Arrhythmic outcome of arrhythmogenic right ventricular cardiomyopathy patients without implantable defibrillators.

BACKGROUND: Implantable defibrillators (ICD) are an important therapy for arrhythmogenic right ventricular cardiomyopathy (ARVC) patients at high risk of sudden death. Given the high appropriate ICD therapy rate, some have argued that the mere act of implanting an ICD inflates the malignant arrhythmia rate in ARVC. OBJECTIVE: To report the arrhythmic course of ARVC patients without ICDs at the fulfillment of the 2010 Task Force Criteria and explore predictors of malignant ventricular arrhythmias. METHODS: We included 131 definite ARVC patients (age 32 ± 15 years, male 39%, proband 50%) eithe

Performance of automated external defibrillators under conditions of in-flight turbulence.

INTRODUCTION: Modern automated external defibrillators (AEDs) are designed to prevent shock delivery when excessive motion produces rhythm disturbances mimicking ventricular fibrillation (VF). This has been reported as a safety issue in airline operations, where turbulent motion is commonplace. We aimed to evaluate whether all seven AEDs can deliver shock appropriately in a flight simulator under turbulent conditions. METHODS: The study was performed in a Boeing 747-400 full motion flight simulator in Hong Kong. An advanced life support manikin and arrhythmia generator were used to produce si

Clinical experience with wearable cardioverter defibrillators at a tertiary electrophysiology program.

BACKGROUND: A wearable cardioverter defibrillator (WCD) is an alternative for patients who are not immediate candidates for an implantable cardioverter defibrillator (ICD). METHODS: Retrospective analysis of 242 patients prescribed a WCD from January 2014 through March 2015 with 1-year follow-up. RESULTS: A WCD was prescribed to 242 patients during the 15-month period. Forty-one patients were excluded due to lack of complete follow-up. The remaining 201 patients received a WCD for primary or secondary prevention of SCD. Seventy-nine percent of WCDs were prescribed by nonelectrophysiologists.

Eligibility of cardiac resynchronization therapy patients for subcutaneous implantable cardioverter defibrillators.

BACKGROUND: Before subcutaneous implantable cardioverter defibrillator (S-ICD) implantation, the adequacy of sensing is required to be verified through surface ECG screening. Our objective was to determine whether S-ICD can be considered as a supplementary therapy in patients who are receiving biventricular (BIV) pacing. METHODS: We evaluated 48 patients with BIV devices to determine S-ICD candidacy during BIV, left ventricular (LV), right ventricular (RV) pacing, and intrinsic conduction (left bundle branch block-LBBB) by using an automated screening tool. Eligibility was defined by the pres

Experiences of Young Adults With Pacemakers and/or Implantable Cardioverter Defibrillators.

BACKGROUND: The number of implanted devices (pacemakers [PMs] and implantable cardioverter defibrillators [ICDs]) is increasing. Because of technological advances, these devices are used in greater numbers in children and adolescents. Challenges of having these devices have been well studied in adults, but little is known about the lived experiences from the perspective of young adults. OBJECTIVE: This study explored the lived experiences, coping behaviors, and challenges of young adults who had cardiac devices implanted during childhood. METHODS: A descriptive qualitative study design was u

Surviving out-of-hospital cardiac arrest: just a matter of defibrillators?

Out-of-hospital sudden cardiac arrest (OHCA) is a leading cause of death all over the world. Although the outcome of OHCA resulting from 'nonshockable' rhythms (asystole and pulseless electrical activity) is poor regardless of resuscitation efforts, 'shockable' rhythms such as ventricular tachycardia or fibrillation may carry a good prognosis if early defibrillation is performed. At present, simplified cardiopulmonary resuscitation techniques (hands-only cardiopulmonary resuscitation) and automated external defibrillators (AEDs) offer lay people the possibility to provide lifesaving treatment

Recommendations for the programming of implantable cardioverter-defibrillators in New Zealand.

AIM: Our manuscript proposes recommendations to standardise implantable cardioverter-defibrillator (ICD) programming practice in New Zealand (NZ) and provides a review of the literature behind such an initiative. BACKGROUND: Shocks have traditionally been the mainstay of ICD therapy for ventricular tachyarrhythmia. There is substantial evidence to suggest shocks may cause psychological harm, are often painful and may increase hospitalisation for heart failure. Shock therapy may be reduced by avoiding unnecessary therapy of non-sustained arrhythmia, utilising anti-tachycardia pacing to termina

Uncertainty is a major concern for patients with implantable cardioverter defibrillators.

OBJECTIVE: The study objective was to explore the main concern of individuals living with an implantable cardioverter defibrillator (ICD) and how they handle this in daily life. For improved management and follow-up, it is important to understand how the ICD affects the recipient's daily life. METHODS: A grounded theory method was used. Sixteen Swedish recipients (9 men) living with an ICD for 6 to 24 months were interviewed. RESULTS: The core category labeled, "Incorporating uncertainty in daily life," illuminates the main concern. To handle uncertainty, recipients used the following strate

Ambient air pollution and cardiac arrhythmias in patients with implantable defibrillators.

BACKGROUND: Previous studies of ambient air pollution and ventricular tachyarrhythmias in patients with implantable cardioverter defibrillators have yielded mixed results. METHODS: We examined this relationship in a study of 518 patients with 6287 tachyarrhythmic event-days over a 10-year period in Atlanta, Georgia. The air quality data included daily measurements of PM10, ozone, nitrogen dioxide, carbon monoxide, and sulfur dioxide for the entire study period, as well as speciated measurements of PM2.5 mass and oxygenated hydrocarbons for the final 4 years of the study. Our primary analyses

[The use of implantable cardioverter-defibrillators (ICD) in children and adolescents].

BACKGROUND: The compelling safety and efficacy data in numerous large, blinded trials on adult patients, and the progress in device- and leadtechnology have led to increasing use of implantable cardioverter defibrillators in pediatric patients. The purpose of our study was to assess the efficacy and safety of ICD in the pediatric age group of a tertiary referral centre. PATIENTS AND METHODS: Between March 1998 and October 2003 12 patients underwent ICD-implantation. The mean age at implantation was 14,8 years with a range between 10-17 years. The underlying cardiac disorders included long QT-

The management of surgical complications of pacemaker and implantable cardioverter-defibrillators.

The rate of implantation of pacemakers and implantable cardioverter-defibrillators (ICDs) is ever-increasing. The relative ease of device implantation utilizing a relatively simple, expeditious, percutaneous approach, without the requirement for general anesthesia or long recuperation times, has fueled enthusiasm for implantation. However, the complication risk is ever-present and forms the subject of this pragmatic review, which is limited to the management of only the surgical complications of device implantation. The management of surgical complications related to the implantation of pacema

Significant differences in charge times among currently available implantable cardioverter defibrillators.

Capacitor charging accounts for most of the delay between arrhythmia detection and therapy delivery in ICDs. Long capacitor charge times may increase the risk of syncope in patients with poorly tolerated arrhythmias. To determine if there are clinically important differences in charge time among currently available devices, we analyzed charge times at various delivered energy levels in three manufacturers' devices: Medtronic, CPI, and Ventritex. Charge times were measured for shocks delivered for spontaneous or induced arrhythmias occurring from time of implant to 4 months after implant. A tot

Enhanced detection criteria in implantable cardioverter-defibrillators to avoid inappropriate therapy.

The aim of this prospective study was to evaluate the efficacy and safety of enhanced detection criteria, stability and sudden onset, for ventricular tachycardia (VT) in the therapy of implantable cardioverter-defibrillators (ICDs). These detection enhancements ensure a high specificity in detecting VT, thereby avoiding inappropriate therapy delivery due to supraventricular tachycardia. However, delayed sensing or even undersensing of VT may lead to a problematic, even fatal, outcome. In our study, the stability detection enhancement was programmed to discriminate atrial fibrillation (AF) in 8

Significance of supraventricular tachyarrhythmias in patients with implanted pacing cardioverter defibrillators.

Eighty-six patients were treated with an implantable cardioverter defibrillator (ICD) because of sustained ventricular tachycardia (VT) or ventricular fibrillation (VF). In 27 patients an epicardial system was used, in 59 patients a transvenous system with a subcutaneous patch electrode was implanted. During a mean follow-up time of 17 +/- 9 months, inappropriate activations of the ICD due to supraventricular tachycardia were documented by Holter monitoring in 14 patients (16%). In 8 patients paroxysmal atrial fibrillation (AF), in 2 patients chronic AF, in 1 patient atrial flutter, and in 3 p

Retrosternal extravascular implantable cardioverter-defibrillators: a systematic review of implant characteristics, defibrillation testing, and acute safety.

INTRODUCTION: Implantable cardioverter-defibrillators (ICD) terminate potentially deadly ventricular arrhythmias in at-risk patients. Retrosternal extravascular ICDs (EV-ICD) place a lead in the mediastinal space to provide both shock and antitachycardia pacing therapy without the need for venous access. A systematic review was performed to assess acute outcomes in patients implanted with a retrosternal EV-ICD. METHODS: A query of EMBASE and MEDLINE databases was performed to identify unique retrosternal EV-ICD cohort studies published between JAN-1-2020 and JAN-31-2026. Two known retrosterna

Cardiopulmonary resuscitation (CPR) plus delayed defibrillation versus immediate defibrillation for out-of-hospital cardiac arrest.

BACKGROUND: Sudden cardiac arrest (SCA) is a common health problem associated with high levels of mortality. Cardiac arrest is caused by three groups of dysrhythmias: ventricular fibrillation (VF) or pulseless ventricular tachycardia (VT), pulseless electric activity (PEA) and asystole. The most common dysrhythmia found in out-of-hospital cardiac arrest (OHCA) is VF. During VF or VT, cardiopulmonary resuscitation (CPR) provides perfusion and oxygenation to the tissues, whilst defibrillation restores a viable cardiac rhythm. Early successful defibrillation is known to improve outcomes in VF/VT.

Ethical and legal perspective of implantable cardioverter defibrillator deactivation or implantable cardioverter defibrillator generator replacement in the elderly.

PURPOSE OF REVIEW: Implantable cardioverter defibrillator (ICD) implantation has become a common and standard treatment for primary and secondary prevention of sudden cardiac death in patients with poor left ventricular ejection fraction across the world. Circumstances, of course, change after the initial implant as patients age. This raises legal and ethical questions about deactivating or not replacing ICD generators when the likelihood of meaningful benefit has diminished. RECENT FINDINGS: Health professionals are reluctant to discuss the end-of-life planning with patients who have ICDs. O
