Interoceptive Signals Bias Decision Making in Rhesus Macaques.

Several influential theories have proposed that interoceptive signals, sent from the body to the brain, contribute to neural processes that coordinate complex behaviors. We altered the physiological state of the body using compounds that have minimal effect on the brain and evaluated their effect on decision-making in rhesus monkeys. We used glycopyrrolate, a non-specific muscarinic (parasympathetic) antagonist, and isoproterenol, a beta-1/2 (sympathetic) agonist, to create a sympathetic-dominated state in the periphery, that was indexed by increased heart rate. Rhesus monkeys were trained on

"Terminal Anorexia", Treatment Refusal and Decision-Making Capacity.

Whether anorexic patients should be able to refuse treatment when this refusal potentially has a fatal outcome is a vexed topic. A recent proposal for a new category of "terminal anorexia" suggests criteria when a move to palliative care or even physician-assisted suicide might be justified. The author argues that this proposed diagnosis presents a false sense of certainty of the illness trajectory by conceptualizing anorexia in analogy with physical disorders and stressing the effects of starvation. Furthermore, this conceptualization is in conflict with the claim that individuals who meet th

Surgical Decision-Making: Who Should Be Offered Sacrocolpopexy?

INTRODUCTION AND HYPOTHESIS: Women undergoing surgery for apical pelvic organ prolapse have several medically sound options for specific surgical approaches. METHODS: We review the principles of shared decision-making as they pertain to surgery for prolapse. We review the literature supporting the superior sacrocolpopexy as a durable treatment for prolapse and the factors that may differentiate it from other repairs in risk and benefit. RESULTS: We emphasize the importance of collaboration between patients and surgeons in surgical decision-making. CONCLUSION: All medically appropriate patie

[Hierarchical Decision-Making Mechanism of the Frontal Lobe].

Actions performed by humans require various levels of volitional control, ranging from highly practiced, automatic actions to those done in unfamiliar environments which necessitate deliberate controls. The multiple cortical motor areas in the frontal lobe regulate actions that vary in the levels of automaticity vs. intentionality. Further more, a part of the medial prefrontal cortex was found to contribute to the selection of the behavioral tactics, an internal protocol of how to decide what to do, which is a more abstract level of decision making than the selection of action per se. The fron

Evidence for Anchoring Bias During Physician Decision-Making.

INTRODUCTION: Cognitive biases are hypothesized to influence physician decision-making, but large-scale evidence consistent with their influence is limited. One such bias is anchoring bias, or the focus on a single-often initial-piece of information when making clinical decisions without sufficiently adjusting to later information. OBJECTIVE: To examine whether physicians were less likely to test patients with congestive heart failure (CHF) presenting to the emergency department (ED) with shortness of breath (SOB) for pulmonary embolism (PE) when the patient visit reason section, documented i

Contralateral Prophylactic Mastectomy Decision-Making: The Partners' Perspective.

BACKGROUND: The rate of contralateral prophylactic mastectomy (CPM) continues to rise despite no improvement in survival, an increased risk of surgical complications, and negative effects on quality of life. This study explored the experiences of the partners of women who undergo CPM. METHODS: This study was part of an investigation into the factors motivating women with early-stage unilateral breast cancer and low genetic risk to opt for contralateral prophylactic mastectomy (CPM). Participating women were asked for permission to invite their partners to take part in interviews. In-depth int

Clinical decision-making and treatment of myelodysplastic syndromes.

The myelodysplastic syndromes (MDSs) constitute a profoundly heterogeneous myeloid malignancy with a common origin in the hemopoietic stem cell compartment. Consequently, patient management and treatment are as heterogeneous. Decision-making includes identifying risk, symptoms, and options for an individual and conducting a risk-benefit analysis. The only potential cure is allogeneic stem cell transplantation, and albeit the fraction of patients with MDS who undergo transplant increase over time because of better management and increased donor availability, a majority are not eligible for this

Explicit effort may not influence perceptuomotor decision-making.

Many decisions that humans make are enacted by the action system. For example, humans use reach-to-grasp movements when making perceptuomotor decisions between and obtaining fruits of varying quality from a pile. Recent work suggests that the characteristics of each action alternative may influence the decision itself-there may be a bias away from making perceptuomotor alternatives associated with high effort when participants are unaware of the effort differences between responses. The present study examined if perceptuomotor decisions were influenced by explicit reaching effort differences.

Confidence and risky decision-making in gambling disorder.

BACKGROUND AND AIMS: People with Gambling Disorder (GD) often make risky decisions and experience cognitive distortions about gambling. Moreover, people with GD have been shown to be overly confident in their decisions, especially when money can be won. Here we investigated if and how the act of making a risky choice with varying monetary stakes impacts confidence differently in patients with GD (n = 27) relative to healthy controls (HCs) (n = 30). METHODS: We used data from our previous mixed-gamble study, in which participants were given the choice of a certain option or a 50/50 gamble with

Improving opioid stewardship programs through shared decision-making.

Opioid stewardship has emerged as an innovative systems-level approach designed to reduce inappropriate opioid prescriptions and improve patient safety in acute care settings. Modeled on the successes of antimicrobial stewardship programs, key distinctions exist; in particular, the inherent subjectivity of managing acute pain is an important consideration of opioid stewardship that differentiates it with the more objective features of antibiotic selection. Shared decision-making, with pharmacists playing a central role, is regarded as an integral part of patient care and plays a vital role in

Shared decision-making for pediatric elective penile surgery.

INTRODUCTION: In pediatric surgery, proxy decision-makers are frequently involved in treatment planning and may experience decisional conflict (DC). Shared decision-making (SDM) approaches may be effective to remedy DC. This study investigates DC and SDM involvement in elective pediatric penile surgery. METHODS: Forty-four parents of children aged <8 years undergoing elective penile surgery consultations at a tertiary pediatric hospital were prospectively enrolled. Patient and physician questionnaires were used to assess the SDM process and the SURE (Sure of myself; Understand information; Ri

Risky decision-making following prefrontal D1 receptor manipulation.

The prefrontal dopamine D1 receptor (D1R) is involved in cognitive processes. Viral overexpression of this receptor in rats further increases the reward-related behaviors and even its termination induces anhedonia and helplessness. In this study, we investigated the risky decision-making during D1R overexpression and its termination. Rats conducted the rodent version of the Iowa gambling task daily. In addition, the methyl CpG-binding protein-2 (MeCP2), one regulator connecting the dopaminergic system, cognitive processes, and mood-related behavior, was investigated after completion of the beh

Ambiguity and conflict: Dissecting uncertainty in decision-making.

Making decisions is fundamental to how we navigate, survive, and thrive in our environment. The quality of information used to support decisions is rarely perfect. Many decisions are made under conditions of uncertainty, arising from ambiguous or conflicting information. Conflict and ambiguity, though conceptually distinct, both generate uncertainty, a commonality that has led to overlapping and inconsistent terminology in the literature. Evidence from human and animal research suggests a behavioral dissociation in responding to conflict and ambiguity. This dissociation can be studied through

Shared Decision Making during the COVID-19 Pandemic.

OBJECTIVE: The COVID-19 pandemic pushed some of the most well-developed health care systems to their limits. In many cases, this has challenged patient-centered care. We set out to examine individuals' attitudes toward shared decision making (SDM) and to identify predictors of participation preference during the pandemic. METHODS: We conducted an online survey with a large convenience sample (N = 1061). Our main measures of interest were participants' generic and COVID-19-related participation preference as well as their acceptance and distress regarding a triage vignette. We also assessed an

Normal-Pressure Hydrocephalus - Patient Evaluation and Decision-Making.

BACKGROUND: Normal-pressure hydrocephalus (NPH) presents with the triad of gait difficulty, urinary incontinence, and cognitive decline. However, the definitive diagnosis and treatment may be challenging at times due to secondary causes of NPH (sNPH) versus idiopathic NPH (iNPH), co-existing other degenerative, vascular, and metabolic causes of similar clinical presentations. OBJECTIVE: The objective of this narrative review is to outline the approach to patient evaluation and decision-making in cases where there is clinical suspicion of iNPH. METHODS: This review article intends to provide

Surrogate Satisfaction with Decision Making After Intracerebral Hemorrhage.

BACKGROUND/OBJECTIVE: Surrogate decision makers for patients with intracerebral hemorrhage (ICH) are frequently asked to make difficult decisions on use of life-sustaining treatments. We explored ICH surrogate satisfaction with decision making and experience of decision regret using validated measures in a prospective multicenter study. METHODS: Cases of non-traumatic ICH were enrolled from three hospitals (September 2015-December 2016), and surrogate decision makers were invited to complete a self-administered survey. The primary outcome was the 10-item decision-making subscale of the Family

Maximizing decision-making style and hoarding disorder symptoms.

BACKGROUND: Past research links hoarding disorder (HD) to indecisiveness and difficulty with decision-making. However, it remains unclear what contributes to difficulty making decisions in HD. Decision-making research suggests that some individuals have a maximizing decision-making style (seeking the best option through an exhaustive search of all existing alternatives) while others "satisfice" (choosing options that are satisfactory even without seeing all options). Past work has linked the dispositional tendency to maximize in decisions to elevated depression, anxiety and obsessive-compulsiv

Dopamine and Risky Decision-Making in Gambling Disorder.

Gambling disorder is a behavioral addiction associated with impairments in value-based decision-making and cognitive control. These functions are thought to be regulated by dopamine within fronto-striatal circuits, but the role of altered dopamine neurotransmission in the etiology of gambling disorder remains controversial. Preliminary evidence suggests that increasing frontal dopamine tone might improve cognitive functioning in gambling disorder. We therefore examined whether increasing frontal dopamine tone via a single dose of the catechol-O-methyltransferase (COMT) inhibitor tolcapone woul
