Deep brain stimulation: current and future clinical applications.

Deep brain stimulation (DBS) has developed during the past 20 years as a remarkable treatment option for several different disorders. Advances in technology and surgical techniques have essentially replaced ablative procedures for most of these conditions. Stimulation of the ventralis intermedius nucleus of the thalamus has clearly been shown to markedly improve tremor control in patients with essential tremor and tremor related to Parkinson disease. Symptoms of bradykinesia, tremor, gait disturbance, and rigidity can be significantly improved in patients with Parkinson disease. Because of the

Update on deep brain stimulation for neuropsychiatric disorders.

Deep brain stimulation (DBS) has proven a powerful treatment for medication refractory movement disorders. Success in this group of patients has allowed preliminary studies of DBS to proceed in severe and medication resistant cases of depression, obsessive-compulsive disorder (OCD) and Tourette's syndrome (TS). Pathophysiological and imaging studies along with attempts at lesioning the basal ganglia, have offered clues as to nodes in the circuitry that may be amenable to neuromodulation. DBS in neuropsychiatric illness has offered hope, but at this time rigorous screening by interdisciplinary

[Deep brain stimulation for Parkinson's disease and dystonia].

Deep brain stimulation (DBS) has proved highly effective in the treatment for Parkinson's disease and dystonia. Presently, many types of dopamine agonists, monoamine oxidase B (MAOB) inhibitors, catechol-O-methyltransferase (COMT) inhibitor or other antiparkinsonian drugs are being developed. However, it is still very difficult to medically treat motor complications associated with levodopa therapy. Surgical intervention using DBS was possible to stimulate the subthalamic nucleus as the treatment of Parkinson's disease. Stimulation of the subthalamic nucleus improves the cardinal parkinsonian

Preparing the ethical future of deep brain stimulation.

BACKGROUND: Deep brain stimulation is an approved and effective neurosurgical intervention for motor disorders such as PD and ET. Deep brain stimulation may also be effective in treating a number of psychiatric disorders, including treatment refractory depression and OCD. Although DBS is a widely accepted therapy in motor disorders, it remains an invasive and expensive procedure. The ethical and social challenges of DBS need further examination, and discussion and emerging applications of DBS in psychiatry may also complicate the ethical landscape of DBS. METHODS: To identify and characterize

[Deep brain stimulation, Parkinson's disease and neuropsychiatric complications].

Bilateral high-frequency stimulation of the subthalamic nuclei is now considered as the method of choice for the treatment of severe and advanced forms of Parkinson's disease. The technique improves patients' quality of life, disability, motor complications and allows the dose of dopaminergic treatment to be reduced. The dramatic motor benefits provided by the technique were firstly accompanied by a number of psychiatric complications--depression hypomania or mania, anxiety disorders or behavioural disorders--reflecting both the deleterious effect of modulation of the limbic circuit and the (i

Deep brain stimulation: a new treatment for hypertension?

We report a 61-year-old hypertensive man who underwent deep brain stimulation of the periventricular/periaqueductal grey area for the relief of chronic neuropathic pain affecting his oral cavity and soft palate. During intraoperative stimulation, we were able to modulate his blood pressure up or down, depending on electrode location. This is the first evidence that hypertension could be effectively treated with electrical stimulation of the midbrain.

Pearls in patient selection for deep brain stimulation.

BACKGROUND: Deep brain stimulation (DBS) has emerged as an important treatment for medication refractory movement and neuropsychiatric disorders. General neurologists and even general practitioners may be called upon to screen potential candidates for DBS. The patient selection process plays an important role in this procedure. REVIEW SUMMARY: In this article, we discuss "pearls" for the clinician who may be called upon to identify appropriate candidates for DBS. Additionally, we will discuss the important points that should be considered when referring patients for surgical intervention. CO

Deep brain stimulation for torsion dystonia in children.

INTRODUCTION: Deep brain stimulation (DBS) at the internal globus pallidus (GPi) is an effective treatment for some patients with medically refractory torsion dystonia. In this article, we review the results of pallidal DBS surgery in children with dystonia. Details of the DBS procedure and programming of the DBS devices are discussed. DISCUSSION: Pallidal DBS is most effective in patients with primary generalized dystonia. Children and adolescents possessing the DYT1 gene mutation may respond best of all. The presence of static dystonic postures and/or fixed orthopedic contractures may limit

Deep brain stimulation in Tourette's syndrome: two targets?

In this report, we describe the effects of bilateral thalamic stimulation in one patient and of bilateral pallidal stimulation in another patient. Both patients suffered from intractable Tourette's syndrome (TS). Any conservative treatment had failed or had been stopped because of unbearable side effects in the 2 patients. In both cases, there was no comorbidity except for associated behavioral symptoms (compulsions). Electrodes were implanted at the level of the medial part of the thalamus (centromedian nucleus, the substantia periventricularis, and the nucleus ventro-oralis internus) in one

Surgery insight: Deep brain stimulation for movement disorders.

Over the past two decades, deep brain stimulation (DBS) has supplanted lesioning techniques for the treatment of movement disorders, and has been shown to be safe and efficacious. The primary therapeutic indications for DBS are essential tremor, dystonia and Parkinson's disease. In the case of Parkinson's disease, DBS is effective for treating the primary symptoms--tremor, bradykinesia and rigidity--as well as the motor complications of drug treatment. Progress has been made in understanding the effects of stimulation at the neuronal level, and this knowledge should eventually improve the effe

Deep brain stimulation in movement and psychiatric disorders.

Deep brain stimulation (DBS) is the most focal and invasive of the electromagnetic brain stimulation therapies. A subcutaneous pulse generator provides continuous stimulation of circumscribed brain tissue via a multicontact microelectrode that terminates within its target. The result is an adjustable, reversible, and specific therapy. Despite limited understanding of its mechanisms of action, DBS efficacy has been established in several movement disorders, and promising reports have emerged for Tourette syndrome, obsessive-compulsive disorder, and major depression. Deep brain stimulation may p

Deep brain stimulation for alleviating chronic intractable pain.

The authors present their 4-year experience with 18 patients who had deep brain stimulation. Most were referred because of chronic pain of varied etiology. All conventional modalities of management had failed. Both the paraventricular gray matter and the sensory thalamus were target sites. The 18 patients underwent a total of 21 implants. Follow-up ranged from 6 months to 4 years with moderate relief of prestimulation pain in 14 patients (77%). Patients with failed back syndrome secondary to multiple disc operations fared well. Patients with pain secondary to progressive neurologic disorders o

Stimulating at the right time: phase-specific deep brain stimulation.

SEE MOLL AND ENGEL DOI101093/AWW308 FOR A SCIENTIFIC COMMENTARY ON THIS ARTICLE: Brain regions dynamically engage and disengage with one another to execute everyday actions from movement to decision making. Pathologies such as Parkinson's disease and tremor emerge when brain regions controlling movement cannot readily decouple, compromising motor function. Here, we propose a novel stimulation strategy that selectively regulates neural synchrony through phase-specific stimulation. We demonstrate for the first time the therapeutic potential of such a stimulation strategy for the treatment of pat

The application of vagus nerve stimulation and deep brain stimulation in depression.

Despite the progress in the pharmacotherapy of depression, there is a substantial proportion of treatment-resistant patients. Recently, reversible invasive stimulation methods, i.e. vagus nerve stimulation (VNS) and deep brain stimulation (DBS), have been introduced into the management of treatment-resistant depression (TRD). VNS has already received regulatory approval for TRD. This paper reviews the available clinical evidence and neurobiology of VNS and DBS in TRD. The principle of VNS is a stimulation of the left cervical vagus nerve with a programmable neurostimulator. VNS was examined in

Optimized Brain Functional MRI for Patients With Deep Brain Stimulation Systems.

BACKGROUND AND OBJECTIVES: Deep brain stimulation (DBS) is an established neuromodulation therapy for Parkinson disease and other neurological disorders. Active DBS-functional MRI (fMRI) enables visualization of circuit engagement and may provide objective biomarkers of therapeutic efficacy, yet susceptibility artifacts compromise data quality. The aim of this study was to develop optimized blood oxygenation-level-dependent (BOLD) fMRI protocols for DBS patients by adjusting operator-accessible parameters to mitigate susceptibility artifacts while preserving image quality.
