Impact of Clostridioides difficile Therapy on Nosocomial Acquisition of Vancomycin-Resistant Enterococci.

Vancomycin is frequently used for the treatment of C. difficile infections (CDI). There are concerns that this might increase the risk of selecting vancomycin resistant enterococci (VRE). Here, we evaluated whether there is an increased risk of VRE acquisition following vancomycin for CDI specific treatment. Patients with CDI, metronidazole, or oral vancomycin treatment and without preexisting VRE were monitored for VRE acquisition. VRE isolates from patients with acquired and preexisting colonization were collected and subjected to whole genome sequencing. In total, 281 patients (median age 5

The prognostic value of toxin B and binary toxin in Clostridioides difficile infection.

To study the association between detection of the Clostridioides difficile gene encoding the binary toxin (CDT) and direct detection of toxinB (TcdB) from feces with the appearance of serious disease, complications, or recurrence in a prospective series of cases. A total of 220 confirmed cases were included, using a two-step algorithm: an initial study to detect the enzyme, glutamate dehydrogenase (GDH), followed, in cases of positivity, by detection of the tcdB. tcdB-positive patients were investigated for the presence of CDT and TcdB. Outcome variables were severe disease, the modified Illin

Repurposing a platelet aggregation inhibitor ticagrelor as an antimicrobial against Clostridioides difficile.

Drug resistance in Clostridioides difficile becomes a public health concern worldwide, especially as the hypervirulent strains show decreased susceptibility to the first-line antibiotics for C. difficile treatment. Therefore, the simultaneous discovery and development of new compounds to fight this pathogen are urgently needed. In order to determinate new drugs active against C. difficile, we identified ticagrelor, utilized for the prevention of thrombotic events, as exhibiting potent growth-inhibitory activity against C. difficile. Whole-cell growth inhibition assays were performed and compar

Influence of prior appendectomy and cholecystectomy on Clostridioides difficile infection recurrence and mortality.

BACKGROUND: Remote appendectomy was linked to increased incidence of Clostridioides difficile infection (CDI). We evaluated the effect of absence of vermiform appendix and/or gallbladder on recurrence rate and severity of CDI. METHODS: We assessed a systemwide patient cohort diagnosed with initial CDI in 2014 (n = 250). The primary outcome was recurrence. RESULTS: Appendix and gallbladder were absent among 47 and 64 patients, respectively. CDI recurrence rate was similar among patients without and with appendix (24/47, 51.1% versus 90/203 patients, 44.3%; p = 0.404) and similar among patient

Nonalcoholic fatty liver disease as a risk factor for Clostridioides difficile infection.

Clostridioides difficile infection (CDI) is the leading cause of healthcare-associated diarrhea while nonalcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease. The aim of this study was to determine whether NAFLD increases susceptibility to CDI. A retrospective cohort study included patients ≥ 65 years, treated with antimicrobial therapy ≥ 24 h, and hospitalized ≥ 72 h in a 36-month period. Three-hundred fourteen patients were included; 83 with NAFLD and 231 controls. Except for diabetes mellitus (37.35% vs. 25.11%, p = 0.0462) and obesity (18.07% vs. 8.23%, p

Isolation of Monoclonal Neutralizing Single-Domain Antibodies Against Clostridioides difficile Toxin B.

The Clostridioides difficile infection (CDI) is one of the most common nosocomial infections around. The key pathogenicity factors of this bacterium include the toxins A and B that cause the disease symptoms. Neutralization of these toxins is one of the promising strategies for CDI treatment. Here, we isolated a panel of single-domain antibodies to the toxin B CROPs domain. Two antibodies, TB5A7 and TB4A8, exhibiting potent neutralizing activity were modified to produce homodimeric forms. The TB5A7 and TB4A8 dimers were characterized by enhanced neutralizing activity and protected animals from

Peritoneal dialysis associated peritonitis and Clostridioides difficile infection: Association or different entities?

Peritoneal dialysis (PD) associated peritonitis is a common complication and main cause of PD failure among end-stage chronic kidney disease (CKD) patients, presenting with abdominal pain, diarrhea and cloudy dialysate. On the other side, these patients may have an increased risk for Clostridioides difficile infection (CDI), with a clinical presentation indistinguishable from PD associated peritonitis. We present the case of a female patient who presented with multiple episodes of watery diarrhea days after PD catheter insertion, treated initially as a suspected PD associated peritonitis but l

Repurposing azole antifungals as rapid bactericidal, membrane-disruptive therapeutics against Clostridioides difficile.

UNLABELLED: Clostridioides difficile is a leading cause of healthcare-associated infections and remains an urgent global public health concern. Current FDA-approved therapies for C. difficile infection (CDI) are limited and are frequently associated with treatment failure and high rates of disease recurrence. To address this unmet clinical need, we employed a drug repurposing strategy focused on FDA-approved compounds with established safety profiles and limited systemic absorption characteristics, which are desirable for localized gastrointestinal infections. Through this approach, we identif

Evaluation of the Xpert Clostridioides difficile BT Assay using FecalSwab stool samples.

OBJECTIVES: The molecular detection of toxigenic Clostridioides difficile (C. difficile) infections mostly relies on the use of unpreserved stool specimens. However, recently the feasibility of sampling systems for stool pathogen detection comprising flocked swabs and preservation media has been highlighted. Hence, we evaluated the Copan FecalSwab stool sampling systems for the detection of toxigenic C. difficile by the Cepheid Xpert C. difficile BT PCR assay. METHODS: Unpreserved stool samples from patients with suspected toxigenic C. difficile infections were tested for C. difficile using t

Transition from aspirin-related upper to Clostridioides difficile-induced lower gastrointestinal bleeding.

Clostridioides difficile infection is a prevalent healthcare-associated infection that can lead to severe gastrointestinal complications, including gastrointestinal bleeding. This case report presents a 77-year-old female with multiple comorbidities, including a history of cerebral infarction, who was on long-term aspirin therapy. She initially presented with melena, suggestive of upper gastrointestinal bleeding. Preliminary treatment with pantoprazole and somatostatin resolved the melena. However, she later developed dark red blood per rectum, indicating a shift to lower gastrointestinal blee

The antimicrobial peptide Angie 5 inhibits TcdA and TcdB from Clostridioides difficile.

Clostridioides (C.) difficile is a spore-forming, toxin-producing nosocomial human gut pathogen and a causative agent of gastrointestinal infections, leading to mild to severe diarrhea. Severe C. difficile infections (CDI) can cause life-threatening conditions, such as pseudomembranous colitis, colonic perforation, or toxic megacolon. The main virulence factors of C. difficile and responsible for CDI symptoms are two AB-type protein toxins, toxin A (TcdA) and toxin B (TcdB). TcdA and TcdB are large, single-chain proteins with multiple domains and glucosyltransferase activity. After receptor-me

Clostridioides difficile Bacteremia in an Elderly Female Patient After Colon Cancer Surgery.

Clostridioides difficile infection (CDI) is a major healthcare-associated concern, particularly in the elderly. While CDI typically manifests as toxin-mediated colitis, hematogenous dissemination leading to bacteremia is relatively rare and associated with high mortality. We report an unusual case of C. difficile bacteremia in an octogenarian female patient occurring shortly after right hemicolectomy for ascending colon adenocarcinoma. This critically ill patient presented with bloodstream infection in the ICU following surgery. Multiple risk factors converged, including advanced age, extensiv

Antibacterial activity of ibezapolstat against antimicrobial-resistant clinical strains of Clostridioides difficile.

Antimicrobial resistance is emerging in clinical strains of Clostridioides difficile. Ibezapolstat (IBZ) is a DNA polymerase IIIC inhibitor that has completed phase II clinical trials. IBZ has potent in vitro activity against wild-type, susceptible strains but its effect on C. difficile strains with reduced susceptibility to metronidazole (MTZ), vancomycin (VAN), or fidaxomicin (FDX) has not been tested. The primary objective of this study was to test the antibacterial properties of IBZ against multidrug-resistant C. difficile strains. The in vitro activity, bactericidal, and time-kill activit
