Antibiotic Resistance and Biofilm Gene Distribution in Colistin-Resistant Acinetobacter baumannii.

Acinetobacter baumannii, a multidrug-resistant opportunistic bacterium, poses a substantial hazard in hospital settings. The emergence of colistin- and tigecycline-resistant strains further limits treatment options and necessitates detailed investigation of resistance mechanisms. A total of 144 clinical A. baumannii isolates from multiple hospitals in Iran were identified using standard microbiological and molecular techniques. Antimicrobial susceptibility was assessed using both disk diffusion and broth microdilution techniques. Biofilm formation was quantified by crystal violet staining. Res

Synergistic activity of colistin and stilbenoids against Pseudomonas aeruginosa clinical isolates.

Pseudomonas aeruginosa is a major opportunistic pathogen responsible for chronic lung infections in cystic fibrosis (CF) patients, contributing significantly to morbidity and mortality. In this study, we investigated the synergistic activity of colistin combined with two natural stilbenoids, pterostilbene and (±)-trans-ε-viniferin, against clinical P. aeruginosa isolates from CF patients. Synergy was assessed through minimum inhibitory concentration (MIC) determination and time-kill assays. The combination of colistin with pterostilbene reduced colistin MIC values in all tested strains, with t

Antibacterial effect of rose bengal against colistin-resistant gram-negative bacteria.

Increasing drug resistance in Gram-negative bacteria presents significant health problems worldwide. Despite notable advances in the development of a new generation of β-lactams, aminoglycosides, and fluoroquinolones, it remains challenging to treat multi-drug resistant Gram-negative bacterial infections. Colistin (polymyxin E) is one of the most efficacious antibiotics for the treatment of multiple drug-resistant Gram-negative bacteria and has been used clinically as a last-resort option. However, the rapid spread of the transferable gene, mcr-1 which confers colistin resistance by encoding a

Colistin-induced acquired Bartter-like syndrome: an unusual cause of meltdown.

Colistin-induced nephrotoxicity is commonly associated with elevation of serum creatinine level or a reduction of urine output. Uncommonly, tubulopathy associated with colistin has been reported. Here we present a unique case of a 46-year-old man who developed polyuria, hypokalaemia, hypocalcaemia, hypomagnesemia and metabolic alkalosis after 3 days of therapy with intravenous colistimethate sodium. After ruling out other causes, a diagnosis of colistin-induced acquired Bartter syndrome was made. The patient required daily aggressive intravenous repletion of fluids and electrolytes. However, p

Repurposing Eukaryotic Kinase Inhibitors as Colistin Adjuvants in Gram-Negative Bacteria.

Kinase inhibitors comprise a diverse cohort of chemical scaffolds that are active in multiple biological systems. Currently, thousands of eukaryotic kinase inhibitors are commercially available, have well-characterized targets, and often carry pharmaceutically favorable toxicity profiles. Recently, our group disclosed that derivatives of the natural product meridianin D, a known inhibitor of eukaryotic kinases, modulated behaviors of both Gram-positive and Gram-negative bacteria. Herein, we expand our exploration of kinase inhibitors in Gram-negative bacilli utilizing three commercially availa

Ultrastructural evaluation of urine alkalinization versus hydration on colistin-induced nephrotoxicity.

OBJECTIVES: Colistin is a vital antibiotic used in multidrug-resistant infections. Its most important side effect is nephrotoxicity. Colistin is a weak acid. This study aims to evaluate whether urine alkalinization is protective in the nephrotoxicity of colistin. METHODS: Twenty-eight male Sprague-Dawley rats were divided into groups. Group I (n = 4) was injected with intramuscular distilled water twice a day for 7 days. Group II (n = 8) was injected with 750,000 IU/kg/day colistin for 7 days. Group III (n = 8) was injected with the same dose of colistin after their urinary pH was ≥7 through

Attributable Risk and Time Course of Colistin-Associated Acute Kidney Injury.

BACKGROUND AND OBJECTIVES: Despite colistin's longstanding reported association with nephrotoxicity, the attributable risk and timing of toxicity onset are still unknown. Whether substantial toxicity occurs during the initial 72 hours of exposure has important implications for early treatment decisions. The objective of this study was to compare colistin-exposed patients with a matched control group given other broad spectrum antibiotics. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: We conducted a retrospective cohort study in patients treated for multidrug-resistant Pseudomonas, Klebsiella

Toxicity after prolonged (more than four weeks) administration of intravenous colistin.

BACKGROUND: The intravenous use of polymyxins has been considered to be associated with considerable nephrotoxicity and neurotoxicity. For this reason, the systemic administration of polymyxins had been abandoned for about 20 years in most areas of the world. However, the problem of infections due to multidrug-resistant (MDR) Gram-negative bacteria such us Pseudomonas aeruginosa and Acinetobacter baumanniii has led to the re-use of polymyxins. Our objective was to study the toxicity of prolonged intravenous administration of colistin (polymyxin E). METHODS: An observational study of a retrosp

Paired In Vitro Susceptibility of Cefiderocol and Colistin in Colistin-Resistant, Extensively Drug-Resistant Gram-Negative Bacilli: A Single-Centre, MIC-Based Analysis of 236 Clinical Isolates.

Extensively drug-resistant (XDR) Gram-negative bacilli increasingly exhaust therapeutic options, leaving colistin and cefiderocol as last-resort agents. We assessed whether cefiderocol retains in vitro activity against colistin-resistant isolates, without inferring clinical efficacy. We analysed 236 non-duplicate XDR Gram-negative isolates from 220 patients. Cefiderocol and colistin MICs were determined via reference broth microdilution with EUCAST-required quality control (iron-depleted broth for cefiderocol; mcr-1-positive Escherichia coli NCTC 13846 for colistin) and interpreted using speci

The Auranofin-Colistin Combination Efficiency Against the Gram-Negative Strains: A Review.

Multidrug-resistant bacteria that possess resistance genes against last-resort antibiotics, such as colistin, are notably prevalent in hospital environments. Therapeutic options for addressing these infections are limited, prompting researchers to explore alternative strategies. One promising approach is drug repurposing, which involves testing combinations of antibiotics with other medications to discover potential synergistic effects against multidrug-resistant (MDR) strains. Auranofin, an antirheumatic drug known for its anti-inflammatory and antineoplastic properties, has recently been inv

Redox biology underlying the efficacy and toxicity of colistin as a last-resort antibiotic.

The increasing prevalence of multidrug-resistant Gram-negative bacteria has reinforced the clinical importance of last-resort antibiotics such as colistin. Their antibacterial efficacy has made them indispensable for treating life-threatening infections, yet their clinical use is constrained by substantial toxicity. Increasing evidence indicates that redox biology may influence both the antibacterial activity and host toxicity of colistin through distinct biological processes. Reactive oxygen species contribute to bacterial killing while simultaneously promoting mitochondrial dysfunction, oxid

Pharmacodynamic modeling of colistin and imipenem against in vitro Pseudomonas aeruginosa biofilms.

INTRODUCTION: Antibiotic treatment of chronic biofilm-associated infections can be challenging. Characterization of pharmacokinetic/pharmacodynamic (PK/PD) relationships for biofilm-associated infections may be relevant to informing the design of antibiotic treatment regimens for biofilm-associated infections. To this end, we aim to develop a mathematical PK/PD model for planktonic and biofilm bacterial infections and demonstrate how PK/PD simulations can be used to design optimized dosing schedules, using colistin and imipenem as proof-of-concept examples. METHODS: Pharmacodynamic models wer

An Overview of Mobile Colistin Resistance (mcr) Genes in Gram-Negative Bacilli.

The increasing spread of mobile colistin resistance (mcr) genes is becoming a major concern in the treatment of infections caused by multidrug-resistant Gram-negative bacilli. Colistin is often used as a last treatment option, but the emergence of mcr genes is reducing its effectiveness. These genes are most commonly found in bacteria, such as Escherichia coli, Klebsiella pneumoniae, and Salmonella, and have also been reported, though less frequently, in organisms like Pseudomonas aeruginosa. This review provides an overview of the occurrence, diversity, and mechanisms of mcr genes in Gram-neg

An Overview of the Genetic Mechanisms of Colistin-Resistance in Bacterial Pathogens: An Indian Perspective.

Colistin resistance in bacteria is a growing global issue, given its role as a critical last-resort antibiotic, particularly for treating Gram-negative bacterial infections. Pathogens adopt multiple resistance mechanisms, mediated either by plasmids or chromosomal changes. Some of the most frequently observed strategies include the occurrence of plasmid-borne mobile colistin resistance (mcr) genes, enhanced efflux pump activity, mutations in the regulatory systems, and alterations in the lipid A structure. This article provides an overview of the studies investigating the genetic mechanisms un

Development of LpxC Inhibitors Based on the Mechanism of Action of Colistin in Acinetobacter baumannii.

Multidrug-resistant (MDR) Acinetobacter baumannii is a major clinical threat with limited treatment options, as current therapies rely on polymyxins such as colistin. Targeting the lipopolysaccharide (LPS) biosynthetic pathway offers a new target for new antibiotic discovery, yet most efforts have focused on enzyme-based assays that do not reflect cell level physiology. Here, we developed a cell-based screening strategy that links colistin resistance to inhibition of lipooligosaccharide (LOS) biogenesis in A. baumannii. Using colistin, we established a phenotypic platform in which compounds th

Colistin antibacterial activity, clinical effectiveness, and mechanisms of intrinsic and acquired resistance.

Following a period of disuse owing to the emergence of multidrug-resistant Gram-negative bacteria, colistin has regained global attention as an antibiotic of last resort. The resurgence in its utilization has led to a concurrent increase in acquired resistance, presenting a significant challenge in clinical treatment. Predominantly, resistance mechanisms involve alterations in the lipid A component of the lipopolysaccharide (LPS) structure. These alterations are facilitated by the addition of cationic phosphoethanolamine (pEtN) or 4-amino-L-arabinose, often driven by the upregulation of two-co
