Clinical Reasoning: A 67-Year-Old Man With Exercise Intolerance and Progressive Weakness.

A 67-year-old man with long-standing HIV on antiretroviral therapy, type 2 diabetes with polyneuropathy, and hyperlipidemia on statin and fibrate therapy presented with difficulty walking and exercise intolerance. Examination revealed new hamstring weakness superimposed on chronic distal weakness and sensory loss. Initial laboratory testing showed elevated creatine kinase, which persisted despite discontinuation of statin and fibrate therapy. In this article, we review the approach to adult-onset myopathy and explore diagnostic challenges when clinical and diagnostic testing seem discordant, e

Clinical Reasoning: A 16-Year-Old Adolescent Boy With Ophthalmoplegia and Unilateral Ptosis.

Ophthalmoplegia with ptosis represents a heterogeneous group of disorders that pose significant diagnostic challenges. We report the case of a 16-year-old adolescent boy presenting with ophthalmoplegia and unilateral ptosis. This case underscores the critical role of comprehensive clinical evaluation and standardized therapeutic protocols in the diagnosis and management of this disorder. Readers will walk through the stepwise diagnostic approach to arrive at the leading diagnosis, with a review of the possible differential diagnoses for the relevant clinical manifestations.

Clinical Reasoning: A 59-Year-Old Man With Acute-Onset Encephalopathy and Aphasia.

Assessing patients with acute encephalopathy presents significant challenges because of the wide range of underlying causes. This complexity is further intensified when seizures are involved, making the selection of effective management even more critical. In this case, a 59-year-old man was evaluated at the emergency department for acute-onset confusion and aphasia, which later on was complicated by seizures. Stroke was initially ruled out, and valproate therapy was instituted. EEG showed slow background activity, CSF was normal, and brain MRI showed bilateral cortical T2 hyperintensities wit

Clinical Reasoning: A 59-Year-Old Female Patient With Urinary Dysfunction and Lightheadedness.

A 59-year-old female patient presented with urinary frequency and incontinence. The urodynamic study suggested stress urinary incontinence. Brain MRI scan revealed changes in both cerebral hemispheres and the cerebellar vermis. This case highlights the importance of a thorough medical history inquiry and comprehensive assessment of diseases affecting several systems.

Clinical Reasoning: A 66-Year-Old Female Patient With Rapidly Progressive Memory Changes.

An otherwise healthy 66-year-old female patient presented with 3 months of memory loss prompting evaluation for rapidly progressive dementia. A broad systemic evaluation for vascular, infectious, toxic metabolic, autoimmune/paraneoplastic, and neoplastic diseases was performed. Neuroimaging revealed symmetric bilateral fluid-attenuated inversion recovery hyperintensities of the thalami, paramedian frontal regions, and mesial temporal lobes without contrast enhancement on MRI. Lumbar puncture did not uncover an inflammatory profile. The patient was treated for presumed seronegative autoimmune e

Clinical Reasoning: A 64-Year-Old Man With Confusion, Nausea, Seizure, and Fever.

Evaluating patients with encephalitis is common, but it can be complicated in immunocompromised patients. In this case, a 64-year-old man with a medical history of multiple myeloma and previous Lyme disease presented with acute onset of confusion, nausea, and fever; developed generalized tonic-clonic seizures; and subsequently became comatose. This case highlights the importance of thinking broadly beyond common viral and bacterial encephalitides and developing a comprehensive differential diagnosis.

Clinical Reasoning: A 55-Year-Old Woman With Painless Hand Weakness and Atrophy.

Hand weakness is a frequent chief concern in neurology practice. We report a case of a 55-year-old woman presenting with a chronic, gradually worsening right hand weakness and atrophy, selectively affecting the thenar muscles, without any sensory symptoms. She had a history of carpal tunnel syndrome and previously underwent surgical carpal tunnel release. This case delves into the differential diagnosis of hand weakness and atrophy, emphasizing the significance of myotomal innervation in intrinsic hand muscles. Furthermore, it outlines a systematic approach to diagnosing an uncommon cause for

Clinical Reasoning: An Unusual Etiology of Lumbosacral Plexopathy in a Patient With HIV.

We present a compelling case of uncontrolled diabetes, who initially presented as diabetic lumbosacral radiculoplexus neuropathy (DLRPN), with radicular pain in the right lower limb (LL) followed by asymmetric weakness of both LLs (right greater than left) with wasting in the medial compartment of the right thigh and significant sensory loss in the bilateral sural and right saphenous nerve distribution. Electrophysiology was suggestive of right lumbosacral radiculoplexus neuropathy. Incidentally, the patient tested positive for HIV-1 at our tertiary care center. CSF analysis revealed markedly

Clinical Reasoning: A 35-Year-Old Woman With Personality Change and Gait Impairment.

A 35-year-old woman presented with a progressive 3-year history of personality changes and gait impairment. Neurologic examination revealed bilateral optic atrophy, spastic paraparesis, and impaired vibratory sensation in all limbs, and neuropsychological evaluation identified a frontotemporal cognitive impairment. In this article, we review the differential diagnosis for a young woman with chronic frontotemporal dysfunction, optic atrophy, and dorsolateral myelopathy in a stepwise multidisciplinary approach. Critical information obtained from the medical history led to a final diagnosis.

Clinical Reasoning: A 14-Year-Old Girl With Reversible Peripheral Neuropathy and Encephalopathy.

A 14-year-old girl presented with acute ascending, symmetric numbness, and flaccid paralysis 3 weeks after a suspected gastrointestinal infection. She had experienced anorexia since this gastrointestinal episode. EMG showed a sensorimotor axonal polyneuropathy. Routine CSF analysis and serum-specific antibodies (antiganglioside and node of Ranvier-associated antibodies) were all negative. Laboratory investigations for possible etiologies revealed only mild metabolic perturbations. During her hospitalization, she developed mild cognitive deficits. Brain MRI showed bilateral symmetric basal gang

Clinical Reasoning: A 22-Year-Old Man With Multifocal Brain and Osseous Lesions.

The evaluation of patients with disseminated processes with CNS and osseous involvement is often challenging. A 22-year-old healthy man developed left-sided weakness, paresthesias, and neck pain over several weeks. On clinical examination, he was noted to have decreased right eye visual acuity, left-sided pyramidal weakness and numbness, and bilateral hyperreflexia. MRI revealed multifocal widespread abnormalities: nonenhancing lesions throughout the infratentorial brain, pituitary gland, right frontal lobe, and optic nerves, in addition to an enhancing intramedullary cervical spinal cord lesi

Clinical Reasoning: A 75-Year-Old Man With Dementia, Incontinence, and Gait Dysfunction.

Idiopathic normal pressure hydrocephalus (iNPH) is a progressive neurological disorder characterized by gait apraxia, cognitive decline, and urinary incontinence. It can be difficult to diagnose iNPH as the symptoms may overlap with other neurodegenerative diseases including cervical spondylotic myelopathy. Cervical spondylotic myelopathy is a progressive degenerative disease in which compression of the cervical spinal cord causes gait disturbances and imbalance, loss of dexterity and strength in the hands, and, at late stages, urinary dysfunction. As with iNPH, increased age is associated wi
