An early male adolescent with externalizing behaviors, school failure, and a colostomy.

Jeff is a 14 (1/2) years old ninth grader who presents with his mother, father, and step-father because he is failing in school. Jeff was born with an imperforate anus, and he had several surgical procedures in the first few years of life. He has a colostomy, but it is not clear how well he cares for it independently. Jeff's parents state that there other concern is that to prepare for school each morning or for other activities, they literally remind him of each step. Jeff has been active and disruptive since preschool, but is also described as "charming." School work and peer relations impro

Prospective analysis of percutaneous endoscopic colostomy at a tertiary referral centre.

BACKGROUND: Percutaneous endoscopic colostomy (PEC) is an alternative to surgery in selected patients with recurrent sigmoid volvulus, recurrent pseudo-obstruction or severe slow-transit constipation. A percutaneous tube acts as an irrigation or decompressant channel, or as a mode of sigmoidopexy. This prospective study evaluated the safety and efficacy of this procedure at a single tertiary referral centre. METHODS: Nineteen patients with recurrent sigmoid volvulus, ten with idiopathic slow-transit constipation and four with pseudo-obstruction underwent PEC. The tube was left in place indefi

Colostomy for large bowel anomalies in children: a case controlled study.

BACKGROUND: In children, colostomy may be required as a crucial part of treatment of some congenital anomalies of the large bowel. The procedure is associated with significant complications. This study reviews the morbidity and mortality of colostomy formation and closure for large bowel anomalies over a 10-year period in southeast Nigeria. METHODS: Evaluation of 182 colostomies and 146 colostomy closures performed in children at the University of Nigeria Teaching Hospital Enugu from January 1995 to December 2004. RESULTS: Hirschsprung's disease (106) and anorectal malformation (76) were the

[The role of continent perineal colostomy in surgical emergencies of the distal bowel].

Diverting colostomy is commonly required in surgical emergencies of the distal colon, especially if the patient is in poor condition. This paper shows that the colostomy could be unnecessary in most cases. In the last 10 years, 6 patients (4 with high-risk or postoperative complicated Hirschsprung disease, 1 with intestinal neuronal dysplasia and 1 with iatrogenic rectal stenosis obstruction) have been submitted to primary continent perineal colostomy-modified Duhamel's procedure. The modified technique consists of the exteriorization of the normal colon or ileum by a retrorectal and trans-ana

Chest wall peritoneal dialysis catheter placement in infants with a colostomy.

The presence of a colostomy in infants with end-stage renal disease (ESRD) receiving peritoneal dialysis (PD) is associated with an inherent risk for contamination and the development of a PD catheter-associated infection. A two-piece presternal catheter designed to reduce the incidence of such infections has been used in a small number of children, but the implantation of the catheter is technically difficult, and there is a risk of disconnection of the two parts secondary to rapid patient growth in the first year of life. Alternatively, a conventional Swan neck catheter, larger than typicall

The use of colonic irrigation to control fecal incontinence in dogs with colostomies.

OBJECTIVE: To determine if once-daily colonic irrigation results in fecal continence for a 24-hour period in dogs with colostomies and if colonic volume increased in response to the irrigation. STUDY DESIGN: A prospective controlled experimental study. ANIMALS: Four intact male and one intact female mixed breed dogs. MATERIALS: All dogs received left end-on paralumbar colostomies. Four dogs received once-daily colonic irrigation for 8 weeks, whereas the control dog did not. Daily fecal weights were recorded for the length of the study in all dogs. Barium enema studies and volumetric studies

[Percutaneous drainage of peri-sigmoid abscesses or how to avoid colostomy].

BACKGROUND: Percutaneous drainage can be a conservative option for abscess formation subsequent to acute inflammation of the sigmoid colon. CASE REPORTS: Three patients, aged 36, 65 and 77 years, were hospitalized for abscesses in the peri-sigmoid region. All three were treated with echoguided percutaneous drainage. The infectious phenomena regressed rapidly allowing secondary left colectomy 6 to 8 dais later with immediate colorectal anastomosis. DISCUSSION: Hartman's resection is indicated for perforated diverticules of the sigmoid colon with formation of pelvic abscess and must be followe

[Complications of colostomies. Follow-up study of 500 colostomized patients].

The complications of colostomies may constitute a handicap for patients: their prevalence severity and methods of treatment remain poorly known. 500 colostomy patients, with a mean age of 66 +/- 14 years, were retrospectively reviewed. The mean follow-up of the study was 6 +/- 5 years. Colorectal cancers represented 65% of the initial diseases. 59.5% of colostomies were terminal. They were performed for resection of the colon and or rectum in 56.5% of cases. 30.5% of patients (n = 152) presented complications (n = 235). The early complications (n = 147) observed in 29.5% of patients were mostl

Management of perianal squamous cell carcinoma with permanent colostomy in a mare.

A 28-year-old Pinto mare was evaluated for multiple recurrences of a perianal squamous cell carcinoma. Clinical signs included lethargy, inappetence, weight loss, dyschezia, and hematochezia. The tumor involved the proximal portion of the vulvar labia, the perineum, and the anus, and there was a rectal fistula just left of the anus. Permanent end-on colostomy without further treatment of the carcinoma was the management option chosen. The colostomy functioned satisfactorily, and the mare's condition was stable for the next 14 months. Recurrence of lethargy and inappetence and development of we

[Pseudo-continent perineal colostomies after amputation of the rectum for cancer].

From February 1989 to February 1982, 23 pseudocontinent perineal colostomies (PC), performed after abdominoperineal excision for rectal carcinoma, were evaluated. Perineal colostomy was performed using a free autotransplant of smooth muscle, according to Schmidt, associated with colonic irrigations. This procedure was proposed to the younger and more alert patients without advanced rectal carcinoma. These 23 cases represented 35% of the rectal extirpations performed during the same period. Four patients did not accept a PC and preferred, after being fully informed about both types of colostomi

Perineal gangrene in the patient with granulocytopenia: the importance of early diverting colostomy.

Perirectal and perineal infections represent a life-threatening complication of hematologic diseases associated with granulocytopenia. Early and aggressive surgical management is essential. This should include hemodynamic support and monitoring, broad-spectrum antibiotics, wide surgical debridement, and a totally diverting colostomy. The prognosis of these infections parallels that of the underlying hematologic disease. Three patients seen on a university surgical service whose clinical management and course underscore these principles are presented.

Management of a subcutaneous colostomy perforation. The role of a new synthetic skin.

Colostomy perforation is an infrequent but often disastrous and lethal complication. In the majority of patients, the traumatic perforation occurs during irrigation through the colostomy stoma. This case report reviews the clinical course of a patient with a subcutaneous colostomy perforation and the subsequent development of an extensive abscess. Aspects of the management included mobilization of the colostomy and thorough surgical debridement and drainage. In addition, the report introduces the use of the new semisynthetic biologic dressing, BioBrane. This synthetic, semipermeable skin subst

Necessity for end colostomy with transection of the descending colon in guinea pigs.

Surgical procedures for resection and transection of the descending colon in the guinea pig were studied. Wedge resection with removal of less than 1 cm diameter of tissue resulted in an 80% (8 of 10 guinea pigs) survival rate. Colonic transection with restorative anastomosis, either end-to-end or side-to-side, resulted in a 94% (35 of 37 guinea pigs) death rate due to colonic impaction proximal to the site of anastomosis. A modified procedure for ventral end colostomy resulted in a 79% (19 of 24 guinea pigs) survival rate. These results indicated that caution should be taken in surgical manip

Surgery camp for Colostomy reversals at a referral hospital in Lilongwe, Malawi.

AIM: An end colostomy is a potentially life-saving surgical intervention, but postoperative ostomy management is challenging in resource-limited settings. Socioeconomic, health system, and surgical capacity barriers may delay colostomy reversal. A surgery camp model for addressing the burden of unreversed colostomies has not previously been undertaken in Malawi. The study aims to present our institution's experience with the surgery camp model, assess patient outcomes, and identify improvement strategies for future efforts. METHODS: The surgery department at Kamuzu Central Hospital (KCH) carr

A technique for laparoscopic extraperitoneal colostomy with an intact posterior sheath of rectus.

Regardless of the advances in surgical techniques, parastomal hernia is still an inevitable complication for many patients with low rectal cancer undergoing abdominal perineal resection (APR). Extraperitoneal colostomy (EPC) seems to be a effective method to reduce the risk of parastomal hernia. We propose a new approach to simplify and standardize laparoscopic EPC to make this operation easy to perform. We used the technique of laparoscopic TEP groin hernia repair to produce an extraperitoneal tunnel, which can not only facilitate precise visualization of the extraperitoneal tunnel but also u

Battery Ingestion with Colonic Perforation after Colostomy Closure in a Toddler.

Disc and button battery ingestion in children is common. In fact, data reports a dramatic increase in battery ingestion during the coronavirus disease 2019 pandemic likely as a result of increased household population density and electronic product utilization. These batteries often remain lodged in the esophagus causing potentially devastating complications if they are not removed urgently. Batteries that are passed beyond the esophagus usually do not cause any complications. We present the case of a 15-month-old male who underwent a colostomy takedown 2 months following a posterior sagittal
