The Collagenase of the Bacterium Clostridium histolyticum in the Treatment of Irradiation-Induced Capsular Contracture.

BACKGROUND: Irradiation therapy is an important pillar in the treatment of breast cancer. However, it can trigger capsular fibrosis, the most significant complication of implant-based breast reconstruction. As collagen is the main component of fibrotic capsules, the collagenase of the bacterium Clostridium histolyticum poses a potential treatment option for this pathological condition. METHODS: Thirty-six rats received miniature silicone implants on their backs. On day 1, the implant sites of two groups were irradiated with 10 Gy. On day 120, one irradiated group received collagenase injectio

Collagenase Clostridium Histolyticum in the Treatment of Urologic Disease: Current and Future Impact.

INTRODUCTION: Peyronie's disease (PD) is a connective tissue disorder resulting in the abnormal accumulation of type I to III collagen, fibrin, and disorganized elastic fibers in the tunica albuginea of the penis. Many medical and non-pharmacologic modalities have been used in the treatment of PD; however, these approaches have proved largely ineffective, with surgery being the only definitive treatment. Intralesional injection of collagenase Clostridium histolyticum (CCH) has recently become the gold standard for minimally invasive treatment of PD, and studies have suggested the role of CCH c

Salvage palmar fasciectomy after initial treatment with collagenase clostridium histolyticum.

BACKGROUND: Collagenase clostridium histolyticum was approved for clinical use in 2010 and has become an accepted treatment modality for Dupuytren's contracture. Because longitudinal experience with injectable collagenase remains limited, the effect of treatment on future surgery is not well defined. METHODS: A retrospective review of the senior author's practice from February of 2010 through March of 2014 was performed. Eleven patients were identified who had digital or palmar fasciectomy after at least one previous injection of collagenase clostridium histolyticum. Cases were reviewed for f

Late complications of clinical clostridium histolyticum collagenase use in Dupuytren's disease.

Источник пометил публикацию как отозванную

INTRODUCTION: While Dupuytren's disease can cause disabling contractures requiring open surgery, a less-invasive option using Clostridium Histolyticum collagenase (CHC) via percutaneous injection was recently reported. A recent prospective, randomized trial demonstrated few complications during 90 days follow-up, however did not assess any longer term follow-up for these patients. Long-term outcomes in this setting have not been adequately reported, and the current manuscript aims to identify late complications from the clinical use of percutaneous CHC. METHODS: The current manuscript reports

Efficacy and safety of injectable mixed collagenase subtypes in the treatment of Dupuytren's contracture.

PURPOSE: To further evaluate the efficacy and safety of an injectable mixed subtype collagenase for the treatment of Dupuytren's contracture (DC). METHODS: Patients with flexion deformities of the metacarpophalangeal (MCP) and/or the proximal interphalangeal (PIP) joints of 20 degrees or greater were randomized in a double-blind, placebo-controlled trial. Patients completing this phase could enter an open-label extension phase. The primary efficacy variable was clinical success: contracture correction to within 5 degrees of normal (normal, 0 degrees ). Additional efficacy variables included t

Locally-applied collagenase and regeneration of transsected and repaired rat sciatic nerves.

The possible effects of collagenase on peripheral nerve regeneration were evaluated after epineurial repair of rat sciatic nerves. In the control group the repair site was covered by fibrin adhesive and infused with isotonic saline and in the experimental group collagenase was infused into the fibrin adhesive. In the short term study the regeneration distance was measured by a pinch test four, six, or eight days postoperatively. In the long term study the evaluation of nerve regeneration and recovery of motor function was made by testing the tetanic contraction force of the anterior tibial mus

Stretching of fetal membranes increases the concentration of interleukin-8 and collagenase activity.

OBJECTIVE: The aim of this study was to determine whether stretching of fetal membranes can increase interleukin-8 concentration and collagenase activity. STUDY DESIGN: Strips of whole fetal membranes, amnion, or muscles of the lower uterine segment were stretched for 2 or 4 hours. Interleukin-8 and collagenase activity were measured in homogenized control and stretched samples. Immunohistochemical staining for interleukin-8 was carried out. RESULTS: The interleukin-8 concentration increased significantly after the whole fetal membranes were stretched for 2 and 4 hours (p <0.0007 and 0.001,

Reduced collagenase gene expression in fibroblasts from hypertrophic scar tissue.

The major histopathological feature of hypertrophic scar lesions is fibrosis, characterized by excessive accumulation of collagen. The purpose of this study was to determine if there is not only increased expression of collagen but also decreased expression of collagenase in hypertrophic scar fibroblasts. We compared the expression of mRNA for alpha 1 (I) and alpha 1 (III) collagen, and collagenase in cultured fibroblasts from different portions of hypertrophic scars and normal dermis. In hypertrophic scar fibroblasts, increased levels of alpha 1 (I) and alpha 1 (III) collagen mRNAs were obser

Site-directed mutagenesis of type I collagen: effect on susceptibility to collagenase.

A genetic approach to define the role of collagenase in physiological and pathological bone remodeling is to identify spontaneous mutations in the collagenase gene which alter enzymatic activity. Alternatively it is possible, though site-directed mutagenesis, to alter genes encoding critical amino acid sequences in the collagen substrate, in a manner analogous to the successful development of animal models for osteogenesis imperfecta. We have thus utilized this approach to alter the Col1a1 gene to encode amino acid substitutions in sequences around the known collagenase cleavage site (glycine-

Collagenase activity in anterior cruciate ligament: protective role of the synovial sheath.

To evaluate the protective role of the synovial sheath of the anterior cruciate ligament (ACL), we have developed a synovectomy model that exposes the ACL substance to the intra-articular environment with and without hemarthrosis. Histology and the level of collagenase activity were studied to assess intrinsic ligament alterations. The treatment groups studied were as follows: ACLs of sham-operated knees receiving arthrotomy only, ACLs of knees receiving arthrotomy and acute hemarthrosis, ACLs of knees that underwent synovectomy, and ACLs of knees that underwent both synovectomy and acute hema

Distribution of collagenase and cell types in sterile ulceration of human corneal grafts.

Extensive clinical and experimental evidence has suggested a role for corneal epithelium, keratocytes, and acute inflammatory cells in sterile corneal ulceration, although the precise role of these cell types in stromal matrix degradation has not been elucidated. We studied two corneal buttons and two whole corneas from 4 patients with corneal grafts and sterile stromal ulceration. Each cornea was divided into several sections and examined morphologically, while adjacent sections were placed in tissue culture and assayed for collagenase activity against type I collagen. In each case, collagena

Inhibition of epiphyseal cartilage collagenase by tetracyclines in low phosphate rickets in rats.

Drugs in the tetracycline family can inhibit mammalian tissue collagenase both in vitro and in vivo by a mechanism that is independent of antibiotic action. The epiphyseal cartilages of rachitic rats contain extremely high levels of collagenase (CGase), and we have used this model to study further the phenomenon of tetracycline inhibition of tissue CGase. Rickets was induced in rats by phosphate/vitamin D deficiency and parameters of gross bone morphology, bone chemistry, and serum chemistry were evaluated in both rachitic and nonrachitic animals with and without treatment with oral tetracycli

Immunolocalization of collagenase and tissue inhibitor of metalloproteinases (TIMP) in hypertrophic scar tissue.

Normal, mature and hypertrophic dermal scars were examined by indirect immunofluorescence for the presence of collagenase, tissue inhibitor of metalloproteinases (TIMP) and cathepsin D. Significant extracellular immunoprecipitation of both collagenase and TIMP were found in areas of all scars judged to be actively remodelling, whereas inactive areas were predominantly negative. TIMP was also present in endothelial cells of patent blood vessels, but found not to be present in the enlarged endothelial cells of occluded vessels. Negligible amounts of extracellular cathepsin D were found.
