Virtual Care: Evidence From the Pandemic Regarding Changes in Access to Psychiatric Care in Canada.

The COVID-19 pandemic disrupted access to clinical services, prompting an easing of restrictions on accessing care virtually. While virtual care can substantially differ from in-person care in most specialties, this is minimally true of psychiatry, making it ideal for examining the impact of virtual delivery on access. This paper shows that those already facing access barriers faced new ones during the pandemic. The findings - based on Canadian Institute for Health Information data - highlight disparities in mental healthcare access for low-income individuals, children and adolescents, seniors

Neurosyphilis with psychotic symptoms and Parkinsonism in a young girl.

A 15-year-old girl with neurosyphilis was misdiagnosed as having viral encephalitis with psychotic symptoms and Parkinsonism. We found that she was experiencing visual hallucinations, persecutory delusions, flattening of affect, poorness of thought, tremors, four-limb rigidity, and restlessness, and she was unable to communicate with others. The Venereal Disease Research Laboratory serum test and further lumbar puncture enabled us to diagnose her with neurosyphilis. After antibiotic treatment, her psychotic symptoms and Parkinsonism were relieved. From this case, we believe that it is importan

Impact of Repeated Questioning on Interviewers: Learning From a Forensic Interview Training Project.

Forensic interviewers have a difficult job with high risk for career burnout and secondary trauma. Few studies have addressed how new forensic interviewers or trainees experience repeated questioning and multiple interviews. This study simulated the process of training new forensic interviewers through the creation of two interview videos in which social work graduate students participated as actors portraying the roles of interviewer and child. These films served as instructional aids preparing graduate social work students for professional child welfare roles while promoting research-based a

Reduced Activity in an Inpatient Liaison Psychiatry Service During the First Wave of the COVID-19 Pandemic: Comparison With 2019 Data and Characterization of the SARS-CoV-2 Positive Cohort.

Background: The COVID-19 pandemic led to changes in the way that healthcare was accessed and delivered in the United Kingdom (UK), particularly during the peak of the first lockdown period (the "first wave") beginning in March 2020. In some patients, COVID-19 is associated with acute neuropsychiatric manifestations, and there is suggestion that there may also be longer term neuropsychiatric complications. Despite this, at the time of writing there are only emerging data on the direct effects of the COVID-19 pandemic on psychiatric care. Methods: In this retrospective study we analyzed referral

Global Adversities, the Media, and Mental Health.

Global communities are currently confronted with a number of complex problems and threats, the reality of which is amplified by the media. These environmental and socio-political stressors have been accompanied by the spread of problematic psychological and behavioural tendencies, such as the growing polarisation of opinions and values, online radicalisation and extremism, deepening xenophobia and nationalism, the proliferation of irrational beliefs and conspiracy theories, and resistance to rational public policy measures. Here we argue that although they fall outside the scope of psychopatho

An Immersive Simulation to Build Empathy for Geriatric Patients with Co-Occurring Physical and Mental Illness.

OBJECTIVE: The objective of the study was to explore the effectiveness of an immersive simulation experience using an aging simulation suit for fostering empathy towards geriatric patients with advanced mental illness. METHOD: Psychiatry residents were recruited during their clinical rotations at a Canadian mental health hospital. The participants took on the first-person perspective of a geriatric patient with mental illness initially through written reflection, and then physically inhabited this role by wearing an aging simulation suit to perform the task of meeting with a pharmacist to rev

A synopsis of recent influential papers published in psychiatric journals from the Arab World.

After a brief introduction to the Arab region, six recent and influential papers that have appeared in one of the three leading psychiatry journals from the region are summarized. The first paper discusses the lifetime prevalence of mental disorders in Lebanon and assesses the influence of war. From a representative sample of 2857 individuals they showed that only a minority of individuals with a diagnosable mental disorder have received professional treatment. War exposure increased the risk of first onset of anxiety, mood, and impulse control disorders. The second paper examines the impact o

Sequential Intercept Mapping: Developing Systems-Level Solutions for the Opioid Epidemic.

Sequential intercept mapping, a community-based application of the sequential intercept model, was recently adapted in Ohio to address the complex challenge of the opioid crisis. Sequential intercept mapping for opioids provides a framework for criminal justice, mental health and addictions treatment providers, family members and opioid-involved individuals, and other stakeholders to develop community-based responses that emphasize prevention, regulation, and treatment for opioid dependency, with a goal of reducing unintended deaths and overdoses. The authors describe a promising approach to u

[Expert decisions concerning disability pensions in the area of psychiatry--in view of a court-appointed expert].

Inability of social services to provide a non-working person with the necessary minimal financial support as well as a high unemployment rate make acquiring of a disability pension appear the only means of coming to terms with existential problems. After the fairly tolerant decision policy of the Social Insurance Institution (ZUS) in the first half of the 1990s, the criteria of granting disability pensions hassince been made stricter--through replacing of the former disability pensions by the concept of inability to work. The author evaluates the theory and practice of experts' decision-making

CCNP Innovations in Neuropsychopharmacology Award: The psychopharmacology of psychedelics: where the brain meets spirituality.

For 3000 years, psychedelics have been used in religious contexts to enhance spiritual thinking, well-being, and a sense of community. In the last few years, a renaissance in the use of psychedelic drugs for mental disorders has occurred in Western society; consequently, a pressing scientific need to elucidate the intricate mechanisms underlying their actions has arisen. Psychedelics mainly bind to serotonin (5-HT) receptors, particularly 5-HT2A receptors, but may also bind to other receptors. Unlike conventional psychotropic drugs used in psychiatry, psychedelics introduce a distinctive compl

How men with alcohol use disorder perceive communication, couple satisfaction, relational boredom, and quality of life compared to wives: Findings from a hospital-based study.

In India, use of alcohol between 10 and 70 years is increasing significantly as per the Government of India, Ministry of Social Justice & Empowerment. Chronic alcohol use in men can potentially disrupt their relationships with their wives in several ways, leading to poor communication, trust issues, emotional disconnection, physical abuse, financial strain, and neglecting responsibilities. These factors may reduce the quality of life of the couple and negatively impact the couple's overall well-being. This cross-sectional study assesses the communication, couple satisfaction, relational boredo

Community-based mental health centers in Ukraine - protocol for a mixed methods evaluation study using the RE-AIM framework.

BACKGROUND: Russia's full-scale invasion of Ukraine has deepened the strain on an already fragile mental health system, long shaped by Soviet institutionalized psychiatry. In response, Ukraine has started a national reform to modernize mental health services by shifting from an institutional to a community-based mental health care model. Implementation of his transition is supported by the international technical project Mental Health for Ukraine (MH4U). AIMS: To generate evidence-based evaluation of the community-based mental health centers as a basis for continued improvement and to guide n

Needs assessment in dementia.

BACKGROUND: Resource allocation and service development traditionally focuses on diagnostic categories and consequent perceived need. Identification of the actual level of need in the elderly with dementia, and the degree to which it is unmet is necessary to plan services both individually and as a group. The aim of this study was to characterise the needs of a sample of community dwelling elderly patients with dementia who were referred to an old age psychiatry service in Ireland between July 2002 and July 2003. METHODOLOGY: Eighty-two consecutively referred community dwelling patients with

Treatment of obsessive compulsive disorder and Kratom dependence with buprenorphine and supportive care.

Kratom is an herbal product derived from Mitragyna speciosa which, depending on the dose, has stimulant and opioid-like effects and is increasingly being used recreationally to self-medicate symptoms of anxiety and depression. This case describes a man in his 30 s who presented to a community drug and alcohol service seeking support for problematic Kratom use, which he started using to try and manage obsessive-compulsive disorder symptoms and low mood. Despite attempts to reduce his use, he was unable to discontinue Kratom use independently. Initiation of low-dose sublingual buprenorphine led
