study evaluated diagnostic strategies and the incidence of PE among postpartum patients presenting to an acute care setting with suspected PE, using data from a previously published individual patient data meta-analysis. METHODS: We analyzed 201 postpartum pat

Simple clinical decision model for predicting leptospirosis: a secondary analysis of a randomized controlled trial.

BACKGROUND: Leptospirosis is a major cause of febrile illness in tropical regions, but early diagnosis is challenging due to overlapping clinical features and delayed serological confirmation. We developed a simple bedside decision model using readily available clinical variables. METHODS: This secondary analysis used archived data from a randomized controlled trial of adults with undifferentiated febrile illness (October 2020-February 2021) of 5-15 days' duration. Independent predictors of IgM-ELISA positive leptospirosis were identified using multivariable logistic regression. A simplified

Compelling new electrocardiographic markers for automatic diagnosis.

BACKGROUND AND OBJECTIVE: The automatic diagnosis of heart diseases from the electrocardiogram (ECG) signal is crucial in clinical decision-making. However, the use of computer-based decision rules in clinical practice is still deficient, mainly due to their complexity and a lack of medical interpretation. The objetive of this research is to address these issues by providing valuable diagnostic rules that can be easily implemented in clinical practice. In this research, efficient diagnostic rules friendly in clinical practice are provided. METHODS: In this paper, interesting parameters obtain

Application of an evidence-based decision rule to patients with suspected pulmonary embolism.

RATIONALE: To support doctors in diagnosing patients who are suspected to have pulmonary embolism, the Christopher evidence-based decision rule was implemented in hospitals in the Netherlands. This study examines whether the Christopher evidence-based decision rule is applied in clinical practice. In addition, doctors' considerations for not applying the decision rule are explored. METHOD: Dyspnoea patients were included in the study prospectively. The diagnostic process of the patients with suspected pulmonary embolism, as judged by the treating doctor, was compared with the Christopher evid

Performance of Vibration-Controlled Transient Elastography and Clinical Prediction Models In Liver Transplant Recipients.

Hepatic fibrosis is a strong predictor of clinical outcomes following liver transplantation (LT).1 Despite the centrality of hepatic fibrosis in clinical outcomes, the published literature with noninvasive fibrosis assessment in LT recipients is limited and liver biopsy, despite its invasive nature, remains the reference standard. Vibration-controlled transient elastography (VCTE) and clinical prediction models (CPM) are point-of-care tests that can provide noninvasive assessment of hepatic fibrosis2-4; however, the data comparing the diagnostic performance of VCTE and CPM in LT recipients are

Serum GP73 - An Additional Biochemical Marker for Liver Inflammation in Chronic HBV Infected Patients with Normal or Slightly Raised ALT.

This study aimed to assess the feasibility of GP73 as a diagnostic marker for liver inflammation and fibrosis in chronic HBV patients with normal or slightly raised ALT (<2 ULN) and to develop models based on GP73 and other biochemical parameters to improve diagnostic accuracy. Serum GP73 levels were analyzed in 220 chronic HBV patients with normal or slightly raised ALT who underwent liver biopsy. The results showed that the area under the receiver operating characteristic (ROC) curve (AUC) was 0.806 for predicting significant liver inflammation (≥G2), while it was 0.742 for predicting signif

Early Prediction of Severe COVID-19 in Patients by a Novel Immune-Related Predictive Model.

During the progression of coronavirus disease 2019 (COVID-19), immune response and inflammation reactions are dynamic events that develop rapidly and are associated with the severity of disease. Here, we aimed to develop a predictive model based on the immune and inflammatory response to discriminate patients with severe COVID-19. COVID-19 patients were enrolled, and their demographic and immune inflammatory reaction indicators were collected and analyzed. Logistic regression analysis was performed to identify the independent predictors, which were further used to construct a predictive model.

ABCD2 risk score does not predict the presence of cerebral microemboli in patients with hyper-acute symptomatic critical carotid artery stenosis.

INTRODUCTION: ABCD2 risk score and cerebral microemboli detected by transcranial Doppler (TCD) have been separately shown to the predict risk of recurrent acute stroke. We studied whether ABCD2 risk score predicts cerebral microemboli in patients with hyper-acute symptomatic carotid artery stenosis. PARTICIPANTS AND METHODS: We studied 206 patients presenting within 2 weeks of transient ischaemic attack or minor stroke and found to have critical carotid artery stenosis (≥50%). 86 patients (age 70±1 (SEM: years), 58 men, 83 Caucasian) had evidence of microemboli; 72 (84%) of these underwent ca

A 20-year perspective on the Ottawa Ankle Rules: Are we still on solid footing?

The Ottawa Ankle Rules, finalized in 1995, are a clinical decision directive for the use of radiographs in patients with traumatic ankle and foot injuries. The researchers who developed the rules defined clinically insignificant ankle fractures as distal fibular or malleolar chip fractures of 3 mm or less. They concluded that at least one-third of all ankle and foot radiographs could be eliminated by using their rules, which have since been adopted as the standard of care in EDs. This article explores whether, after more than 20 years in use, the Ottawa Ankle Rules are still valid, and whether

Risk of Traumatic Brain Injuries in Infants Younger than 3 Months With Minor Blunt Head Trauma.

STUDY OBJECTIVE: Infants with head trauma often have subtle findings suggestive of traumatic brain injury. Prediction rules for traumatic brain injury among children with minor head trauma have not been specifically evaluated in infants younger than 3 months old. We aimed to determine the risk of clinically important traumatic brain injuries, traumatic brain injuries on computed tomography (CT) images, and skull fractures in infants younger than 3 months of age who did and did not meet the age-specific Pediatric Emergency Care Applied Research Network (PECARN) low-risk criteria for children wi
