Recognition and Treatment of Cognitive Dysfunction in Major Depressive Disorder.

Major Depressive Disorder (MDD) is a prevalent, chronic, disabling, and multidimensional mental disorder. Cognitive dysfunction represents a core diagnostic and symptomatic criterion of MDD, and is a principal determinant of functional non-recovery. Cognitive impairment has been observed to persist despite remission of mood symptoms, suggesting dissociability of mood and cognitive symptoms in MDD. Recurrent impairments in several domains including, but not limited to, executive function, learning and memory, processing speed, and attention and concentration, are associated with poor psychosoci

Multiple sclerosis and cognition: synaptic failure and network dysfunction.

Cognitive impairment is increasingly recognized to be a core feature of multiple sclerosis (MS), with important implications for the everyday life of individuals with MS and for disease management. Unfortunately, the exact mechanisms that underlie this cognitive impairment are poorly understood and there are no effective therapeutic options for this aspect of the disease. During MS, focal brain inflammatory lesions, together with pathological changes of both CNS grey matter and normal-appearing white matter, can interfere with cognitive functions. Moreover, inflammation may alter the crosstalk

Neuroimaging and genetic correlates of cognitive dysfunction in multiple sclerosis.

Cognitive impairment occurs in 40-70% of patients with multiple sclerosis (MS). It is observed even at the early stage of disease, including clinically isolated syndrome (CIS). Cognitive dysfunction develops irrespectively of the physical disability. Affected domains include: information processing speed, visuospatial abilities, attention, verbal memory and executive functions. Cognitive deficits have relevant implication because of their impact on daily living, quality of life and increased risk of conversion from CIS to MS. In the recent years the issue of cognitive impairment in MS became a

Cognitive estimations as a measure of executive dysfunction in childhood epilepsy.

Children and adolescents with epilepsy are known to demonstrate executive function deficits. Despite prior work that has shown that cognitive estimation tasks are sensitive to executive dysfunction in children, such tasks have not been studied in children with epilepsy. This is particularly important given the fact that executive tasks have heretofore shown poor ecological validity, and it has been speculated that estimation tasks may show stronger ecological validity than other executive tests. One hundred and thirteen clinically referred children and adolescents with epilepsy were included.

Dysfunctional Cognitions among Offspring of Individuals with Bipolar Disorder.

BACKGROUND: Individuals with bipolar disorder often endorse dysfunctional beliefs consistent with cognitive models of bipolar disorder (Beck, 1976; Mansell, 2007). AIMS: The present study sought to assess whether young adult offspring of those with bipolar disorder would also endorse these beliefs, independent of their own mood episode history. METHOD: Participants (N = 89) were young adult college students with a parent with bipolar disorder (n = 27), major depressive disorder (MDD; n = 30), or no mood disorder (n = 32). Semi-structured interviews of the offspring were used to assess diagno

White matter atrophy and cognitive dysfunctions in neuromyelitis optica.

Neuromyelitis optica (NMO) is an inflammatory disease of central nervous system characterized by optic neuritis and longitudinally extensive acute transverse myelitis. NMO patients have cognitive dysfunctions but other clinical symptoms of brain origin are rare. In the present study, we aimed to investigate cognitive functions and brain volume in NMO. The study population consisted of 28 patients with NMO and 28 healthy control subjects matched for age, sex and educational level. We applied a French translation of the Brief Repeatable Battery (BRB-N) to the NMO patients. Using SIENAx for globa

Cognitive visual dysfunctions in preterm children with periventricular leukomalacia.

AIM: Cognitive visual dysfunctions (CVDs) reflect an impairment of the capacity to process visual information. The question of whether CVDs might be classifiable according to the nature and distribution of the underlying brain damage is an intriguing one in child neuropsychology. METHOD: We studied 22 children born preterm (12 males, 10 females; mean age at examination 8y, range 6-15y; mean gestational age 30wks, range 28-36wks) with periventricular leukomalacia, spastic diplegia, normal intelligence (mean Full-scale IQ 84; mean Verbal IQ 97; mean Performance IQ 74), and normal visual acuity,

Is donepezil useful for improving cognitive dysfunction in bipolar disorder?

BACKGROUND: Cognitive dysfunctions are being recognized as a major roadblock to functional recovery in patients with bipolar disorders. Little is known about the treatment of these cognitive dysfunctions. Donepezil, approved to treat memory dysfunction in Alzheimer's disease, is evaluated for cognitive dysfunctions common in bipolar disorder. Of concern is some evidence that donepezil may trigger affective instability. METHODS: All bipolar disordered patients in a private practice setting treated with donepezil for memory problems were analyzed. Patients were assessed for memory improvement a

Cognitive dysfunctions in bipolar disorder: evidence of neuropsychological disturbances.

Although cognitive dysfunctions in psychosis have classically been associated with schizophrenia, there is clinical evidence that some bipolar patients show cognitive disturbances either during acute phases or in remission periods. The authors critically review the data on cognitive impairment in bipolar disorder. The main computerized databases (Medline, Psychological Abstracts, Current Contents) have been consulted crossing the terms 'cognitive deficits', 'neuropsychology', 'intellectual impairment', 'mania', 'depression' and 'bipolar disorder'. Changes in the fluency of thought and speech,

Behavioral dysfunction and cognitive efficiency in male and female alcoholics.

This study was conducted to examine the role of childhood behavioral disorders (CBDs) and residual attention deficit disorder (ADDRT) in alcohol-related cognitive dysfunction in male and female subjects. Alcoholic (n = 44 females, 56 males) and control (n = 40 females, 40 males) subjects completed assessments that included measures of CBDs, ADDRT, and cognitive and psychosocial functioning. Cognitive tests were specifically designed to assess efficiency in function. As expected, alcoholics were inferior to controls in their cognitive efficiency [F(1,171) = 10.43, p = 0.0015]. General and specific cognitive dysfunctions in patients with Alzheimer's disease.

The purpose of the present study was to investigate the effect of Alzheimer's disease (AD) on general and specific neuropsychological function. Thirty-five subjects diagnosed with AD were compared to 30 medically normal aging control subjects using measures from the Halstead-Reitan Battery (HRB). All AD subjects were classified as "probable" AD (NINCDS-ADRDA) and found to be in the "late confusional" to "early dementia" stages of the disease (Global Deterioration Scale, GDS = 4-5). The AD group performed significantly worse than the controls on all measures of general neuropsychological functi

Arketamine alleviates cognitive impairments and demyelination in mice with postoperative cognitive dysfunction via TGF-β1 activation.

Postoperative cognitive dysfunction (POCD) is characterized by a decline in cognitive functions, including memory, attention, and executive abilities, following surgery, with no effective therapeutic drugs currently available. Arketamine, the (R)-enantiomer of ketamine, has shown promise in mitigating cognitive deficits in animal models. In this study, we investigated whether arketamine could ameliorate cognitive deficits in a mouse model of POCD, with a focus on the role of transforming growth factor (TGF)-β1 in its effects. POCD mice displayed cognitive impairments and demyelination in the c

Pre-existing cognitive impairment and post-operative cognitive dysfunction: should we be talking the same language?

Changes in cognition are known to follow anesthesia and surgery in older individuals (Evered et al., 2011). Although survival per se was the prime outcome in the 19th and early 20th centuries for invasive procedures, a link was none-the-less observed with adverse cognitive outcomes as far back as 1887 (Savage, 1887). Historical reports of "insanity" or "weak mindedness" after anesthesia appeared within 40 years of the first anesthetic having been administered and anecdotal and retrospective reports have implicated anesthesia ever since. However, it was not until the 1970s that these observatio

Targeting Mitochondria for Postoperative Cognitive Dysfunction: From Mechanisms to Therapeutics.

Postoperative cognitive dysfunction (POCD) is a frequent neurological complication in older surgical patients, leading to substantial declines in quality of life and placing a considerable burden on society. This review provides a comprehensive overview of how mitochondrial dysfunction contributes to POCD, including disrupted energy metabolism, excessive reactive oxygen species production, calcium imbalance, and abnormalities in mitochondrial dynamics and quality control processes. These mitochondrial impairments further trigger neuroinflammation and activate multiple cell death pathways. In a

Linking subjective cognitive decline to glymphatic dysfunction in Parkinson's disease.

BACKGROUND: The role of glymphatic system in the subjective cognitive decline (SCD) stage of Parkinson's disease (PD) remains unexplored. This study investigated whether glymphatic dysfunction occurs in PD-SCD and its potential role in mediating the link between sleep disturbance and cognitive impairment. METHODS: We recruited 73 PD patients, categorized into three groups: PD with normal cognition (PD-NC, n = 24), PD-SCD (n = 22), and PD with mild cognitive impairment (PD-MCI, n = 27), alongside 27 healthy controls (HC). All participants underwent 3 T MRI to calculate the diffusion tensor ima
