Role and outcomes of community health workers in HIV care in sub-Saharan Africa: a systematic review.

INTRODUCTION: The provision of HIV treatment and care in sub-Saharan Africa faces multiple challenges, including weak health systems and attrition of trained health workers. One potential response to overcome these challenges has been to engage community health workers (CHWs). METHODOLOGY: A systematic literature search for quantitative and qualitative studies describing the role and outcomes of CHWs in HIV care between inception and December 2012 in sub-Saharan Africa was performed in the following databases: PubMed, PsychINFO, Embase, Web of Science, JSTOR, WHOLIS, Google Scholar and SAGE j

Strengthening vaccine uptake: a qualitative assessment of community health worker educational resource needs and community perspectives on vaccination in Western Kenya.

INTRODUCTION: Vaccination strengthens health systems by preventing the spread of infectious diseases and reducing morbidity and mortality. The introduction of the malaria vaccine in Kenya, alongside the growing access to technological tools, offers a timely opportunity to explore the educational needs of community health workers (CHWs) and the feasibility of digital training and health education resources for CHWs. METHODS: A qualitative descriptive study was conducted among CHWs and community members in two sub-counties in Migori County. In-depth interviews were held with 20 CHWs selected th

Addressing Tobacco Use in Underserved Communities Outside of Primary Care: The Need to Tailor Tobacco Cessation Training for Community Health Workers.

Individuals from communities with a low socioeconomic status have the highest rates of tobacco use but are less likely to receive assistance with quitting. Community health workers (CHWs) are well-positioned to engage these communities; however, CHWs face barriers in receiving relevant tobacco cessation training. The objective of this study was to conduct a mixed methods needs assessment to describe tobacco practices and the desire for training among CHWs. After incorporating CHW feedback, we developed a needs assessment survey to understand knowledge, practices, and attitudes about tobacco ce

Community Health Workers in the Veterans Administration: Exploring Opportunities and Barriers to Integration for Rural and Caregiver Populations.

BACKGROUND: Rural Veterans and caregivers face challenges related to health care access, care coordination, and institutional trust-areas where Community Health Workers (CHWs) are well positioned to make an impact. Despite their potential, CHW integration into complex federal systems such as the US Department of Veterans Affairs (VA) remains limited. CHWs represent a promising strategy to enhance access and care continuity for rural Veterans and caregivers. OBJECTIVES: To examine perceptions of community and institutional partners regarding integration of CHWs into the VA to support rural Vet

Barriers to and facilitators of the implementation of Community Health Worker programmes in high-income countries: a systematic review.

BACKGROUND: Global interest in Community Health Workers (CHWs) is rising as health systems seek to address persistent health inequities and the social determinants of health. However, in high-income countries (HICs), CHW implementation remains fragmented. This systematic review synthesises evidence on the factors that act as barriers and facilitators to the implementation and integration of CHW programmes across HIC primary healthcare settings. METHODS: We searched MEDLINE, Scopus, PsycINFO, and CINAHL for studies published between January 2001 and August 2025. Inclusion focused on qualitativ

IMPORTANCE: Community health workers (CHWs) improve chronic disease management in underserved populations, but scalable integration strategies are limited. OBJECTIVE: To evaluate whether a multidimensional intervention incorporating telementored CHWs and a structured participant-CHW-clinician feedback loop can improve diabetes outcomes. DESIGN, SETTING, AND PARTICIPANTS: This 12-month randomized clinical trial was conducted at 3 institutionally and geographically diverse community clinics in Texas between September 1, 2023, and April 30, 2025, and included low-income, uninsured White Hispani

Blending insights and evidence: Co-creating a community health worker intervention for postpartum patients with preeclampsia.

BACKGROUND: Preeclampsia is associated with an increased risk of future maternal cardiovascular disease and death. The American Heart Association (AHA) advocates for embedding place-based interventions within communities to reduce long-term cardiovascular health consequences. Yet a significant gap remains in proven strategies for postpartum risk reduction. Our objective was to design a pilot intervention that is now being tested in a randomized controlled acceptability and feasibility study. METHODS: We leveraged our existing partnerships with a nonprofit community health worker (CHW) organiz

Identifying Who Benefits the Most from a Community Health Worker-Led Multicomponent Intervention for Hypertension.

BACKGROUND: Uncontrolled hypertension is a major public health challenge in low- and middle-income countries. The Hypertension Control Program in Argentina (HCPIA) showed that a community health worker-led multicomponent intervention was effective for blood pressure (BP) reduction in resource-limited settings, but whether the intervention was equally effective across participant subgroups is unknown. OBJECTIVE: To identify participants who benefit the most from the HCPIA BP control intervention. METHODS: This secondary analysis used data from HCPIA, a successful 18-month cluster-randomized t

A Pragmatic Cluster-Randomized Trial of Provider Education and Community Health Worker Support for Tobacco Cessation.

OBJECTIVE: Individuals with serious mental illness have a high prevalence of tobacco use disorder and related early mortality but underutilize smoking cessation medication. The authors determined whether clinician-delivered education to primary care providers regarding safety, efficacy, and importance of cessation medication (provider education [PE]) alone or combined with community health worker (CHW) support would increase tobacco abstinence in this population, compared with usual care. METHODS: All adult current tobacco smokers receiving psychiatric rehabilitation for serious mental illnes

Patient-level Predictors of Extent of Exposure to a Community Health Worker Intervention in a Randomized Controlled Trial.

OBJECTIVE: Community health worker (CHW) interventions have been cited as a best practice for reducing health disparities, but patient-level attributes may contribute to differential uptake. We examined patient characteristics associated with the extent of exposure to a CHW coaching intervention among a predominantly low-income, African American population participating in a randomized controlled trial of hypertension interventions. DESIGN: We conducted a within-group longitudinal analysis of those receiving a CHW intervention from a study conducted between September 2003 and August 2005. We

Building Bridges: A Case for Community Health Worker Provision of Misoprostol-Only Abortion in the First Trimester.

INTRODUCTION: The consequences of unsafe abortion are devastating to women, their families, and their communities. Medication abortion presents an important alternative to harmful self-induction practices and surgical intervention under questionable circumstances. In settings where mifepristone is unavailable, the use of misoprostol alone is a safe and effective option for terminating an unwanted pregnancy. Studies have demonstrated the safety and efficacy of administration of misoprostol by community health workers (CHW) for indications such as postpartum hemorrhage and treatment of incomplet

Use of community health workers for management of malaria and pneumonia in urban and rural areas in eastern Uganda.

Use of community health workers (CHWs) has been implemented the same way in urban and rural areas despite differences in availability of health providers and sociodemographic characteristics. A household survey was conducted in rural and urban areas in eastern Uganda, and all children who were febrile in the previous two weeks were assessed for their symptoms, treatment received at home, and when and where they first went for treatment. Rural children were more likely to use CHWs than urban children. Urban children received outside treatment more promptly, and used herbs at home less. Symptoms

Cost-Effectiveness of Community Health Worker Interventions for Type 2 Diabetes Mellitus: A Systematic Review.

BACKGROUND: Type 2 diabetes contributes substantially to healthcare costs, disability, and premature mortality worldwide. Although community health worker (CHW) interventions improve glycemic outcomes and address social determinants of health, current cost-effectiveness evidence has not been systematically synthesized. We reviewed the economic evidence for CHW interventions in type 2 diabetes management. METHODS: We searched Ovid MEDLINE, Scopus, CINAHL, and ProQuest from inception through August 2025 for peer-reviewed primary studies evaluating economic outcomes of CHW interventions for adul

Community Health Worker-Integrated Model for Sickle Cell Disease Management: Protocol for a Feasibility Study.

BACKGROUND: Sickle cell disease (SCD) is a chronic inherited blood disorder that disproportionately affects African Americans. Managing SCD is complex, often requiring coordination of care among multiple health care professionals. These care challenges result in fragmented service delivery and poor health outcomes. Integrating community health workers (CHWs) into patient-centered coordinated care may enhance disease management and patient satisfaction with the care received. OBJECTIVE: The proposed study aims to test the feasibility and acceptability of a CHW-integrated model for SCD manageme

Decision-analytic models in the economic evaluation of community health worker programmes globally: a systematic review.

INTRODUCTION: Economic evidence on community health worker (CHW) programmes is crucial for scaling these initiatives. Although decision-analytic models (DAMs) are essential for projecting long-term value, it is unclear how rigorously they have been applied to CHW evaluations, potentially compromising the reliability and comparability of cost-effectiveness estimates used for policy decisions. METHODS: A systematic review was conducted to identify full economic evaluations of CHW-led or CHW-integrated interventions that employed a DAM. Following Preferred Reporting Items for Systematic Reviews

Examining theoretical mediators in a culturally adapted, community health worker-led intervention for depression and anxiety.

OBJECTIVE: Emerging research suggests that mental health interventions led by community health workers (CHW) are effective and increase access to services for minoritized groups. However, the question of how these interventions work has rarely been tested. In this secondary analysis of data from a randomized clinical trial of a CHW-led intervention that included training in mindfulness and encouraged active engagement and self-efficacy in managing mental health care, we aimed to examine several mediators of change based on theoretical mechanisms of action. METHOD: A sample of 1,044 racially a

Pilot deployment of a community health care worker in distributing and offering the COVID-19 AgRDT in Tanzania.

A pilot implementation of the rapid diagnostic test program was performed to collect evidence of the feasibility, acceptability, and uptake of the COVID-19 AgRDT in Tanzania. We conducted a prospective cross-sectional study in the community to provide quantitative details of the pilot implementation of the antigen rapid diagnostic test (AgRDT) in Tanzania. This study was undertaken between March 2022 and September 2022. The pilot was implemented by distributing and offering test kits to people suspected of having COVID-19 in Dar es Salaam through community health workers. A total of 1039 parti
