Optimising co-design methodologies for people with communication disorders following neurological impairment: establishing criteria, adapting practices, and enhancing inclusivity.

BACKGROUND: Co-design methodologies, which actively involve interest-holders in intervention development, are increasingly used in health care. In brain injury rehabilitation, they offer potential for tailored support for individuals with communication disabilities. However, the diversity of approaches and lack of communication-specific adaptations can limit their effectiveness and authenticity in equitably engaging interest-holders with communication disability. METHODS: This meta-synthesis reviewed 30 studies involving adolescents (aged ≥12 years) and adults with acquired brain injury affec

Patients with functional gastrointestinal disorders-importance of communication between physician and patient assessed in a cross-sectional cohort study.

Functional gastrointestinal disorders are frequent diseases often associated with a pronounced burden reflected in a greatly reduced quality of life. Patients are seeking medical help but may be perceived as demanding and challenging. For successful diagnosis and treatment of these patients, a good doctor-patient communication is key. However, so far, only few studies focus on the physicians' perspective of the doctor-patient communication. The present study cross-sectionally investigated 520 physicians using the validated difficult doctor-patient relationship questionnaire and the treatment s

Validation of the ISTH/SSC bleeding assessment tool for inherited platelet disorders: A communication from the Platelet Physiology SSC.

BACKGROUND: Careful assessment of bleeding history is the first step in the evaluation of patients with mild/moderate bleeding disorders, and the use of a bleeding assessment tool (BAT) is strongly encouraged. Although a few studies have assessed the utility of the ISTH-BAT in patients with inherited platelet function disorders (IPFD) none of them was sufficiently large to draw conclusions and/or included appropriate control groups. OBJECTIVES: The aim of the present study was to test the utility of the ISTH-BAT in a large cohort of patients with a well-defined diagnosis of inherited platelet

Metacognition in speech and language therapy for children with social (pragmatic) communication disorders: implications for a theory of therapy.

BACKGROUND: Metacognition is a significant component of complex interventions for children who have developmental language disorders. Research into how metacognition operates in the content or process of developmental language therapy delivery is limited. Identification and description of proposed active therapy components, such as metacognition, may contribute to our understanding of how to deliver complex communication interventions in an optimal manner. AIMS: To analyse aspects of metacognition during therapy derived from a manualized speech and language intervention (the Social Communicat

Evidence-based decision-making in the treatment of speech, language, and communication disorders in Down syndrome; a scoping review.

This scoping review examined available scientific evidence according to the PRISMA-ScR guideline on the subject of treatment interventions by speech and language therapists of speech, language, and communication needs in people with Down syndrome. A literature search in PubMed, Embase, Cinahl, Cochrane, and Web of Science yielded 41 studies suitable for inclusion. All studies examined the effect of an intervention in speech, language or communicative behaviour, alone or in combination. Most studies were conducted among children, only four were conducted among adults. Twenty-three studies (49%)

Tasks performed by parents to enable telepractice for children with communication disorders: an interview study with clinicians and parents.

PURPOSE: Current service models in childhood rehabilitation promote the active participation of parents/caregivers in their children's therapies. The existing literature provides a limited understanding of the tasks and responsibilities parents undertake in their children's therapies, especially over telepractice. This study describes the tasks undertaken by parents in their children's speech therapy delivered virtually during the COVID-19 pandemic. METHODS: A qualitative descriptive study was conducted with parents and speech-language pathologists, using open-ended interviews. Interviews wer

Restless legs syndrome is associated with DSM-IV major depressive disorder and panic disorder in the community.

The authors examined the association between restless legs syndrome (RLS) and DSM-IV major depressive disorder and panic disorder based on Wave III and IV of the Baltimore ECA follow-up study. Of 1071 participants, 1024 completed the RLS Questionnaire and Diagnostic Interview Schedule. Adjusted odds ratio for diagnosis of major depressive disorder (4.7, 95% confidence interval [1.6, 14.5]) and panic disorder (12.9 [3.6, 46.0]) and comorbidity of major depressive disorder and panic disorder (9.7 [1.4, 69.0]) in the past 12 months suggested a strong association between restless legs syndrome and

Gambling disorders among homeless persons with substance use disorders seeking treatment at a community center.

OBJECTIVES: Epidemiological research suggests that homelessness is a risk factor for elevated levels of psychiatric comorbidity and other health risks. This study examined the prevalence of disordered gambling and its association with treatment of psychiatric and substance use disorders among a cohort of homeless people seeking treatment at a community services program. METHODS: Between 1998 and 2000, intake workers evaluated the level of gambling disorder among 171 consecutive homeless persons with substance use disorders who sought treatment at the Moving Ahead Program in Boston. Overlapping symptoms of attention deficit hyperactivity disorder (ADHD), autism, and personality disorder in a community sample.

OBJECTIVES: Attention deficit hyperactivity disorder (ADHD), autism, and personality disorder (PD) often present with overlapping symptom profiles, complicating diagnostic assessment and treatment planning. Yet, few studies have examined the extent and nature of these overlaps, or the mechanisms that may underpin them. The current study aimed to address this gap by investigating overlapping symptom profiles of ADHD, autism, and PD, and the potential transdiagnostic mechanisms contributing to such in a community sample. DESIGN AND METHODS: A total of 512 adults with a history of mental health

Obsessive-compulsive disorder and the risk of subsequent mental disorders: A community study of adolescents and young adults.

BACKGROUND: Comorbidity of obsessive-compulsive disorder (OCD) with other mental disorders has been demonstrated repeatedly. Few longitudinal studies, however, have evaluated the temporal association of prior OCD and subsequent mental disorders across the age period of highest risk for first onset of mental disorders. We examined associations between prior OCD and a broad range of subsequent mental disorders and simulated proportions of new onsets of mental disorders that could potentially be attributed to prior OCD, assuming a causal relationship. METHODS: Data from 3,021 14- to 24-year-old

Peptic ulcer and mental disorders among adults in the community: the role of nicotine and alcohol use disorders.

OBJECTIVE: Previous studies have documented links between peptic ulcer disease (PUD) and mood and anxiety disorders among adults in the community. Several substance use disorders (e.g., nicotine and alcohol dependence) are highly comorbid with mood/anxiety disorders and have been also linked with PUD. No previous study has examined the potentially explanatory role of substance use disorders in the link between mood and anxiety disorders and PUD. The objective of the study is to examine relationships between a range of mental disorders and PUD among adults in the United States and to examine th

Performing Large-Scale Genetic Analysis in the Bleeding Disorders Community.

Inherited bleeding disorders encompass a diverse group of conditions caused by genetic defects affecting coagulation factors, fibrinogen, von Willebrand factor, or platelet function. Despite major advances in quantitative and functional laboratory assays, a substantial diagnostic gap remains, particularly in patients with mild or atypical bleeding phenotypes. Genetic testing has become an important tool to complement traditional phenotypic testing, allowing for precise molecular characterisation and improved classification across the spectrum of bleeding disorders. This review summarises the r

Alcohol use disorder in community management of chronic liver diseases.

Rising rates of alcohol use disorder (AUD) combined with increases in alcohol-related liver disease (ALD) and other liver disease have resulted in the need to develop alcohol management strategies at all levels of patient care. For those with pre-existing liver disease, whether ALD or others, attention to alcohol use treatment and abstinence becomes critical to avoiding worsening liver-related consequences. Modalities to help patients reduce or stop alcohol include screening/brief intervention/referral to treatment, various therapeutic modalities including cognitive behavioral therapy, motivat
