Fractures of the proximal humerus in children are rare and constitute only 3% of all epiphyseal injuries. From 1992 to 2002 sixteen patients aged 4 - 15 years with a displaced fracture of the proximal humerus were treated at our level I trauma unit. The mean follow-up of the patients was 23.8 months (8 - 72). Ten children had a metaphyseal fracture and six a Salter and Harris Type II injury. Only one metaphyseal fracture was treated conservatively; the other patients underwent surgery (ORIF [= open reduction internal fixation] in ten patients, CRIF [= closed reduction internal fixation] in fiv

A subcapital femoral neck fracture in a healed intertrochanteric fracture treated by an open reduction and internal fixation is a rare, but catastrophic, event. We present the case of an 86-year-old woman, a community ambulator, who sustained a displaced right intertrochanteric hip fracture during a fall. She was treated with closed reduction and internal fixation with a dynamic compression hip screw and side plate. Four months later, she was noted to have a displaced subcapital femoral neck fracture and underwent hip screw and side plate hardware removal and cemented bipolar hemiarthroplasty.

Forearm fractures involving both bones are prevalent among pediatric patients. Elastic stable intramedullary nailing (ESIN) is an excellent option for forearm fractures that require surgical stabilization in children and adolescents. Proximal third fractures can be particularly challenging to reduce using closed techniques, and multiple unsuccessful attempts at nail insertion can increase the risk of compartment syndrome. In this article, we present a novel technique called Shanz-assisted closed reduction (SACR), which involves using a 4-mm Shanz to facilitate alignment and nail insertion whil

BACKGROUND: Mandibular fracture is the second most common fracture of facial bone, next to nasal bone. Twenty-five to forty percent of mandibular fractures involve the condyle. In the literature, there exists no consensus "gold standard" treatment for mandibular condylar fractures, and there is a continuing debate on whether condylar fractures should undergo closed or open reduction.
MATERIALS AND METHOD: Twenty patients who had undergone open reduction and closed reduction treatment were included in the study. Clinically maximal interincisal opening, laterotrusive and protrusive movements, p

Distal tibial fractures are challenging injuries because of limited soft-tissue coverage, frequent high-energy mechanisms and the potential for soft-tissue compromise. Extensive surgical exposure may further disturb the local soft-tissue and biological environment. Ilizarov circular external fixation permits stable fixation with limited exposure of the fracture site. This prospective observational study evaluated fracture union, bone and functional outcomes and complications following closed or indirect reduction and Ilizarov circular external fixation in patients with open and closed distal t

OBJECTIVE: To investigate whether closed suction drainage (CSD) is related to accelerated rehabilitation of patients after open reduction and internal fixation (ORIF) for closed distal femur fractures.
METHODS: This study was a prospective, randomized controlled clinical trial. Between October 2018 and June 2020, 160 closed distal femur fracture patients who were prepared for ORIF were prospectively randomized into two groups: a CSD group with the mean age of 57.91 ± 14.38 years (32 [40%] men and 48 [60%] women) and a non-CSD group with the mean age of 59.73 ± 17.55 years (27 [34%] men and 54

Delayed severe epistaxis after closed reduction of a nasal bone fracture is uncommon, and postoperative nasal synechia is rarely considered as a bleeding source. A 19-year-old woman developed recurrent severe epistaxis 5 weeks after closed reduction and intranasal packing. Bleeding recurred immediately after pack removal despite repeated packing. Emergency endoscopic exploration showed bilateral adhesions between the septum and inferior turbinate, with active bleeding directly visualized from right-sided synechial tissue. Suction coagulation and synechiolysis achieved hemostasis. Hemoglobin de

BACKGROUND: Supracondylar humerus fractures (SCH) are the most common type of elbow fracture in children, with many cases requiring surgery. Treatment of postoperative pain for SCH has high variability, but can often be treated effectively with minimal or no opioids. Furthermore, there is significant morbidity related to pediatric opioid consumption. The goal of this study was to characterize prescription patterns in the United States following closed reduction and percutaneous pinning (CRPP) of SCH.
METHODS: All patients aged 10 or younger years who underwent CRPP of SCH from January 2010 to

We present the case of a child who had a posterior-lateral elbow dislocation with an intact ulnar nerve documented before an attempted reduction, with subsequent loss of ulnar nerve function after the elbow was reduced.Dislocations of the elbow in children represent only 3% to 6% of all elbow injuries. Posterior elbow dislocations are the most common, with more than one half involving an associated fracture about the elbow. The most common associated fracture is the medial epicondyle. Even though it is known that injury to the ulnar nerve can occur in elbow dislocations, we found only 1 other

Flexor tendon entrapment after a pediatric forearm fracture is a rarely reported complication that is often diagnosed late. Flexor tendon entrapment is more frequently reported after distal forearm fractures, and possible etiologies include fibrosis secondary to hemorrhage at the fracture site and simple entrapment of the muscle belly. This article describes a case of ring finger flexor digitorum profundus entrapment in a 12-year-old boy with a closed both-bone forearm fracture that was treated with closed reduction and intramedullary nail fixation. Preoperatively, the patient had full flexion

Segmental fractures of the proximal humeral shaft are rare and challenging to manage, particularly in elderly osteoporotic patients with multiple co-morbidities. Traditional open fixation methods carry risks of soft tissue complications, neurovascular injury, and delayed recovery. Minimally invasive techniques such as Closed Reduction and Internal Fixation (CRIF) with Titanium Elastic Nailing System (TENS) offer an alternative, though their use in segmental proximal humeral fractures remains underreported. We report the case of a 72-year-old female with diabetes mellitus, hypertension, and cor

BACKGROUND: Pelvic ring disruption (PRD) is a serious trauma associated with high mortality and disability rates. Poor reduction can lead to complications such as pelvic deformity and delayed fracture healing. Here, we introduce a new technology using mixed reality surgical navigation (MRSN) with an unlocking closed reduction technique (UCRT) frame to assist pelvic fracture reduction and fixation.
METHODS: Thirty patients with PRD were enrolled in this study. All of the patients underwent preoperative CT scans, with the pelvis and tracker segmented into three-dimensional models. Under MRSN gu

OBJECTIVE: To evaluate the radiographic and functional outcomes of closed reduction and percutaneous fixation (CRPF) compared to open reduction and internal fixation (ORIF) in the treatment of isolated displaced medial malleolar fractures.
METHODS: This randomized controlled trial included 50 adult patients with isolated displaced medial malleolar fractures, Herscovici classification type B and C, across two hospitals. Participants were randomly assigned (1:1) to either CRPF (Group A, n = 25) or ORIF (Group B, n = 25). The principal objectives were radiological union and clinical function eva

The aim of this study was to compare closed percutaneous screw reduction to traditional open reduction-internal fixation (OR-IF) for the treatment of anterior table fractures. Both groups were evaluated in terms of operative variables, complications, and treatment success. Of 32 patients included, 19 patients underwent OR-IF, while 13 underwent percutaneous screw reduction. The median operative time, length of hospital stays, and treatment cost of the OR-IF group were 100 min (range 60-130), 4 days (range 3-9), and $727 (range $642-$1291), respectively. The same variables for the closed reduct

UNLABELLED: The best treatment plan for femoral neck fractures in children in whom treatment is delayed by more than 24 h is unclear because of the limited number of such cases reported. This study compares the outcomes of closed and open reduction performed 24 h after the injury was sustained. We retrospectively reviewed 58 cases (40 males, 18 females) of femoral neck fractures reset after more than a 24-h delay at our hospital (May 2005 to May 2014). According to Delbet's classification, there were 30 type II fractures (51.7%), 21 type III fractures (36.2%), and seven type IV fractures (12.1
