Based on practical experience, a systematic approach to conversion of ileal J-pouches into continent ileostomies is developed by defining three types of conversion surgery, each with two subtypes. Type 1 refers to conversion without pouch reconstruction, type 2 to partial pouch reconstruction, and type 3 to complete pouch reconstruction. Physiological disturbances can be caused by loss of large intestine, ileocecal valve and half of anal canal mucosa. Pouch can not compensate the whole colonic absorptive function and patients have to live with some of functional abnormality. Pouch should be made as large as possible with good compliance to maintain reservoir function. Post-operative anal complications also should be minimized to prevent anal dysfunction caused by late fistula or stricture.

To study mucosal changes in ileal pouches, three different types of pelvic ileal reservoir (2-loop, 3-loop and 4-loop), interposed in place of resected rectum, were constructed in 12 piglets, 4 animals in each group (n = 12). Ultralow anterior resection with coloanal anastomosis has been proven to be oncologically sound and the majority of patients will have acceptable functional outcome. Here we describe a technique that combines laparoscopic ultralow total mesorectal excision with an intersphincteric dissection in order to allow the mobilized rectum and descending colon to be extracted via the muscular anal canal and so avoid any further abdominal incision other than laparoscopic port sites. We believe this novel approach to have significant clinical potential in selected patients.

A sigmoidorectal pouch was constructed in 20 patients (18 with invasive bladder cancer, 2 with complete urethral destruction with multiple vesicovaginal fistulas).
