Maternal smoking and the risk for clubfoot in infants.

BACKGROUND: Clubfoot is one of the most common major birth defects, with a prevalence of approximately 1 per 1,000 live births. The etiology of clubfoot is complex and not well understood, and yet, few epidemiologic studies of risk factors have been conducted. Maternal smoking has been suggested as a possible risk factor. The purpose of this population-based, case-control study was to examine the association between maternal smoking and clubfoot. METHODS: Data from the North Carolina Birth Defects Monitoring Program matched to North Carolina birth certificates and health services data were us

[Treatment of congenital clubfoot with the Ponseti method].

AIM: The primary therapy for congenital clubfoot is non-surgical involving manipulation and serial casting. With traditional casting, relatively large numbers of feet require extensive surgery to achieve full correction. The purpose of this study was to evaluate the efficacy of the Ponseti method. METHODS: Between 1.1.2004 und 31.12.2005, 29 patients with 41 clubfeet were treated with the Ponseti method. Only patients without any prior treatment were included. Classification followed Pirani's score. The number of casts to full correction, tenotomies, number of posteromedial releases, dorsi-,

Demographics of idiopathic clubfoot: is there a seasonal variation?

A seasonal variation in the incidence of idiopathic clubfoot has been reported. The purpose of this study was to determine whether a seasonal variation existed in the authors' population. The medical records of 245 children who presented with idiopathic clubfoot were reviewed. Expected month of birth, month of conception, and season of birth were determined. National Vital Statistics Reports for Births: Final Data for 2001 provided comparison data. The authors' group differed from the national data, with a male predominance and a greater percentage of mothers less than 25 years old at delivery

Correction of equinus in clubfoot: the contribution of arthrography.

The equinus deformity in clubfoot is twofold: talocrural and talocalcaneal. No study has been carried out on the link between these two elements and whether they are mutually dependent or independent. This has led to an unvarying operative strategy confined to opening the talocrural capsule and section of the talofibular ligament. To assess the extent to which these procedures are warranted, the authors performed perioperative talocrural arthrography, taking radiographs before and after each stage of operative treatment. In all cases, talocrural equinus seemed to disappear without the need for

Anthropometric study of patients treated for clubfoot.

The anthropometric characteristics of patients treated for clubfoot were used to investigate whether the dimensions of the foot were affected by the method of treatment. A total of 68 patients followed up for an average of 9 years were divided into three groups: group 1, conservatively treated; group 2, surgically treated; group 3, conservatively treated on one side and surgically treated on the other. The following average discrepancies in foot length were obtained: group 1, 0.91 cm; group 2, 1.5 cm; group 3, 1.09 cm. Toe lengths were shorter to the same extent as the other dimensions of the

Treatment of idiopathic clubfoot: an historical review.

Idiopathic clubfoot, one of the most common problems in pediatric orthopaedics, is characterized by a complex three-dimensional deformity of the foot. The treatment of clubfoot is controversial and continues to be one of the biggest challenges in pediatric orthopaedics. This controversy is due in part to the difficulty in measuring and evaluating the effectiveness of different treatment methods. We believe the heart of the debate is a lack of understanding of the functional anatomy of the deformity, the biological response of young connective tissue to injury and repair, and their combined eff

[Treatment for recurrent clubfoot with the Ilizarov technique].

Jlizarov technique was used to treat 34 recurrent clubfeet in 31 children between 1993 and 1996. No additional surgery was done; various configurations of the distractor were employed. The age at the operation ranged from 2.5 to 14 years (mean 6.7) and mean follow-up was 22 months (range 9-31 months). Twenty-six results were rated good, 4 fair and 4 poor. The authors claim ilizarov method is contraindicated in cerebral palsy and meningomyelocele. The ideal arrangement of the distractor is hinge connection between all rings and semi-rings allowing for simultaneous correction of all components o

Regional anesthesia for clubfoot surgery in children.

In children, general anesthesia is usually chosen as the primary technique during lower extremity orthopedic procedures. Certain situations or underlying conditions may be present, however, that make the conduct of general anesthesia more difficult or even relatively contraindicated. We present 10 patients with underlying conditions or circumstances that may increase the risk of general anesthesia. Regional anesthesia (spinal or epidural) was used to provide surgical anesthesia, thereby eliminating the need for general anesthesia. The techniques used and the applications of regional anesthesia

[McKay operation in the correction of rigid clubfoot].

For the evaluation of the result from McKay operation in the correction of rigid clubfoot, 71 patients (103 feet) were followed up postoperatively for 2 to 9 years. According to the criteria concerning the functional recovery of the foot, the external appearance of the foot after correction and the roentgenographic findings, the result was evaluated as excellent in 55 patients (78 feet), good in 11 patients (16 feet), fair in 5 patients (9 feet) and with no recurrence. It was considered that the favorable time for operation was when the child being 6 to 18 months old. In 7 patients (12 feet),

Critical aspects of neonatal surgery in clubfoot.

Good results after early operative treatment of clubfoot has been reported by some investigators. Most have recommended early conservative treatment and later surgical procedures in resistant cases. Operation immediately after birth is demanding, and the operative team must master all problems of neonatal surgery, anesthesiology, and postoperative care. Conservative treatment in experienced hands may lead to a satisfactory result in one third of cases. We recommend primarily conservative treatment, but patients with resistant cases must undergo operation at age 2-5 months. We need more prospec

[The radiographical changes with age of the treated clubfoot].

One hundred and eighteen feet out of 83 cases of congenital clubfoot treated mainly by early posterior release were examined for changes with age in radiographical measurements. They were classified into three groups according to treatment courses. Group I is the cases treated conservatively, group II is the cases which received early posterior release and group III is the cases that needed other operations or treatment courses. In all groups, talo-calcaneal index (TC index) decreased and calcaneal pitch (CP) increased with age but no remarkable change occurred in talo-1st-metatarsal angle (TM

Problems in surgical treatment of congenital clubfoot.

Twenty-three patients with 32 congenital rigid clubfeet treated by posteromedial release were reviewed after a follow-up averaging 33 months. The feet were assessed both clinically and radiologically; a satisfactory result was obtained in 17 feet inspite of residual deformity of forefoot adduction. Early operation and adequate postoperative immobilisation were major factors in contributing to a satisfactory result.

The four quadrant approach to clubfoot surgery.

In the last 20 years, the approach to clubfoot surgery has changed. When surgery is performed, a comprehensive posterior, medial, and lateral release is indicated. The concept of derotation of the os calcis has proved beneficial in resolving the residual pigeon-toed gait. This constitutes the fourth quadrant of correction in clubfoot surgery. The ultimate goal is to achieve a supple functioning, plantargrade foot in one sitting.

Results of posteromedial release for the resistant clubfoot.

Sixty-nine children with 99 resistant clubfeet were operated on between 1973 and 1979 at the James Whitcomb Riley Hospital for Children (Indianapolis, IN). Three groups of children according to age at the time of surgery were identified: Group I, less than 12 months of age; Group II, between 12 and 36 months of age; and Group III, greater than 36 months of age. Minimum follow-up was 30 months, with an average follow-up for the entire series of 58 months. The radiographic results based on the talocalcaneal index revealed 95 of the 99 feet to have good or acceptable results. The clinical assessm

Revised tarsectomy for correction of relapsed clubfoot.

A modification of the wedge tarsectomy used for correction of components of talipes equinovarus that helps preserve the length of the foot is presented. The procedure is demonstrated with the report of a case that was treated successfully. Correction of deformity in all three body planes in the relation of the forefoot to the rearfoot can be achieved with this combination tarsectomy-bone graft procedure that will simultaneously prevent gross shortening of the foot.

The microsurgical dissection of a stillborn fetal clubfoot.

A microsurgical dissection was performed on a clubfoot of a seven-month-old stillborn fetus with camptomelic dysplasia. The most important anatomic alteration was marked rotation of the calcaneus beneath the talus. The calcaneus was rotated around a vertical axis. Its anterior aspect was rotated medially, while its posterior aspect was rotated laterally. A series of photographs obtained during the dissection shows the degree of realignment achieved by each stage of the progressive or stepwise surgical dissection. Dissection simulating a posteromedial release and plantar release failed to reali
