A network-based response to the two-factor theory of delusion formation.

In response to Dr. Corlett's paper regarding the two factor theory of delusional misidentifications, I discuss further evidence that VMPFC damaged patients do not have an isolated factor 1 defect. I then discuss more broadly the limitations in the modular view of brain function that leads to the 2-factor theory. Finally, I propose a connectionist based interpretation of delusional misidentifications that better fits with the clinical data from patients with focal brain lesions showing how these lesion locations relate to complex brain networks.

Dental Implants: A Remote Option in Case of Somatic Delusion Disorder.

A 29-year-old female was referred by a psychiatrist for dental implantation and prosthodontic rehabilitation, as she had all her healthy permanent teeth extracted within a span of 2-6 months, due to somatic delusional disorder. She presently refuses artificial dentures and implants with the fear of having worms in her teeth and the fear not yet allayed. The patient cannot be treated for her edentulous state until her psychiatric symptoms are treated. This is the limitation for the implant surgeon where service rendered is impaired in spite of advanced professional skills, ideal patient ridge,

Esquirol's change of view towards Pinel's mania without delusion.

We recount how Jean-Étienne Dominique Esquirol (1772-1840) gradually changed his position towards what Philipe Pinel (1745-1826) referred to as mania without delusion. Between 1805 and 1838, Esquirol moved from outright rejection, questioning the very idea of insane persons committing motiveless acts of violence without delusion, to relative acceptance. He eventually incorporated the clinical characteristics of mania without delusion in his description of homicidal monomania, dividing them between reasoning monomania and instinctive monomania. We examine this change by detailing each of Esquir

Agency and Anxiety: Delusions of Control and Loss of Control in Schizophrenia and Agoraphobia.

We review the distinction between sense of agency and sense of ownership, and then explore these concepts, and their reflective attributions, in schizophrenic symptoms and agoraphobia. We show how the underlying dynamics of these experiences are different across these disorders. We argue that these concepts are complex and cannot be reduced to neural mechanisms, but involve embodied and situated processes that include the physical and social environments. We conclude by arguing that the subjective and intersubjective dimensions of agency and ownership cannot be considered in isolation from one

"Cat-gras" delusion: a unique misidentification syndrome and a novel explanation.

ABSRACT Capgras syndrome is a distressing delusion found in a variety of neurological and psychiatric diseases where a patient believes that a family member, friend, or loved one has been replaced by an imposter. Patients recognize the physical resemblance of a familiar acquaintance but feel that the identity of that person is no longer the same. Here we describe a 73-year-old male with right posterior frontal and bilateral anterior-medial frontal damage from prior brain trauma with a similar delusion of an imposter replacing his pet cat. Misidentification syndromes for animals, as opposed to

Grandiose delusions: a review and theoretical integration of cognitive and affective perspectives.

Grandiose delusions (GDs) are found across a wide range of psychiatric conditions, including in around two-thirds of patients diagnosed with bipolar disorder, half of patients diagnosed with schizophrenia, as well as in a substantial proportion of patients with substance abuse disorders. In addition, over 10% of the healthy general population experience grandiose thoughts that do not meet full delusional criteria. Yet in contrast to other psychotic phenomena, such as auditory hallucinations and persecutory delusions, GDs have received little attention from researchers. This paper offers a comp

Mirror agnosia and the mirrored-self misidentification delusion: a hypnotic analogue.

INTRODUCTION: Mirrored-self misidentification is the delusional belief that one's reflection in the mirror is a stranger. Current theories suggest that one pathway to the delusion is mirror agnosia (a deficit in which patients are unable to use mirror knowledge when interacting with mirrors). This study examined whether a hypnotic suggestion for mirror agnosia can recreate features of the delusion. METHOD: Ten high hypnotisable participants were given either a suggestion to not understand mirrors or to see the mirror as a window. Participants were asked to look into a mirror and describe what

Attentional modulation of external speech attribution in patients with hallucinations and delusions.

A range of psychological theories have been proposed to account for the experience of auditory hallucinations and delusions in schizophrenic patients. Most influential theories are those implicating the defective self-monitoring of inner speech. Some recent studies measured response bias independently of self-monitoring and found the results inconsistent with the defective self-monitoring model, but explained by an externalizing response bias. We aimed to investigate the role of attentional bias in external misattribution of source by modulating participant's endogenous expectancies. Compariso

The case of a 48 year-old woman with bizarre and complex delusions.

BACKGROUND: A 48 year-old woman presented with an 18 month history of bizarre and complex delusions on a background of social, behavioral and cognitive decline over several years. Her psychosis progressed despite receiving high doses of antipsychotics. The patient's father also had a psychotic episode in his 40s. He subsequently developed motor neuron disease, which caused his death at 68 years of age. INVESTIGATIONS: Physical examination, neuropsychological testing, nerve conduction studies, brain MRI and transcranial magnetic stimulation. DIAGNOSIS: On the basis of the patient's age at ons

[Social phobia with interpersonal attitude delusions on the background of endogenous diseases].

The paper presents data on 38 patients with slow-progredient schizophrenia and cyclothymia with clinical picture of social phobias combined with sensitive ideas of reference. Two types of such states were singled out: social phobia of self-insufficiency with isolated sensitive ideas of reference (13 patients) and social phobia with the ideas of physical or mental defect and generalized ideas of reference (25 cases). The states of the first type were observed in the structure of circular melancholic depressions and were characterised by the presence of the ideas of low self-esteem. Phobic avoid

Major depressive disorder with delusion of having AIDS: a case report.

Acquired immune deficiency syndrome (AIDS) has become a topic of increasing public concern. Several conditions have been described in patients who are afraid that they have acquired AIDS, including depression, somatization disorders, hypochondriasis, adjustment disorders and various psychoses. This paper presents a case study of a young man with persistent fear of having AIDS. In spite of negative HIV test results, the patient still insisted that he suffered from AIDS. The psychiatric diagnosis was major depressive disorder with delusion of having AIDS. The patient was treated with fluoxetine

Delusions of infestation during delirium in a patient with restless legs syndrome.

A case is described in which delusions of infestation developed during delirium secondary to pneumonia in a patient with restless legs syndrome (RLS). This case demonstrates how tactile sensations may precede and give rise to delusional misinterpretations. Since RLS is relatively common in elderly people and may be due to a treatable cause such as iron deficiency, it is worth considering this syndrome in patients with delusions of infestation.

Delusions and hallucinations of cocaine abusers and paranoid schizophrenics: a comparative study.

We compared delusions and hallucinations of 100 cocaine abusers and 100 paranoid schizophrenic subjects admitted to an East Texas state psychiatric hospital. Subjects in both groups feared that individuals or organized groups might harm them in some way, but delusions of the paranoid schizophrenic subjects were more often bizarre than those of the cocaine abuse subjects. "Cocaine bugs" (parasitosis) were more often found in the cocaine abuse subjects. Command hallucinations were found in both groups, but the commands of the schizophrenic group more often related to harming or killing others. C

Acquired immune deficiency syndrome as a novel content in delusion symptomatology.

The case histories of three female patients are presented (two of them diagnosed as paranoid schizophrenia, one as endogenous depression in the course of involution), who manifested the delusion of AIDS disease in the psychopathological image of their primary illness. This novel content of delusional symptomatology appears in the thinking of psychiatric patients as a distorted reflexion of a new objective reality of the world, the AIDS disease, becoming recently one of the global problems of the human population.

[Apropos of delusions of grandeur in general paresis: considerations in 1 case].

Nowadays diagnosis of neurosyphilis has become an infrequent event. Furthermore, the few patients studied have a clinical picture different from that described in the past. We report the clinical evolution of a patient with general paresis who suffered from delusions of grandeur, a clinical feature almost disappeared in these times. This mental disorder did not change after prolonged phenothyazines treatment. Among possible causes that made delusions of grandeur disappear in general paresis, remarkable improvements in therapy and major changes in social life should be considered.

Social tinkering generates complexity, but also system-scale fragility and collective delusions.

The hypothesis that human culture emerges through deliberate, decentralized social tinkering rather than random variation is persuasive. However, the hypothesis is ambiguous about the stability of the resulting structures. Expanding scale may increase fragility and normative conflict, and networks of socially constructed coordination spaces may amplify imaginative disconnection from reality. Consequently, system-scale risks arise that exceed the grasp of governable tinkering.

The Difficulties of Grandiose Delusions: Harms, Challenges, and Implications for Treatment Engagement.

BACKGROUND AND HYPOTHESIS: Grandiose delusions may entail difficult responsibilities and detrimental actions for patients. Recognition of these consequences by patients may provide an avenue for engagement in treatment. Furthermore, when patients carry out actions within the delusional system ("immersion behaviors") or spend considerable time thinking about their grandiose beliefs this may contribute to the persistence of the grandiosity and further harmful consequences. We, therefore, investigated grandiose-related subjective harm, immersion behaviors, and perseverative thinking. STUDY DESIG

Prevalence, clinical features, and risk factors of delusions in patients with delirium.

OBJECTIVES: Delirium may be divided into multiple subtypes with different pathological factors. This study aimed to focus on the delirium subtype in which delusions are conspicuous and explore its prevalence, clinical characteristics, and risk factors. METHODS: The subjects were 601 delirium cases referred to the department of psychiatry over 5 years at a general hospital. The Delirium Rating Scale-Revised-98 was used to assess the delusions in patients with delirium, and the features of delusions (delusional form, object, and content) were examined. Multiple regression analysis was applied t

Psychological Impact of Parkinson Disease Delusions on Spouse Caregivers: A Qualitative Study.

There is growing research on carers of people with Parkinson's disease (PD) experiences. However, the impact on carers by PD delusions is not specifically examined. We conducted a qualitative study using semi-structured interviews of spouse carers of PD patients with delusions. Thematic analysis was employed using MAXQDA 2018. Twelve spouse participants (SPs) were interviewed. Four themes emerged: Managing incredulity: trying to make sense of delusion content; Hypervigilance: constant alertness to bizarre and threatening discourse and behavior; Defensive strategizing: anticipating delusions an
