Understanding, detecting, and stimulating consciousness recovery in the ICU.

Coma is a medical and socioeconomic emergency. Although underfunded, research on coma and disorders of consciousness has made impressive progress. Lesion-network-mapping studies have delineated the precise brainstem regions that consistently produce coma when damaged. Functional neuroimaging has revealed how mechanisms like "communication through coherence" and "inhibition by gating" work in synergy to enable cortico-cortical processing and how this information transfer is disrupted in brain injury. On the cellular level, break-down of intracellular communication between the layer 5 pyramidal

The nature of blindsight: implications for current theories of consciousness.

Blindsight regroups the different manifestations of preserved discriminatory visual capacities following the damage to the primary visual cortex. Blindsight types differentially impact objective and subjective perception, patients can report having no visual awareness whilst their behaviour suggests visual processing still occurs at some cortical level. This phenomenon hence presents a unique opportunity to study consciousness and perceptual consciousness, and for this reason, it has had an historical importance for the development of this field of research. From these studies, two main opposi

The altered state of consciousness induced by Δ9-THC.

Altered states of consciousness (ASC) provide an opportunity for researchers to study the neurophysiological basis of changes in phenomenal experience. Δ9-tetrahydrocannabinol (THC) is the primary psychoactive constituent of cannabis, however whether the effects of THC include ASC features that are shared with other ASC induction mechanisms, such as classical psychedelics, has not been systematically addressed. We used survey (11D-ASC; State Mindfulness), self-report, and natural language processing (NLP) to assess 7.5 and 15 mg oral THC, relative to placebo, in 25 healthy, infrequent cannabis

Shame, Guilt, and Self-Consciousness in Anorexia Nervosa.

BACKGROUND: the role of self-conscious emotions (SCE) such as shame and guilt in eating disorders (ED) has been systematically studied only in recent years, but it is still debated. This study aims to investigate the role of SCE in anorexia nervosa (AN), evaluating the role of self-consciousness. METHODS: fifty-five individuals with AN and seventy-four healthy controls (HC) were enrolled and completed a battery of tests evaluating the proneness to feel shame and guilt, as well as comparing self-consciousness, eating, and general psychopathology. RESULTS: individuals with AN showed a higher p

Migraine with Brainstem Aura Accompanied by Disorders of Consciousness.

Migraine with brainstem aura (MBA) accompanied by disorders of consciousness (DOC) is a rare subtype of migraine. The pathophysiology of MBA with DOC has not been elucidated yet. Some patients have a family history of migraine, and women are more affected than men. The aura symptoms are diverse; however, when MBA is combined with DOC, the clinical manifestations are more complicated. Coma is the most common clinical manifestation. The overall duration of the patient's DOC is short and can often return to normal within half an hour. Headache often occurs after regaining consciousness and can al

Epilepsy in prolonged disorders of consciousness: a systematic review.

To date, no guideline exists for the management of epilepsy in patients with prolonged Disorders of Consciousness (DoC). This review aimed to assess the occurrence of epilepsy and epileptic abnormalities (EA) in these patients, to determine their impact on recovery; and to review the effect of antiepileptic drugs (AED) and therapeutic interventions on seizure occurrence and consciousness recovery. A structured search for studies on prolonged DoC and epilepsy was undertaken following PRISMA guidelines. From an initial search resulting in 5,775 titles, twelve studies met inclusion criteria. The

Foundations of Human Consciousness: Imaging the Twilight Zone.

What happens in the brain when conscious awareness of the surrounding world fades? We manipulated consciousness in two experiments in a group of healthy males and measured brain activity with positron emission tomography. Measurements were made during wakefulness, escalating and constant levels of two anesthetic agents (experiment 1, n = 39), and during sleep-deprived wakefulness and non-rapid eye movement sleep (experiment 2, n = 37). In experiment 1, the subjects were randomized to receive either propofol or dexmedetomidine until unresponsiveness. In both experiments, forced awakenings were

Electromagnetic Brain Stimulation in Patients With Disorders of Consciousness.

Severe brain injury is a common cause of coma. In some cases, despite vigilance improvement, disorders of consciousness (DoC) persist. Several states of impaired consciousness have been defined, according to whether the patient exhibits only reflexive behaviors as in the vegetative state/unresponsive wakefulness syndrome (VS/UWS) or purposeful behaviors distinct from reflexes as in the minimally conscious state (MCS). Recently, this clinical distinction has been enriched by electrophysiological and neuroimaging data resulting from a better understanding of the physiopathology of DoC. However,

Disorders of human consciousness in the Tri-partite synapses.

Conscious Action Theory extends quantum theory to macroscopic phenomena and suggests physical correlates of consciousness occur at the intersection of external measurement signals and internally generated signals from memories that model the outside world. This physical theory predicts conscious phenomena happen at all scales and differ only by the size and complexity of material organizations involved. At the scale of the human "Brain" consciousness is predicted to happen where the processing loop of activity in the Glial network interfaces with the real world input-output processing loop of

Neuroscience: The Key to Consciousness May Not Be under the Streetlight.

Key theories of consciousness predict that the prefrontal cortex (PFC) plays important roles, but there has been relatively little causal evidence showing that manipulation of activity in the region can broadly affect conscious experiences. A new study provides crucial findings to help resolve this issue, showing that direct pharmacological stimulation of PFC restores wakefulness in anesthetized rats.

Degrees of functional connectome abnormality in disorders of consciousness.

Understanding the neuronal basis of disorders of consciousness can help improve the accuracy of their diagnosis, indicate potential targets for therapeutic interventions, and provide insights into the organization of normal conscious information processing. Measurements of brain activity have been used to find associations of the levels of consciousness with brain complexity, topological features of functional connectomes, and disruption of resting-state networks. However, obtainment of a detailed picture of activity patterns underlying the vegetative state/unresponsive wakefulness syndrome an

Mapping the functional connectome traits of levels of consciousness.

Examining task-free functional connectivity (FC) in the human brain offers insights on how spontaneous integration and segregation of information relate to human cognition, and how this organization may be altered in different conditions, and neurological disorders. This is particularly relevant for patients in disorders of consciousness (DOC) following severe acquired brain damage and coma, one of the most devastating conditions in modern medical care. We present a novel data-driven methodology, connICA, which implements Independent Component Analysis (ICA) for the extraction of robust indepe

Probing consciousness in a sensory-disconnected paralyzed patient.

BACKGROUND: Diagnosis of consciousness can be very challenging in some clinical situations such as severe sensory-motor impairments. CASE STUDY: We report the case study of a patient who presented a total "locked-in syndrome" associated with and a multi-sensory deafferentation (visual, auditory and tactile modalities) following a protuberantial infarction. RESULT: In spite of this severe and extreme disconnection from the external world, we could detect reliable evidence of consciousness using a multivariate analysis of his high-density resting state electroencephalogram. This EEG-based diag

Electromyographic decoding of response to command in disorders of consciousness.

OBJECTIVE: To propose a new methodology based on single-trial analysis for detecting residual response to command with EMG in patients with disorders of consciousness (DOC), overcoming the issue of trial dependency and decreasing the influence of a patient's fluctuation of vigilance or arousal over time on diagnostic accuracy. METHODS: Forty-five patients with DOC (18 with vegetative/unresponsive wakefulness syndrome [VS/UWS], 22 in a minimally conscious state [MCS], 3 who emerged from MCS [EMCS], and 2 with locked-in syndrome [LIS]) and 20 healthy controls were included in the study. Waking and dreaming consciousness: neurobiological and functional considerations.

This paper presents a theoretical review of rapid eye movement sleep with a special focus on pontine-geniculate-occipital waves and what they might tell us about the functional anatomy of sleep and consciousness. In particular, we review established ideas about the nature and purpose of sleep in terms of protoconsciousness and free energy minimization. By combining these theoretical perspectives, we discover answers to some fundamental questions about sleep: for example, why is homeothermy suspended during sleep? Why is sleep necessary? Why are we not surprised by our dreams? What is the role

How neuroscience will change our view on consciousness.

Is there consciousness in machines? Or in animals? What happens to consciousness when we are asleep, or in vegetative state? These are just a few examples of the many questions about consciousness that are troubling scientists and laypersons alike. Moreover, these questions share a striking feature: They seem to have been around forever, yet neither science nor philosophy has been able to provide an answer. Why is that? In my view, the main reason is that the study of consciousness is dominated by what we know from introspection and behavior. This has fooled us into thinking that we know what

A network approach to assessing cognition in disorders of consciousness.

OBJECTIVE: Conventional assessments of consciousness rely on motor responses to indicate awareness. However, overt behaviors may be absent or ambiguous in patients with disorders of consciousness (DOC) resulting in underrating capacity for cognition. fMRI during a silent picture-naming task was evaluated as an indicator of command following when conventional methods are not sufficient. METHODS: A total of 10 patients with and without conventional evidence of awareness, who met diagnostic criteria for the minimally conscious state (MCS) (n = 5), vegetative state (VS) (n = 3), emerged from MCS
