Colocolonic intussusception of a colostomy after colonoscopy.

The incidence of complications after colonoscopy is low and has been reported to range of 0.01-0.9%. Of these complications, colocolonic intussusception after colonoscopy is exceedingly rare, with around 12 known cases described in the literature. This case report details the presentation and operative management of a patient who developed an ischemic stoma due to a colocolonic intussusception of an end colostomy after a colonoscopy. Intraoperative surgical exploration revealed a colocolonic intussusception involving the end colostomy. This is the first known documented occurrence of this phen

Adequacy of Anesthesia Guidance for Colonoscopy Procedures.

In patients undergoing colonoscopy procedures (CPs), inadequate dosing of hypnotic drugs (HD) and opioid analgesics (OA) during intravenous sedoanalgesia (ISA) may lead to intraprocedural awareness with recall (IAwR), intraprocedural (IPP) and postprocedural pain (PPP), as well as postoperative nausea and vomiting (PONV). The aim of this study was to evaluate whether the titration of HD and OA based on the observance of changing values of state entropy (SE) and surgical pleth index (SPI) (adequacy of anesthesia-AoA), state entropy alone, or standard practice may reduce the number of adverse ev

Appendicitis: a rare adverse event in colonoscopy.

We present a case of a 41-year-old woman who visited the emergency department (ED) with acute abdomen. She was diagnosed with perforated appendicitis and abscess formation on CT. She was treated conservatively with antibiotics and discharged. On control CT 3 months later, the appendix had healed, but signs of thickening of the terminal ileum were noticed and colonoscopy was performed, which was uneventful and showed no signs of inflammation. Twelve hours later, she developed pain in the right lower quadrant, followed by fever, and visited the ED. Physical examination and blood work showed sign

Anisakiasis of the Terminal Ileum Removed by Colonoscopy.

A 35-year-old Japanese man with no significant medical history presented to the emergency department complaining of abdominal pain, nausea, and vomiting 35 h after ingesting raw horse mackerel, sardines, and tuna. Computed tomography of the abdomen with intravenous contrast revealed significant thickening of the terminal ileum with mild proximal dilatation and moderate ascites. On colonoscopy, an actively moving anisakis larva was discovered in the terminal ileum. The larva was removed with endoscopic forceps. The patient's symptoms resolved quickly following the procedure.

Potential of colonoscopy as a treatment for intussusception in children.

BACKGROUND AND STUDY AIMS: Invagination, which can occur in any part of the intestine, usually affects children and is the second most common cause of intestinal obstruction after pyloric stenosis. The cause of these intussusceptions in children is usually unknown and they typically occur within the ileocolic region. Management of pediatric patients with the condition often consists of surgical intervention. However, this retrospective study from the University Hospital of the University of São Paulo, Brazil, reports a series of cases of intussusception in children in whom a colonoscopy was us

[A case of diaphragmatic hernia following colonoscopy].

An 89-year-old woman with a history of traumatic injury was referred to our hospital for further evaluation of anemia. Two days after colonoscopy, she complained of intermittent abdominal pain. An abdominal computed tomography confirmed a left diaphragmatic defect with a herniated transverse colon. She underwent elective laparoscopic repair of the diaphragmatic hernia. Colonoscopy rarely causes or worsens a diaphragmatic hernia. This is a rare case where we observed the development and exacerbation of a diaphragmatic hernia. It is important to pay attention to the development of a diaphragmati

Clinical correlates of trichuriasis diagnosed at colonoscopy.

Diagnosis of Trichuris trichiura infestations is usually based on identification of barrel-shaped ova in stool, but is frequently missed on stool microscopy. We describe the clinical profile of patients in whom Trichuris infection was incidentally diagnosed at colonoscopy. In a cross-sectional study, patients with colonoscopic diagnosis of trichuriasis were enrolled from the endoscopy unit in a tertiary care center. Blood and stool samples were collected from all those who were willing to participate and provide samples. Sixty-two patients participated, with mean (SD) age of 50.5 (13.6) years

Time trends in quality indicators of colonoscopy.

BACKGROUND: There is considerable variation in the quality of colonoscopy performance. The Norwegian quality assurance programme Gastronet registers outpatient colonoscopies performed in Norwegian endoscopy centres. The aim of Gastronet is long-term improvement of endoscopist and centre performance by annual feedback of performance data. OBJECTIVE: The objective of this article is to perform an analysis of trends of quality indicators for colonoscopy in Gastronet. METHODS: This prospective cohort study included 73,522 outpatient colonoscopies from 73 endoscopists at 25 endoscopy centres from

Bowel preparations as quality indicators for colonoscopy.

Colonoscopy is the principal investigative procedure for colorectal neoplasms because it can detect and remove most precancerous lesions. The effectiveness of colonoscopy depends on the quality of the examination. Bowel preparation is an essential part of high-quality colonoscopies because only an optimal colonic cleansing allows the colonoscopist to clearly view the entire colonic mucosa and to identify any polyps or other lesions. Suboptimal bowel preparation not only prolongs the overall procedure time, decreases the cecal intubation rate, and increases the costs associated with colonoscopy

PDR or ADR as a quality indicator for colonoscopy.

Interval (missed) cancers and lower-than-expected mortality reduction of proximal colon cancers after screening colonoscopy drew attention to quality indicators. Small proximal polyps (prone to be advanced neoplasms) missed by colonoscopy are possible contributing factors. In this issue of AJG, the subject of polyp detection rates (PDRs) and adenoma detection rates (ADRs) in the proximal and distal colon is discussed by one group of investigators to address the issue of monitoring performance and to achieve improvement. The authors observed that these two parameters correlated well in segments

Repeat colonoscopy's value in gastrointestinal bleeding.

AIM: To assess the diagnostic yield and clinical value of early repeat colonoscopies for indications other than colorectal cancer (CRC) screening/surveillance. METHODS: A retrospective review of patients who had more than one colonoscopy performed for the same indication within a three year time frame at our tertiary care referral hospital between January 1, 2000 and January 1, 2010 was conducted. Exclusion criteria included repeat colonoscopies performed for CRC screening/surveillance, poor bowel preparation, suspected complications from the index procedure, and incomplete initial procedure.

[A case of acute cholecystitis after colonoscopy].

A 43-year-old man, who received total gastrectomy five years ago for advanced gastric cancer, underwent a screening colonoscopy and abdominal CT scan. Abdominal CT scan revealed no abnormal findings. Colonoscopy revealed polyps at the rectum, which were removed by polypectomy. The patient did not complain of abdominal pain or discomfort throughout the procedure. But, he developed right upper quadrant abdominal pain on the next day after colonoscopy. Abdominal CT scan revealed the distended gallbladder with mild wall thickening and suspicious sandy stones or sludge in the gallbladder. The patie

Diagnosis of a cecal tumour with virtual colonoscopy.

The authors presented a 75-year-old female patient at high risk, suspected of a cecal cancer (CC) due to discomfort in the inguinal fossa, microcytic anemia (although she was postmenopausal), liquid stools and a positive faecal occult blood test. A standard examination of the large intestine was undertaken. Conventional colonoscopy was not completed and the results of barium enema were questionable. Therefore, virtual colonoscopy (VC) was performed, which helped to localize an accurate operation site. As a result, the patient underwent right hemicolectomy. Postoperative histopathological asses

Utilization of surveillance colonoscopy in community practice.

BACKGROUND & AIMS: The recommended timing of surveillance colonoscopy for individuals with adenomatous polyps is based on adenoma histology, size, and number. The burden and cost of surveillance colonoscopy are significant. The aim of this study was to examine the use of surveillance colonoscopy on a community-wide basis. METHODS: We retrospectively queried participants in the Prostate, Lung, Colorectal, and Ovarian Cancer screening trial in 9 US communities about use of surveillance colonoscopy. Subjects whose initial colonoscopy showed advanced adenoma (AA), nonadvanced adenoma (NAA), or no

IUD removal with colonoscopy: a case report.

BACKGROUND: Intrauterine device (IUD) perforation of the bowel is uncommon. Although IUD perforation may be asymptomatic, the most common complaint is unexplained abdominal pain. CASE: A case of IUD perforation of the large bowel was diagnosed 7 years after insertion. The patient presented with unexplained lower abdominal pain diagnosed initially as pelvic inflammatory disease. Laparoscopy revealed that the IUD was embedded deeply in the rectum. Bowel preparation and intravenous antibiotics followed by colonoscopy using a grasping snare resulted in successful IUD removal. CONCLUSION: Patient

Appropriateness of colonoscopy in a tertiary referral centre.

The aim of this study was to determine the appropriateness of colonoscopy in relation to its diagnostic yield, with reference to the guidelines set by the American Society of Gastrointestinal Endoscopy (ASGE). A prospective 90-day audit was performed at Hospital Kualal Lumpur, which is a tertiary referral centre in Malaysia, to examine the appropriateness of colonoscopy by indication. During that time, 257 colonoscopies were performed in 244 patients. The predominant indications for colonoscopy were altered bowl habit (37%) and rectal bleeding (18%). Of the 257 colonoscopies, 216 (84%) were ju

Consider colonoscopy for young patients with hematochezia.

BACKGROUND: Hematochezia is a common complaint in adult patients aged <50 years. Most studies of lower endoscopy for rectal bleeding have concentrated on older patients or have failed to mention the location of lesions. OBJECTIVE: To determine the findings of complete colonoscopy in adults younger than 50 years with rectal bleeding. METHODS: Data were retrieved from medical records and included demographics, indications, endoscopic findings, and histology. Lesions were labeled according to location: proximal to the splenic flexure or distal to (and including) the splenic flexure. Excluded we
