INTRODUCTION: Complete hand degloving injuries are traumatic avulsion injuries causing the skin to pull away from the underlying tissues and are most often caused by industrial machinery. We present the case of a degloving trauma of the whole fingers, hand, and wrist resulting in a "watch hand" by analogy with the "ring finger" and discuss alternatives and recommendations from the rare cases described of similar traumas.
CASE PRESENTATION: A 33-year-old manual worker, a non-smoker with no significant medical or surgical comorbidities, was admitted for a complete skin avulsion of the left hand

Extremity injuries to employees in an industrial setting are not an uncommon event in today's mechanised workspace. Cut injuries of the foot during handling of machines are common in the workplace and the management of such injuries depends upon various factors at the time of presentation. This case report describes a rare foot injury which has not previously been reported in the literature and describes its subsequent successful management.

Complete degloving injury of three digits not amenable to revascularization may leave poor cosmetic and functional results. We used a compound thoracodorsal artery perforator (TDAP) flap in a 34-year-old, right-handed, male worker with traumatic degloving injury. The flap consisted of a thin nonbulky skin component isolated on two perforators in combination with serratus fascia, both pedicled on the thoracodorsal vessels. The mobility of the two flap components allowed the palmar and dorsal part of the fingers to be reconstructed without relying on multiple flaps or anastomoses. The skin compo

An avulsed elbow with severe degloving and crush injury was reconstructed in a 66-year-old man. The biceps brachii was completely transected, and the triceps brachii tendon was partly transected. In addition, the skin around the elbow had become totally necrotic. Four weeks after the injury, a free anterolateral thigh flap with a fascial flap was transplanted to reconstruct both the biceps brachii and triceps brachii tendons simultaneously. Six months after the initial injury, the range of elbow motion had recovered to almost the same level as that before injury.

This is a case of a proximal corpus cavernosa fracture presenting with scrotal edema and butterfly perineal ecchymosis sparing the penile shaft. Preoperative MRI obviated the need for circumferential incision and degloving of the penis and guided immediate incision over the area of corporal injury. The presentation, diagnostic work-up, and surgical treatment are discussed.

One of the most impressive soft tissue injuries is the facial degloving, normally associated with industrial machines and traffic accidents. This injury is characterized by the separation of the skin and cartilage from the bones, compromising the soft tissues correlated in the trauma area, nerves, and blood vessels. A 28-year-old patient, male, was referred to Araçatuba's Santa Casa Hospital, after a motorcycle accident, hitting his face on the sidewalk. The patient was conscious, oriented, denying fainting and unconsciousness during the accident, and complaining of pain in the nasal region of

We present a technique of providing complete skin cover for total degloving injuries of the hand and distal third of the forearm. This involves the use of a Y-shaped hypogastric groin flap. The proximal portion of the Y is tubed allowing the distal portion of each flap to be applied to the dorsal and volar surfaces respectively. This technique allows complete skin cover of both surfaces and yet retains the advantages of the groin flap in that it still has a long pedicle allowing the hand to be put through a full range of motion, speeding rehabilitation.

A case of massive degloving injury of the trunk, with open pelvic fracture, and evisceration of abdominal contents from blunt trauma is presented. The most significant aspect of this case was the transfusion of 173 units of packed cells and 176 units of fresh frozen plasma in the first thirty hours. The patient ultimately recovered and returned to work.

BACKGROUND First described in 1863 by French surgeon Victor-Auguste-François Morel-Lavallee, the Morel-Lavallee lesion (MLL) is a closed traumatic soft-tissue degloving injury. These lesions most commonly occur following motor vehicle collisions (MVCs). The pathophysiology stems from a shearing force that causes separation of the soft tissue from the fascia underneath, which disrupts the vasculature and lymphatic vessels that perforate between the tissue layers. Timely diagnosis and treatment are imperative, as a delayed diagnosis can lead to complications. However, at present there is no univ

Severe degloving injuries to the pediatric lower extremity are difficult to treat, traditionally requiring local or free flaps for coverage. Combining vacuum-assisted closure techniques with a dermal regeneration template is proposed as a means for covering these difficult wounds. We retrospectively reviewed the charts of 7 consecutive patients (age range, 2-12 years) who underwent this treatment. All extremities healed without flap reconstruction or amputation. Mean follow-up was 24.4 months, and mean wound size was 196 cm2. There were 2 superficial graft complications, 1 nonunion successfull

PURPOSE: Crush injuries result from the physical compression of muscles and may lead to crush syndrome. Early fluid resuscitation and surgical intervention is key. Few studies have reported the outcomes of crush injuries in the non-disaster setting. This retrospective study aims to characterise such cases.
METHODS: Patients with lower limb crush injuries were identified from an internal database. Non-crush injuries and patients under the age of 18 were excluded. Types of injuries, management, and complications were extracted.
RESULTS: 27 patients were included. The right leg (n = 10) was the

We report the first pediatric case of penile fracture with complete urethral transection caused by accidental fall. The patient presented atypically with the majority of swelling and ecchymosis occurring in the scrotal area rather than the penile shaft. The urethral injury occurred at the junction of the penile and bulbar urethra with significant urinary extravasation into the scrotum. The injury was repaired with plastic reanastomosis of the corpora cavernosa and urethra using a scrotal approach, rather than the commonly described degloving incision. The diagnosis and management of this unusu

The Morel-Lavallée lesion (MLL) is a closed degloving injury caused by traumatic separation of the subcutaneous tissue from the underlying fascia, without a break in the overlying skin. We present two cases that demonstrate a previously unrecognised association of the MLL with thoracolumbar spine fractures. The lesion is frequently missed, or its significance is overlooked, on initial evaluation. Awareness of this injury should allow tailored strategies to decrease the high risk of wound complications.

Synthetic electrospun fiber materials (SEFMs) may offer a versatile tool for soft-tissue management in orthopedic trauma. Composed of bioresorbable synthetic polymers engineered to resemble extracellular matrix, SEFM supports cellular infiltration, revascularization, and mechanical reinforcement of primary closures. Preclinical studies demonstrate improved wound healing, tensile strength comparable to human skin, and in vitro passing results per United States Pharmacopeia <51> Antimicrobial Effectiveness standards. Clinical evidence demonstrates both topical and subdermal use in open fractures

Circumferential degloving injuries of the digits are common but continue to be a challenge to achieve stable coverage. Many techniques have been used to tackle these, ranging from skin grafts to multiple free flaps. This article describes a straightforward technique of using 2 pedicled flaps from the abdominal region to simultaneously cover both the volar and dorsal aspects of the digit. The dimensions, design, and method of using the technique are explained in detail with illustrative case examples. This "sandwich" flap design is simple, reliable, does not require a delay stage and minimises

The Morel-Lavallée lesion is a collection of serous fluid that develops after closed degloving injuries and after surgical procedures particularly in the pelvis and abdomen. It is a persistent seroma and is usually resistant to conservative methods of treatment such as percutaneous drainage and compression. Various methods of curative treatment have been reported in the literature, such as application of fibrin sealant, doxycycline, or alcohol sclerodhesis. We present a case of a huge recurrent Morel-Lavallée lesion in the lower back and buttock region that was treated with quilting sutures, f

Abdominal wall injuries occur in nearly one of 10 patients coming to the emergency department after nonpenetrating trauma. Injuries range from minor, such as abdominal wall contusion, to severe, such as abdominal wall rupture with evisceration of abdominal contents. Examples of specific injuries that can be detected at cross-sectional imaging include abdominal muscle strain, tear, or hematoma, including rectus sheath hematoma (RSH); traumatic abdominal wall hernia (TAWH); and Morel-Lavallée lesion (MLL) (closed degloving injury). These injuries are often overlooked clinically because of (a) a

Morel-Lavallee lesion is a closed degloving soft tissue injury in which subcutaneous tissue is torn from the underlying muscular fascia. The tear leads to venolymphatic leak, and concomitant adipose tissue necrosis from the force of the trauma causes swelling and possible infection at the site of injury. The traditional treatment for the lesion is surgical drainage and debridement. In this report, an endoscopic method is described, which achieves the goal of an open surgical debridement but minimizes surgical morbidity.

INTRODUCTION: Penile strangulation is a rare urological emergency that necessitates urgent management. The reasoning behind it may include sexual pleasure, erection reinforcement, or a psychiatric disorder.
CASE PRESENTATION: Despite being an uncommon complication of penile strangulation, we report a 45-year-old prison inmate who presented with penile shaft necrosis secondary to using a non-metallic constriction object. The patient reported a 5-day history of progressive penile pain, edema, and skin injury but no urinary symptoms. The patient underwent complete penile skin degloving, circumci
