PURPOSE OF REVIEW: The purpose of the present review is to describe currently available tools for the diagnosis of allergy with ocular involvement, and to define a simple and rapid diagnostic algorithm to facilitate identification of the various clinical types of ocular allergic diseases.
RECENT FINDINGS: Evidence indicates that all clinical forms of allergic conjunctivitis share several common features including signs, symptoms and biochemical allergy markers. Nevertheless, to date there are no standardized criteria available to general practitioners and allergists that would allow them to c

Conjunctivitis, or inflammation of the conjunctiva, refers to a diverse group of ocular surface diseases of viral or bacterial origin that primarily affect the conjunctiva. In developed countries, the most common causative bacterial pathogens are Staphylococcus aureus, Haemophilus influenzae, and Streptococcus pneumoniae. Most varieties of conjunctivitis are self-limiting; however, some cases can be extremely contagious or cause serious complications if left unchecked. New ocular antibiotics are needed to keep pace with the increasing incidence of bacterial resistance and provide options that

BACKGROUND: Punctal plugs are used for the treatment of tear-deficient type dry eye. We recently examined a case of acute conjunctivitis associated with bacterial biofilm formation on a punctal plug.
CASE: A 63-year-old woman diagnosed as having tear-deficient type dry eye came to our hospital with a complaint of soreness in her right eye. Punctal plugs had been inserted into this eye 5 1/2 months previously. On the day of her visit, she presented with acute conjunctivitis.
OBSERVATIONS: In biomicroscopical examination, the top of the punctal plug was seen to be covered with a whitish soft m

Conjunctivitis is a very common presentation to general practitioners and general paediatricians. The investigation of conjunctivitis can be a significant cost to microbiology laboratories due to the high volume of samples that can be submitted, particularly from patients in the community. The key issue is to send eye swabs in clinical situations where it can make a difference to management, and limiting the use of eye swabs in routine cases of conjunctivitis which are likely to be due to viruses. For investigation of neonatal conjunctivitis we recommend sending a bacterial swab for routine cu

BACKGROUND: Infective conjunctivitis is common and mostly responds well to supportive management and/or empiric antibiotic treatment. At times the differential diagnosis includes more serious conditions that potentially threaten vision. Recurrence and treatment failures are relatively common.
OBJECTIVE: To outline the usual clinical appearance of conjunctivitis and its management. To explore the clinical appearance (with the help of case studies) of conditions that have been mistaken for conjunctivitis, and to consider some of the conditions that may coexist that contribute to recurrence and

Neisseria meningitidis represents an uncommon pathogen of acute bacterial conjunctivitis. In this brief report, we describe a case of meningococcal conjunctivitis in an immunocompetent adult male, with a review of the literature. The patient went to the outpatient ophthalmology clinic complaining of severe ocular discomfort, burning, and redness for more than 2 weeks and, at slit lamp examination, he was diagnosed with a mild conjunctivitis. Microbiology cultures of ocular swabs revealed the growth of colonies, as pure culture, identified as N. meningitidis of serogroup B. A diagnosis of prima

BACKGROUND: Millions of acute conjunctivitis cases occur in the United States annually. The impact of COVID-19 mitigation practices on viral conjunctivitis incidence within ophthalmology clinics has not been reported. We hypothesized that viral conjunctivitis rates would decrease with implementation of such practices.
METHODS: A retrospective chart review was conducted at a single academic center's ophthalmology clinics. Electronic health record data was queried using ICD-10 diagnostic codes to include 649 patients aged 2-97 with viral, bacterial, or allergic conjunctivitis diagnosed either b

Ophthalmia neonatorum is the most common eye infection in the first month of life. The etiologies have been attributed to chemical, bacterial, and viral agents. Neonatal bacterial conjunctivitis is caused by many species of bacteria, the most important being Neisseria gonorrhoea and Chlamydia trachomatis. Eikenella corrodens is a Gram-negative bacillus. It is a fastidious, slow growing, facultative anaerobic bacterium that may be found as the normal flora of the human mouth, nasopharynx, gut, and genitourinary tract. Its role in causing infections in humans has been recognized in the last 2 de

In late summer 1981, a widespread outbreak of acute bacterial conjunctivitis occurred in southeast Georgia, affecting primarily grade-school children. This outbreak was similar to previously described seasonal conjunctivitis in the South. As suggested in previous studies, the eye gnat, Hippelates pusio, may serve as a mechanical vehicle in the transmission of the suspected causative agent, a possible Haemophilus species. The 1981 Georgia outbreak illustrates that "gnat sore eyes" is not obsolete and apparently can occur unpredictably during warm months in the southern states. Physicians and ot

Aerobic and anaerobic cultures and clinical data were obtained from 131 patients presenting with acute conjunctivitis. Similar cultures were obtained from 60 noninflamed individuals. Anaerobes were isolated from 66 patients, 51 times in mixed culture with aerobes and 15 times (11.5%) as the only isolates. Aerobes only were recovered in 54 patients. The organisms found to be statistically significantly more commonly recovered from eyes with conjunctivitis were Staphylococcus aureus, Hemophilus influenzae, Peptostreptococcus, and Propionibacterium acnes. No statistical difference was present bet

Corneal and conjunctival biopsies of 13 patients with rheumatoid arthritis and corneal ulceration (RA-keratomalacia) have been characterized by immunohistological and histochemical analysis. Biopsies from 13 patients with bacterial conjunctivitis, 7 patients with allergic conjunctivitis, 15 patients with senile cataract and 15 patients with keratokonus served as controls. The phenotypic composition of the conjunctival inflammatory infiltration of rheumatoid corneal ulceration was not significantly different from the other inflammatory eye diseases studied. However, conjunctival epithelial cell

BACKGROUND: Tuberculous conjunctivitis has been described only rarely during the course of lupus vulgaris. We report a case of hemifacial cutaneous tuberculosis, diagnosed as atypical lupus vulgaris, associated with homolateral fibrosing tuberculous conjunctivitis.
PATIENTS AND METHODS: An 83-year-old woman presented inflammatory conjunctivitis without bullous involvement in the left eye leading to corneal neovascularisation, symblepharons and ptosis. Erythematous and atrophic papules were seen on the left side of the face. Biopsy of the skin and conjunctiva revealed a tuberculoid granulomato

N-Chlorotaurine (NCT) is an endogenous microbicidal oxidant. This open pilot study (phase IIa) with 9 patients was done to gain first knowledge on the tolerance of NCT in infectious conjunctivitis. By application of 1% NCT 5 times a day, no adverse effects could be observed. All 6 subjects with bacterial conjunctivitis were cured within 3-5 days. Two subjects with epidemic keratoconjunctivitis were treated for 7-10 days and 1 subject with herpes simplex blepharitis for 3 days with no rapid improvement but probable mitigation of inflammation. Therefore, NCT seems to be useful in the treatment o

The efficacy and safety of ciprofloxacin ophthalmic solution (0.3%) and fusidic acid gel (1%) were compared in the treatment of bacterial conjunctivitis and blepharitis in a randomized, open, parallel group study. Thirty-nine patients, 21 treated with ciprofloxacin solution and 18 treated with fusidic acid gel, were culture-positive on admission and were evaluable for efficacy. At the end of a 7-day treatment, the infecting organism was eradicated in 81% of those treated with ciprofloxacin and 72% of those treated with fusidic acid gel. There was clinical cure or improvement in 95% and 89% res

A series of eye cultures was obtained in 1961 in three areas on Taiwan from both normal and trachomatous children. About one-half of the cultures were from clinical cases classified as being in one of the various trachomatous stages. The other half were from cases of normal or simple conjunctivitis. Total isolation rates were 36.6% for the trachoma group and 35.2% for the control group. If chronic follicular conjunctivitis and simple conjunctivitis cases were disregarded, that did not affect the ratio of isolations between the two groups. As the temperature and humidity increased from February

OBJECTIVE: To discover the best treatments for acute infective conjunctivitis and to discern whether antibiotics are necessary for the resolution of bacterial conjunctivitis in particular.
QUALITY OF EVIDENCE: MEDLINE, EMBASE, and the Cochrane Database of Systematic reviews were searched. Findings were limited to full-text articles from core clinical journals in the English language, and are based on level I or level II evidence. Clinical Evidence was also searched, from which moderate-quality results have been cited.
MAIN MESSAGE: Infective conjunctivitis should be managed conservatively, w

Background Acute red eye is a common presenting complaint to primary care. Although conjunctivitis is the most common cause, some conditions can be sight-threatening. Diagnosing such conditions can be difficult given the broad differential diagnoses and limited specialist equipment. The National Institute for Health and Care Excellence (NICE) has published five "red flags," which may indicate the need for urgent ophthalmological assessment: reduced visual acuity (VA), copious discharge, marked eye pain/photophobia, contact lens use and recent trauma. For patients not requiring specialist refer
