OBJECTIVES: A small number of cochlear implant (CI) users experience facial nerve stimulation (FNS), which can manifest as facial twitching. In some patients, this can be resolved by adjusting the electrical stimulation parameters. However, for others, facial stimulation can significantly impair CI outcomes or even prevent its use. The exact mechanisms underlying FNS are unclear and may vary among patients.
DESIGN: Transimpedance measurements were used to assess lateral and longitudinal spread of current within 15 cochlea of nucleus CI recipients with FNS (13 unilateral recipients and 1 bilat

HYPOTHESIS: Unintentional electrical stimulation of the facial nerve by cochlear implants occurs when advanced otosclerosis invades the endosteum of both the upper basal turn of the cochlea (UBTC) and the facial nerve canal (FNC) and all the bone between these 2 structures.
BACKGROUND: A complication of cochlear implantation is facial nerve stimulation (FNS) known to be more common in otosclerosis. Otosclerotic involvement of the enchondral bone of the otic capsule results in areas of bone resorption, new bone formation, vascular proliferation, and a connective tissue stroma. This may reduce

Preservation of facial nerve function during acoustic neuroma surgery can be improved significantly by monitoring of facial electromyography (EMG) during surgery. Mechanical trauma during dissection causes EMG activity that can be played over a loudspeaker for direct feedback to the surgeon. Electrical stimulation can be used to locate the nerve even when it is out of direct view, and the threshold for stimulation provides a measure of facial (or other motor nerve) integrity. Cochlear nerve function also can be monitored by the recording of auditory brain stem responses or compound action pote

Auditory neuropathy (AN) is a term used to describe an auditory disorder in which there is evidence of normal outer hair cell function (otoacoustic emissions and/or cochlear microphonics) and poor function of the auditory nerve (absent or highly distorted auditory brain stem response starting with wave I). Many of these patients have evidence of generalized peripheral nerve disease, leading to an assumption that the peripheral portion of the auditory nerve is the most likely site of lesion. A small group of these patients has received cochlear implants, and the majority of them achieve average

Although the identity of all the variables that may influence speech recognition after cochlear implantation is unknown, the degree of preservation of spiral ganglion cells is generally considered to be of primary importance. A series of experiments in our laboratories, directed at quantification of surviving spiral ganglion cells in the profoundly deaf, evaluation of the predictive value of a variety of clinical parameters, and the evaluation of the consequences of implantation in the inner ear, is summarized. Histologic study of the inner ears of patients who were deafened during life demons

1. Tuning properties and spontaneous discharge rate of single cochlear fibres in the anaesthetized cat were determined under conditions where millimolar concentrations of KCN were instilled into the scala tympani. 2. Short-term effects on the tuning properties were obtained, in which the threshold of the low threshold sharply tuned tip segment of the frequency-threshold ('tuning') curve (f.t.c.) was elevated by up to 40 db, without changes in the threshold of the low frequency 'tail' segment of the f.t.c., or necessarily changes in the spontaneous and maximally evoked activity. These changes w

Acoustic reflex and reflex decay tests were administered to 50 normal ears, 50 ears with hearing loss due to noise trauma, 50 ears that had Meniere disease, and 28 ears that had involvement of the eighth nerve. In one normal ear, ten noise trauma ears, 11 Meniere disease ears, and 24 eighth nerve lesion ears to reflexes or reflex decay that were suggestive or retrocochlear lesions were observed. Acoustic reflex and reflex decay results are also compared to tone decay results for these patients.

1. Micro-electrode recordings were obtained from over 100 single fibres in the cochlear nerve of the pentobarbitone or urethane anaesthetized guinea-pig. The acoustic system was calibrated at the tympanic membrane and threshold sound level measurements so corrected.2. The minimum thresholds of the fibres approached with 10-20 dB of the behavioural thresholds reported in the literature. Exceptions to this were fibres from preparations where there was evidence of malfunction of the cochlea either from abnormally low perfusion or local damage, and a few high frequency fibres. With these high thre

Facial nerve anomalies are a potential problem in patients with cochleovestibular malformations. A case of cochlear implant (CI) surgery in the presence of intra-temporalbone facial nerve bifurcation is presented. During the first surgery, the facial nerve bifurcation obscured the promontory and round window. It was difficult to perform cochleostomy because of the lack of landmarks of the basal turn of the cochlea, and the first surgical attempt at cochleostomy was abandoned. A repeat CT scan was performed after the first surgery with reconstructed 3D images of the temporal bone and the cochle

Electrically evoked compound action potentials (ECAPs) have been used to examine temporal response patterns of the auditory nerve in cochlear implant (CI) recipients. ECAP responses to individual pulses in a pulse train vary across stimulation rates for individual CI users. For very slow rates, auditory neurons have ample time to discharge, recover, and respond to each pulse in the train. As the pulse rate increases, an alternating ECAP-amplitude pattern occurs. As the stimulation rate increases further, the alternating pattern eventually ceases and the overall ECAP amplitudes are diminished,

BACKGROUND AND PURPOSE: Typewriter tinnitus, a symptom characterized by paroxysmal attacks of staccato sounds, has been thought to be caused by neurovascular compression of the cochlear nerve, but the correlation between radiologic evidence of neurovascular compression of the cochlear nerve and symptom presentation has not been thoroughly investigated. The purpose of this study was to examine whether radiologic evidence of neurovascular compression of the cochlear nerve is pathognomonic in typewriter tinnitus.
MATERIALS AND METHODS: Fifteen carbamazepine-responding patients with typewriter ti

This study reports pre- and post-operative compound action potentials (CAPs) that were recorded from a 27-year-old woman with an acoustic neurinoma. During surgery it was necessary to totally sever her vestibulo-cochlear nerve to excise a large tumor. A pure tone audiogram changed to the scale-out pattern immediately after operation. However, CAP, the waveform of which was broadened, has been recorded 3 years post-operatively with a threshold elevation of 10 dB over the pre-operative threshold. This phenomenon suggests that CAP may originate from the extreme periphery of the auditory nerve wit

A 12-year-old female patient presented with unilateral sensorineural hearing loss. Distortion-product otoacoustic emission testing failed to reveal any measurable emissions in the affected side. MR imaging did not reveal labyrinthine malformation. Three-dimensional Fourier transformation-constructive interference in steady-state MR images showed a thin cochlear branch. We speculated that mumps infection or developmental malformation caused the unilateral sensorineural hearing loss.

The treatment of acoustic neuroma (vestibular schwannoma) has evolved greatly. In this report, we studied the history of acoustic tumour surgery, and documented the value of technical advances in benefiting patients. We also present our outcome of surgery for this benign tumour in support of its use as the treatment of choice. In 611 patients undergoing initial surgery at Johns Hopkins from 1973-1994, complete resection was obtained in all but one case (intentional), and permanent morbidity and mortality rate was 0.3%. Including temporary morbidity, the rate was 3.8%. Tumour recurrence was see

The short- and long-term effects of static compression of the cochlear nerve were studied in dogs. The nerve was exposed in the cerebellopontine angle and a modified aneurysm clip was applied to reduce the diameter of the nerve trunk to 50%, 40%, 30% or 20% of normal (designated respectively as 50%, 60%, 70%, and 80% compression). Brainstem auditory evoked potentials (BAEPs) were monitored intraoperatively and post-operatively. The animals were sacrificed between 5 and 119 days after nerve compression and temporal bones were examined histologically. In the 50% compression group, all peaks exce

Promontory stimulation is an accurate method of assessing the functional integrity of the cochlear nerve by electrical stimulation. This technique is widely used in screening prospective candidates for cochlear implants. Recent reports have confirmed the integrity of cochlear nerve responses to electrical stimulation following labyrinthectomy and unsuccessful hearing preservation attempt by the retrosigmoid approach. In this report, the authors present three patients with intracanalicular acoustic neuromas excised through a translabyrinthine approach in which the cochlear nerve was preserved.
