Low-dose ticagrelor combined with aspirin for preventing early neurological deterioration in patients with high-risk non-disabling ischemic cerebrovascular events: a multicenter, prospective, randomized, open-label, clinical trial.

BACKGROUND: Early neurological deterioration (END) is associated with a poor prognosis in patients with high‑risk non‑disabling ischemic cerebrovascular events (HR‑NICE). Clopidogrel-based dual antiplatelet therapy is substantially limited by the CYP2C19 gene polymorphism. This study aimed to investigate whether low‑dose ticagrelor (60 mg twice daily) plus aspirin dual antiplatelet therapy was superior to clopidogrel plus aspirin for preventing END within 7 days in HR-NICE patients, without increasing the bleeding risk. METHODS: This was a multicenter, prospective, randomized, open-label, bli

Insulinoma in Type 2 Diabetes Mellitus Becoming Clinically Evident in the Setting of Acute Kidney Injury: A Case with a Postoperative Deterioration of the Glucose Metabolism and a Quantitative Islet Cell Analysis.

Insulinoma rarely coexists with type 2 diabetes mellitus and it may thus remain clinically unrecognized. We describe a case of a 73-year-old man with long-standing type 2 diabetes mellitus in whom recurrent hypoglycemia became evident during progressive renal dysfunction after acute kidney injury. Following an aneurysm repair and distal pancreatectomy, hypoglycemia resolved, but his glucose metabolism deteriorated markedly. A quantitative analysis of non-tumorous pancreatic tissue revealed an intermediate β-cell area and a relatively increased α-cell area. A postoperative glucose deterioration

Clinical course of Coronavirus Disease-19 in patients with haematological malignancies is characterized by a longer time to respiratory deterioration compared to non-haematological ones: results from a case-control study.

BACKGROUND: We evaluated clinical features and risk factors for mortality in patients with haematological malignancies and COVID-19. METHODS: Retrospective, case-control (1:3) study in hospitalized patients with COVID-19. Cases were patients with haematological malignancies and COVID-19, controls had COVID-19 without haematological malignancies. Patients were matched for sex, age and time of hospitalization. RESULTS: Overall, 66 cases and 198 controls were included in the study. Cases had higher prior corticosteroid use, infection rates, thrombocytopenia and neutropenia and more likely recei

Enhancing Paediatric Deterioration Assessment Across Diverse Skin Tones: Insights and Future Directions.

BACKGROUND: Skin tone can affect clinical signs and device accuracy in paediatric anaemia and deterioration detection, creating risks of inequity. KEY ISSUES: Studies show disparities in diagnostic accuracy across different skin tones, supported by community-based evidence. RECOMMENDATIONS: (1) Build multicenter networks with standardised skin-tone data. (2) Validate devices across varied skin tones. (3) Train clinicians in dark-skin sign recognition. (4) Include equity metrics in paediatric quality indicators. CONCLUSION: Reducing skin-tone bias is essential for equitable paediatric care.

Martha's rule: applying a behaviour change framework to understand the potential of complementary roles of clinicians and patients in improving safety of patients deteriorating in hospital.

AIMS/BACKGROUND: Martha's rule stipulates the right of patients and their families to escalate care as a way to improve safety while in hospital. This article analyses the possible impact of the proposed policy through the lens of a behaviour change framework and explores new opportunities presented by the implementation of Martha's rule.. METHODS: A descriptive analysis was undertaken of interactions between patients, family, friends and clinicians during clinical deterioration in hospital. The capability-opportunity-motivation behaviour change framework was applied to understand reasons for

Predicting patient deterioration by nurse intuition: The development and validation of the nurse intuition patient deterioration scale.

INTRODUCTION: Early warning scores based on vital signs are used in hospitals to estimate patient deterioration and to initiate an adequate and timely response when necessary. These scores show acceptable performance in predicting patient outcomes. However, they tend to generate many false positives leading to an increased workload in clinical practice. Additionally, nurses feel a tension between the application of an early warning score and their own clinical judgement. Nurse intuition is often included as an extra call criterion next to an early warning score. It is therefore important to in

NEWS2 and the older person.

The National Early Warning Score (NEWS), published in 2012, made no specific adjustments for older people. The updated NEWS2 (2017) incorporated new confusion as a category for consciousness. In this article, we consider the role of NEWS2 in detection of acute clinical deterioration in older people and how the score may be used to inform care, highlighting the additional aspects, such as care escalation decisions, that may ensue. We consider the evidence of NEWS and NEWS2 in assessment of the older person in different settings, including the potential benefits and limitations for care home res

Does ossification of the posterior longitudinal ligament progress after laminoplasty? Radiographic and clinical evidence of ossification of the posterior longitudinal ligament lesion growth and the risk factors for late neurologic deterioration.

Ossification of the posterior longitudinal ligament (OPLL) is a disease process characterized by progressive growth and calcification resulting in spinal canal compromise and serious neurological sequelae in advanced cases. Historically, OPLL has most commonly been treated with posterior surgical decompression. Although this procedure indirectly decompresses the spinal cord, it does not address the offending pathological entity, and further growth of the lesion may result in delayed neurological deterioration. This fact is particularly relevant because a number of long-term studies have reveal

Predictors of early neurological deterioration in patients with intracerebral hemorrhage: a systematic review and meta-analysis.

BACKGROUND: Early neurological deterioration, a common complication in patients with intracerebral hemorrhage, is associated with poor outcomes. Despite the fact that the prevalence and predictors of early neurological impairment are widely addressed, few studies have consolidated these findings. This study aimed to systematically investigate the prevalence and predictors of early neurological deterioration. METHODS: The PubMed, Embase, Cochrane Library, CIHNAL, and Web of Science databases were systematically searched for relevant studies from the inception to December 2023. The data were ex

A global view of pulmonary hypertension.

Pulmonary hypertension is a substantial global health issue. All age groups are affected with rapidly growing importance in elderly people, particularly in countries with ageing populations. Present estimates suggest a pulmonary hypertension prevalence of about 1% of the global population, which increases up to 10% in individuals aged more than 65 years. In almost all parts of the world, left-sided heart and lung diseases have become the most frequent causes of pulmonary hypertension. About 80% of affected patients live in developing countries, where pulmonary hypertension is frequently associ

Deviations in continuously monitored electrodermal activity before severe clinical complications: a clinical prospective observational explorative cohort study.

Monitoring of high-risk patients in hospital wards is crucial in identifying and preventing clinical deterioration. Sympathetic nervous system activity measured continuously and non-invasively by Electrodermal activity (EDA) may relate to complications, but the clinical use remains untested. The aim of this study was to explore associations between deviations of EDA and subsequent serious adverse events (SAE). Patients admitted to general wards after major abdominal cancer surgery or with acute exacerbation of chronic obstructive pulmonary disease were continuously EDA-monitored for up to 5 da

Effect of Preoperative Home-Based Exercise Training on Quality of Life After Lung Cancer Surgery: A Multicenter Randomized Controlled Trial.

BACKGROUND: Preoperative exercise training is recommended for improvement of clinical outcomes after lung cancer (LC) surgery. However, its effectiveness in preventing postoperative decline in quality of life (QoL) remains unknown. This study investigated the effect of preoperative home-based exercise training (PHET) on QoL after LC surgery. METHODS: Patients awaiting LC resection were randomized to PHET or a control group (CG). The PHET program combined aerobic and resistance exercise, with weekly telephone supervision. Primary outcome was QoL-assessed with the European Organization for Rese

Towards a new definition of decompensated cirrhosis.

There is a universal agreement that the occurrence of clinical complications, such as ascites, hepatic encephalopathy, gastrointestinal bleeding, and jaundice mark the transition from the compensated to the decompensated stage of cirrhosis. Decompensation is associated with a substantial worsening of patient prognosis and is therefore considered the most important stratification variable for the risk of death. However, this classification is an oversimplification, as it does not discriminate between the prognostic subgroups that characterise the course of decompensation, which depends on the t

Causally-informed deep learning towards explainable and generalizable outcome prediction in critical care.

The rapid evolution of deep learning is revolutionizing critical care medicine by enabling highly accurate early warning systems. These systems are capable of predicting diverse clinical deteriorations, including acute kidney injury, myocardial infarction, and circulatory failure. While demonstrating strong predictive capabilities, conventional deep learning models face two fundamental limitations in clinical implementation: their opaque decision-making processes and limited generalizability across diverse patient populations and care environments. To address these challenges, we present a cau

Effect of Magnesium on Deterioration and Symptomatic Hemorrhagic Transformation in Cerebral Ischemia: An Ancillary Analysis of the FAST-MAG Trial.

BACKGROUND: Magnesium (Mg) is a neuroprotectant in preclinical models. Lower serum Mg levels have been associated with symptomatic hemorrhagic transformation (HT) in patients with ischemic stroke. Early treatment of acute ischemic stroke with Mg may reduce rates of symptomatic HT. METHODS: In this post hoc study of the Field Administration of Stroke Therapy Magnesium (FAST-MAG) trial, 1,245 participants with a diagnosis of cerebral ischemia received 20 g of Mg or placebo initiated in the prehospital setting. Posttreatment serum Mg level was measured for 809 participants. Cases of clinical det
