Effect of a brief intervention with small financial incentives on alcohol consumption in China: study protocol for a randomised controlled trial.

INTRODUCTION: Alcohol consumption is the seventh leading risk factor for disability-adjusted life years in the world, according to the Global Burden of Disease Study 2017. As the largest developing country, China has a substantial population of alcohol consumers who suffer from related health risks. Despite having made significant advancements in eradicating absolute poverty, many people still live in relative poverty, which suggests that the adverse health effects caused by alcohol consumption among vulnerable populations in China warrant more attention. This paper aims to provide an overview

Unhealthy Alcohol Use.

Unhealthy alcohol use is common and routine screening is essential to identify patients and initiate appropriate treatment. At-risk or hazardous drinking is best managed with brief interventions, which can be performed by any provider and are designed to enhance patients' motivations and promote behavioral change. Alcohol withdrawal can be managed, preferably with benzodiazepines, using a symptom-triggered approach. Twelve-step programs and provider-driven behavioral therapies have robust data supporting their effectiveness and patients with alcohol use disorder should be referred for these se

Randomized, controlled trial of three levels of critical incident stress intervention.

BACKGROUND: Stress debriefing following exposure to a critical incident is becoming more prevalent. Its aim is to prevent or minimize the development of excessive stress response symptoms that lead to loss of productivity or effectiveness in the workplace or at home. There is little evidence that any form of psychological debriefing is effective. This study evaluated the effectiveness of three intervention strategies, and attempted to correlate the symptoms with the severity of the incident and level of intervention. METHODS: A randomized, controlled trial of three levels of critical stress i

A review of post-traumatic stress disorder. Part II: Treatment.

This paper describes the history of the development of understanding of psychological responses to traumatic life-events and their treatment. One major response, post-traumatic stress disorder (PTSD), is a recognized condition which has appeared relatively recently in diagnostic manuals. PTSD is a condition of major significance, not only to mental health professionals, but also to trauma surgeons and allied professionals. This paper focuses on the current definition of PTSD in the International Classification of Diseases (ICD-10, 1992, World Health Organization) and the Diagnostic and Statist

[Child abuse--a pediatric emergency?].

Many cases of child abuse mark the culmination of a complex psychosocial process and proceed in a chronic way. An acute intervention in these chronic cases is not only inefficient, but also counterproductive; only a careful evaluation by a multidisciplinary team that pays attention to all circumstances and a stepwise action following a generally agreed plan will meet with success. This type of proceeding applies especially for cases of chronic sexual abuse that originates from close family range. A model of the procedure is presented. Real emergencies in the contest of child abuse have therape

Nightmares in crisis: clinical applications of lucid dreaming techniques.

A patient in crisis was offered treatment with a major focus on alleviating nightmares using lucid dreaming (dreaming while knowing that one is dreaming). Of sixty-eight non-psychotic patients seen consecutively in a psychiatry emergency room, she was one of 16 (23.5%) found to have a concurrent complaint of nightmares (dream anxiety disorder). The benefits of the skills developed with lucid dreaming extended into areas other than nightmares as the patient entered psychotherapy. The techniques appeared to play a role in the reduction of nightmare frequency, intensity, and distress, and to enha

At-risk registers integrated into primary care to stop asthma crises in the UK: cluster randomised controlled trial with economic evaluation.

BACKGROUND: A regional trial indicated that implementing at-risk asthma registers in primary care could reduce hospital admissions. This national study assessed whether the intervention lowered asthma crisis events. METHODS: This cluster randomised trial involved 275 UK primary care practices. The intervention included identifying at-risk patients, staff training, a clinical decision support system alerting practice staff to patients' at-risk status to facilitate prompt and opportunistic care, and ongoing support. Control practices continued with standard care. Patients (n=10 945) were includ

Emergency Department Behavioral Health Observation and Stabilization.

Emergency department visits for pediatric mental health crises have risen sharply in recent years, while inpatient psychiatric bed capacity has declined, resulting in prolonged patient boarding. This article describes the development and implementation of behavioral health observation (BHO) models to address this gap in care. BHO provides time-limited, intensive psychiatric assessment and crisis stabilization with a multidisciplinary team focused on symptom reduction, diagnostic clarification, and structured disposition planning for patients. The article outlines patient selection, interdiscip

Brief psychiatric treatment for borderline personality disorder as a first step of care: Adapting general psychiatric management to a 10-session intervention.

The considerable demand versus supply gap of evidence-based treatments for borderline personality disorder (BPD) indicates the need for modular steps of care to tailor fit between individual patients' needs along the trajectory of their clinical course and effective interventions. These trajectories may or may not include lengthy specialized psychotherapies. Good psychiatric management (GPM) for BPD is being practiced by an increasing number of mental health professionals as a basic starting block of mental health services in the community. It remains an open question what duration of GPM is o

Care Wounds: Precarious Vulnerability and the Potential of Exposure.

What does it mean to offer care when the act of caring is wounding to its giver? For peer specialists-individuals with lived experience as patients in the psychiatric system-this question shapes how they use their own histories to provide support for individuals experiencing psychiatric crisis. Peer support is unique in the way it draws on empathetic resonance and depends on carefully deployed vulnerability; where one connects with others through the recognition of shared experience and mutual hurt. For peers, care works when this guidance, reassurance, and "being with"-all of which draw upon

Crisis as deferred closure-clairvoyant counselling in contemporary Danish society.

Clairvoyance, spiritualism and healing are popular ways of seeking guidance and personal development in contemporary Danish society. Although few Danes are self-declared spiritualists, many believe in the existence of ghosts and the ability of clairvoyants to communicate with the departed, and the market of alternative therapies offers a number of mediumistic activities. In anthropological writings, such activities are often associated with crisis and the re-establishment of order. The concept of crisis refers to a time of great difficulty or danger or when an important decision must be made.

[Engaging patients with serious mental illness in care services by Critical Time Intervention].

Patients with serious mental illness who are difficult to engage in services often face multiple complex problems, like homelessness, debts, and poor hygiene and social skills. Critical Time Intervention (CTI) is a time-limited, manual-guided case management approach intended to enhance continuity of care for mentally ill patients. We present two patients who received intensive outpatient CTI care. A 42-year-old woman diagnosed with depression and addiction to alcohol and medication and a 38-year-old man diagnosed with autism were offered practical and emotional support while they became linke

[Psychological interventions in the field of neurotraumatology during the acute care].

Neurotrauma means head or spine injury caused by an external force. Neurotraumatology care requires coordinated teamwork on the part of specialists, including psychological care as part of the multidisciplinary treatment team. Psychological interventions in the field of neurotraumatology aim to address the psychological consequences and challenges associated with head or spine injury. These interventions play a vital role in crisis intervention, promoting recovery, enhancing quality of life, and supporting individuals and their families in coping with the psychological impact of neurotrauma. Reframing the Overdose Crisis: Stigma, Industry Influence, and the Politics of Abuse-Deterrent Opioids.

Between 2013 and 2017, Canadian federal policymakers grappled with mandating abuse-deterrent formulations (ADFs) for oxycodone products as a response to the overdose crisis. Marketed as a safeguard against misuse and diversion, ADFs promised a technological fix to opioid-related harms, yet their population-level effectiveness remained contested. This study systematically analyzes federal parliamentary debates and committee hearings, identifying key arguments in framings to support or oppose ADF mandates. Proponents framed the crisis through the lens of individual misuse, positioning ADFs as ph

Counselor value guidance and lower psychological crisis vulnerability: dual meaning-and-resilience pathways in cross-sectional and cluster-randomized evidence from Chinese universities.

INTRODUCTION: Psychological crisis vulnerability among Chinese university students remains an urgent concern, yet protective institutional resources are less well specified than risk pathways. We examined whether counselor value guidance, operationalized as autonomy-supportive counselor relating in a value-relevant context, is associated with lower crisis vulnerability through two complementary mechanisms: meaning construction and resilience building. METHODS: Study 1 was a cross-sectional survey of 800 undergraduates testing parallel mediation and moderation by value internalization. Study 2

Crisis and acute mental health care for people who have been given a diagnosis of a 'personality disorder': a systematic review.

BACKGROUND: People who have been given a diagnosis of a 'personality disorder' need access to good quality mental healthcare when in crisis, but the evidence underpinning crisis services for this group is limited. We synthesised quantitative studies reporting outcomes for people with a 'personality disorder' diagnosis using crisis and acute mental health services. METHODS: We searched OVID Medline, PsycInfo, PsycExtra, Web of Science, HMIC, CINAHL Plus, Clinical Trials and Cochrane CENTRAL for randomised controlled trials (RCTs) and observational studies that reported at least one clinical or
