Health promotion in the city: a review of current practice and future prospects in the United States.

To achieve its health goals, the United States must reduce the disproportionate burden of illness and poor health borne by urban populations. In the 20th century, patterns of immigration and migration, changes in the global economy, increases in income inequality, and more federal support for suburbanization have made it increasingly difficult for cities to protect the health of all residents. In the last 25 years, epidemics of human immunodeficiency virus infections and substance abuse and increases in homelessness, lack of health insurance, rates of violence, and concentrations of certain po

Determining successful pain clinic treatment through validation of cost effectiveness.

Initially, the Center for Pain Control designed and implemented multiple measures to indicate treatment efficacy in an outpatient setting. Decreased use of medical care at 1 year after treatment was one of five measures used. Economic concerns, both in the health and the insurance industries, mandated greater emphasis on cost effectiveness at a primary success determinate. Cost effectiveness was measured through broad diagnostic categories: lumbar pain with or without radiculopathy, cervical pain with and without radiculopathy, and a combination of lumbar and cervical pain. Cost for surgical t

Sexually Transmitted Infections at a Health Clinic Serving Older Adolescents and Young Adults: An Analysis of Demographics, Healthcare Access, and Positivity Rates.

OBJECTIVES: Young adults aged 18 to 24 face increased risk for acquiring sexually transmitted infections (STIs) and reduced access to healthcare as they transition into adulthood and assume responsibility for their health. Furthermore, racial, ethnic, gender, and sexual minority young adults are at an increased risk for STIs and are less likely to have health insurance. This study evaluates the demographics of young adults undergoing STI and human immunodeficiency virus (HIV) testing at a sexual health clinic, and the impact of healthcare access and insurance status on the likelihood of STI di

Improving Access to Eye Care: A Systematic Review of the Literature.

PURPOSE: The goals were to develop a working and inclusive definition of access to eye care, identify gaps in the current system that preclude access, and highlight recommendations that have been identified in prior studies. This manuscript serves as a narrative summary of the literature. CLINICAL RELEVANCE: Health care disparities continue to plague the nation's well-being, and eye care is no exception. Inequities in eye care negatively affect disease processes (i.e., glaucoma, cataracts, diabetic retinopathy), interventions (surgical treatment, prescription of glasses, referrals), and popul

Financial arrangements for rehabilitation services in health systems: an overview of systematic reviews.

BACKGROUND: Cochrane Rehabilitation and the World Health Organization (WHO) Rehabilitation Programme have collaborated to produce four Cochrane overviews of systematic reviews that synthesize current available evidence from health policy and systems research (HPSR) in rehabilitation. Each overview focuses on one of the four pillars of HPSR as identified by the Cochrane Effective Practice and Organisation of Care (EPOC) taxonomy: delivery arrangements, financial arrangements, governance arrangements, and implementation strategies. This overview focuses on financial arrangements, which Cochrane

A Pragmatic Trial of a Multilevel Community-Centered Intervention to Reduce Pregnancy Related and Associated Morbidity and Mortality and Disparities: A Study Protocol.

INTRODUCTION: Maternal morbidity and mortality in the US disproportionately affect some women, including the Medicaid-insured, rural, and Black, Hispanic, and Native American populations. The drivers of disparities are complex and there is an urgent need for multilevel interventions focused on pregnancy-related and associated morbidity and mortality (PRAMM). This study will be the first large-scale test of Community Health Worker (CHW) inclusive home visiting and provider/practice/system level improvement strategies that support empowered patients, pro-active providers, and integrates communit

Rethinking Palliative Care Through Three Institutional Ethnographic Stories of People Living With Homelessness and Life-Limiting Illness.

Fifteen to thirty percent of Canadians have access to palliative care, with even fewer access opportunities for people with experiences of homelessness. Existing research identifies barriers to access but rarely shows how health and social institutions actively organize exclusion. Part of a larger study, this paper examines how health and social systems shape the need for community-based palliative and end-of-life care, using 3 stories from clients of the Community Allied Mobile Palliative Partnership (CAMPP). Using institutional ethnography, data were collected between Fall 2019 and Summer 20

Sociodemographic correlates of cervix, breast and oral cancer screening among Indian women.

INTRODUCTION: Cervix, breast and oral cancers account for about one-third of all cancers in India which as a group is a major contributor to all non-communicable disease-related morbidity and mortality among women. Existing evidence suggests that early diagnosis plays a pivotal role in the prevention and intervention of these cancers, and many community-based early screening and awareness programs have been in place in developed countries. Currently, there is not enough research evidence regarding the sociodemographic correlates of cervix, breast and oral cancer screening among Indian women. I

Same Average, Different Odds: Maternal-health Lessons for India from Rwanda and Peru.

BACKGROUND: India has reduced its maternal mortality ratio faster than the global average, reaching 93 per 100,000 live births in 2019-21 and 88 in 2021-23. The national figure hides a nearly nine-fold gap between the best and worst-performing states. Deaths that remain fall mainly on rural, tribal, and poor women, in whom the three classic delays in obtaining care overlap with poverty, distance, caste, and gender. POLICY AND IMPLICATIONS: The most useful lessons for India come not from wealthy systems but from countries that started poorer and still moved faster on equity. Rwanda combined a

Disability Prevalence and the Awareness-Utilization Gap for Disability Services in an Urban Community of Delhi, India: A Cross-Sectional Study.

INTRODUCTION: Disability-related welfare schemes form a critical component of India's social protection architecture, yet community-level evidence on the gap between awareness and utilization in urban settings remains limited. This study aimed to estimate the prevalence of disability and its socio-demographic correlates and to assess awareness and utilization of disability-related services among adults in an urban community of Delhi, generating baseline data on the awareness-utilization gap for this setting. METHOD: A community-based cross-sectional study was conducted in Palam, an urban rese

The impact of digital interventions on health insurance coverage for reproductive, maternal, newborn and child health services utilization in Kakamega, Kenya: a cluster randomized controlled trial.

The National Hospital Insurance Fund (NHIF) of Kenya was upgraded to improve access to healthcare for impoverished households, expand universal health coverage, and boost the uptake of essential reproductive, maternal, newborn and child health (RMNCH) services. However, premiums may be unaffordable for the poorest households. The Innovative Partnership for Universal Sustainable Healthcare (i-PUSH) programme targets low-income women and their households to improve their access to and utilization of quality healthcare, including RMNCH services, by providing subsidized, mobile phone-based NHIF co

Association between social determinants of health and systemic lupus erythematosus: a nationally representative analysis of 2017-2021 data.

Limited US recent data is available on the prevalence of systemic lupus erythematosus (SLE) by patient's social determinants of health (SDOH). Careful assessment of individual SDOH that affects SLE is crucial, as such evidence could help improve care and hence reduce health disparities for patients with SLE, especially for those who are most vulnerable and at the highest risk of poor outcomes. We estimated the prevalence of systemic lupus erythematosus (SLE) overall and by patient's social determinants of health (SDOH), and also explored the associations between SDOH and SLE. We conducted a po
