Transurethral ureteroscopy in women: a ready addition to the urological armamentarium.

Ureteroscopy via the transurethral route and limited to the terminal ureter in women lends itself to inclusion in the urological armamentarium. The technique requires no equipment other than routine urological instruments and makes endoscopic inspection, biopsy and resection within the distal ureter possible. The procedure is done with the patient under anesthesia, following urethral dilation to 32F. With the aid of a small caliber cystoscope, 20F or smaller, straight Jewett sounds can be passed into the urethra alongside the cystoscope and directed under cystoscopic control into the ureteral

What Is the Best Lens Angle for Rigid Diagnostic Cystoscopy? A Comparison of 30-Degree and 70-Degree Lenses.

UNLABELLED: The optimal degree of cystoscope to perform diagnostic cystoscopy is dependent on the surgeon's discretion because there are no studies addressing the superiority of one degree of cystoscope over another. The objective was to determine which lens, 70-degree versus 30-degree lens, was better in identifying lesions scattered throughout the bladder. METHODS: A simulation was created using 3 different artificial bladder models complete with sutures representing lesions placed at different locations in each bladder. Gynecologists and urologists performed cystoscopy using both the 30- a

Reducing Clinical Waste in Flexible Cystoscopy: A Prospective Quality Improvement Study.

AIMS: Flexible cystoscopy is a very common procedure that relies heavily on single-use consumables, resulting in significant procedural waste. This study aimed to evaluate whether a targeted waste-segregation intervention could reduce clinical waste generation during flexible cystoscopy without compromising clinical practice. METHODS: A prospective audit of 50 consecutive diagnostic flexible cystoscopies performed at a tertiary urology centre was undertaken. Twenty-five procedures were evaluated before intervention, followed by structured contamination-based waste segregation training for cli

Adjunctive endourological technique for transurethral resection in patients with urethral stricture.

Attempts to dilate the urethra with metal sounds and to insert a resectoscope sheath with the metal obturator can be traumatizing in patients with urethral stricture or false passage. We describe a simple technique that uses endourological instruments to facilitate insertion of the resectoscope sheath and placement of the post-resection Foley catheter.

Objective Evaluation of Gaze Location Patterns Using Eye Tracking During Cystoscopy and Artificial Intelligence-Assisted Lesion Detection.

Background: The diagnostic accuracy of cystoscopy varies according to the knowledge and experience of the performing physician. In this study, we evaluated the difference in cystoscopic gaze location patterns between medical students and urologists and assessed the differences in their eye movements when simultaneously observing conventional cystoscopic images and images with lesions detected by artificial intelligence (AI). Methodology: Eye-tracking measurements were performed, and observation patterns of participants (24 medical students and 10 urologists) viewing images from routine cystosc

The diagnosis, epidemiology and control of urinary infection in urology and gynaecology.

In a study of the urine after urological operations, quantitative bacteriological criteria for the diagnosis of infection were determined. The bacteria responsible for urinary infection after urological or gynaecological procedures sometimes came from the same patient, but more often came from other patients by cross-infection. The principal final route of entry was the urethra or the indwelling catheter lying in it. These findings led to the elaboration of methods of destroying bacteria in the urethra before operation, improving the disinfection of cystoscopes, and preventing the entry of bac

The Management and Efficacy of Surgical Techniques Used for Erosive Mesh in the Urethra and Bladder: A Systematic Review.

BACKGROUND: This study reports the re-operation and symptom resolution rates of different techniques used in surgical excision of genitourinary mesh erosions. METHODS: A comprehensive systematic review was completed after searching electronic databases for studies involving outcomes of mesh erosion in humans that were managed surgically using a transvaginal, cystoscopic, or abdominal approach. Surgical outcomes were reported in percentages, ranges, and simple pooling to generate trends in management techniques. RESULTS: There were 177 cases that met our inclusion criteria. Forty-one patients

Different Techniques for Management of Problematic Urethral Catheterization in Male Children.

Background: Urethral catheterization is very common, considered a minor procedure done by physicians and well-trained nurses. However, in some traumatic or congenital cases, male catheterization is problematic. A multitude of techniques are available and still can utilize the natural urethral opening to manage this distressing situation. Among these techniques, cystoscopic-assisted retrograde catheterization and railroad techniques are reliable options. This article aimed to document application and to evaluate outcomes of both techniques for problematic male urethral catheterization. Patients

Hysteroscopic management of intrauterine lesions and intractable uterine bleeding.

Müllerian fusion defects, submucous myomas, and intractable uterine bleeding have been managed traditionally by major surgical intervention. However, the cystoscope-resectoscope provides the operative versatility allowing transvaginal surgical management of these situations. Forty women underwent hysteroscopy and treatment with the cystoscope-resectoscope at the Yale-New Haven Hospital. Those patients treated for septate müllerian defects also underwent concomitant laparoscopy. Therapeutic surgical use of the cystoscope-resectoscope resulted in no immediate or long-term complications. Of 11 pa

Transurethral cystolitholapaxy in children.

Five boys, 5 to 9 (mean 6) years old, who had primary vesical stones underwent transurethral cystolitholapaxy. All stones were disintegrated by the 3F metal probe of the Swiss Lithoclast, which is perfectly adapted to the working channel of the 10F and 12F pediatric cystoscopes. A 13F sheath was then used to wash out the stone fragments. The mean stone diameter was 1.1 cm, and the mean operating time was 15 minutes. A 1-day hospital stay was recorded for all patients with no morbidity. During a follow-up period of 3 to 13 (mean 7) months, there were no micturition disturbances and no urinary t

Flexible cystoscopy during urodynamic evaluation of spinal cord-injured patients.

Eighty-one consecutive flexible cystoscopic examinations were performed on 69 patients with spinal cord injury (SCI) at the time of their urodynamic study. The indications for cystoscopy included hematuria, recurrent urinary tract infections, symptoms of bladder outlet obstruction, the presence of an intraurethral sphincter stent requiring evaluation, neurogenic vesical dysfunction requiring endourodynamic study (cystometrogram through the working port of the flexible cystoscope), or bladder calculi. Flexible cystoscopy was accomplished in all patients, whether lying supine or seated in a whee

A new method of adjusting mesh tension using cystoscopy during laparoscopic sacrocolpopexy.

We aimed to examine the usefulness of cystoscopic findings to guide mesh tension adjustment during laparoscopic sacrocolpopexy (LSC) to promote better repair of pelvic organ prolapse (POP) and prevent de novo stress urinary incontinence (SUI). In this technique, the bladder wall was observed using a cystoscope when various traction pressures were applied by pulling the mesh arm with forceps before fixation to the promontory during LSC. Adjustment was performed on 20 patients, and postoperative outcomes of POP repair and development of de novo SUI were evaluated. When excessive traction was app

Anterior urethral valve presenting in an adult male.

Anterior urethral valves are a rare congenital anomaly which can occur singly or in combination with a proximal urethral diverticulum. A retrograde urethrogram and voiding cystourethrogram can easily make the diagnosis unless the valve is located with the glandular urethra. Cystoscopic examination of the urethra can overlook anterior urethral valves, but by altering the flow of the irrigant, the cystoscope can be used to make these valves very apparent.

Treatment of vesicouterine fistula by fulguration.

A 29-year-old woman suffered a vesicouterine fistula following cesarean section. The fistula was treated successfully after cystoscopic identification and fulguration of the tract. Although various surgical approaches to this problem have been described, and a number of cases of spontaneous resolution are reported, to date there have been no reports of treatment of this problem via cystoscopic fulguration. We advocate this simple technique as a primary approach to vesicouterine fistulas.
