Clinical considerations of microleakage.

Most dental materials permit the microleakage of bacteria and bacterial products from oral fluids to reach dentin. Thus, an understanding of the clinical consequences of microleakage demands that we analyze the permeability characteristics of dentin. The more dentin surface that is exposed during tooth preparation, the greater the potential for microleakage. Thicker dentin is less permeable than thin dentin. Dentin over pulp horns is more permeable than central dentin. Similarly, the dentin making up axial walls is more permeable than dentin forming the pulpal floor of cavities. Coronal dentin

[Causes and treatment of hypersensitivity in tooth root].

The review article describes aetiology and treatment of dental root hypersensitivity. The three main theories of pain transduction from the exposed dentin surface to pain receptors in dental pulp are described. In addition, therapeutic compounds used to stimulate reparatory dentin formation and mineralization of peritubular dentin are described as well. The application of resins and adhesives on exposed dentin root surfaces is advocated in cases of chronic hypersensitivity. Finally, the authors emphasize the need for correct oral hygiene and early orthodontic treatment, if indicated, to preven

Intraoral bacterial growth on tetracycline-impregnated dentin.

Previous studies have shown that tetracyclines react with enamel and dentin in vitro to form slightly soluble compounds with a pronounced and long-lasting antimicrobial capacity. The purpose of the present project was to study the effect of doxycycline-impregnation of dentin on the growth of oral microorganisms in vivo. Slabs of native human dentin were immersed in aqueous solutions of doxycycline HCl, 10 mg/ml, for 10 min, and were subsequently ligated to orthodontic brackets placed bucally on maxillary molars in seven individuals. Untreated native dentin specimens were similarly placed on co

Scientific Interest and Publication Patterns in Odontoblast-Neuron Crosstalk Research Related to Dentinal Hypersensitivity: A Bibliometric and Altmetric Review.

Dentin hypersensitivity is a common oral condition characterized by sharp pain triggered by exposed dentin in response to external stimuli, reflecting complex interactions between dentin structure, fluid dynamics, and neural responses, although its underlying biological mechanisms remain incompletely understood. Among the proposed mechanisms of dentinal nociception, the odontoblast transducer theory suggests that odontoblasts act as sensory cells capable of detecting stimuli and activating adjacent pulpal nerve fibers. This study aimed to characterize the scientific landscape of the odontoblas

Aspects of dentinal and pulpal pain. Pain of dentinal and pulpal origin--a review for the clinician.

Recent advances in understanding the mechanisms of pain arising from the dental pulp serve to benefit patients by improving the clinician's ability to diagnose and treat pain. There are two types of pain arising from the pulp which are mediated by entirely different nerve fibres, each with their own individual characteristics. One is a short, sharp fast pain which is induced by stimuli which cause a rapid fluid flow within the dentinal tubules. Such stimuli include cold, heat, air, drilling, and osmotic stimuli. Once the affected teeth are identified, they can often be treated by sealing the o

The influence of different radiotherapy doses on the mechanical properties and microstructure of the enamel and dentin of human premolar teeth.

OBJECTIVE: Radiation therapy is applied in the treatment of head and neck cancer patients. However, oral-health-related side effects like hyposalivation and a higher prevalence of caries have been shown. This study aims to assess the influence of different radiotherapy doses on the mechanical properties, roughness, superficial microstructure, and crystallinity of the enamel and dentin of human premolar teeth. METHODS: Specimens (n = 25) were categorized into five groups based on the radiation dose received (0, 10, 30, 50, and 70 Gy). The enamel and dentin of these specimens were subjected to

Effect of chlorhexidine pretreatment on clinical performance of adhesive restorations: a systematic review and meta-analysis.

STATEMENT OF PROBLEM: The long-term stability of resin-dentin adhesion remains a major concern in restorative dentistry. Hydrolytic and enzymatic degradation of the hybrid layer contributes to the deterioration of resin-bonded restorations. Inhibition of matrix metalloproteinases (MMPs) by chlorhexidine digluconate (CHX) has been proposed to preserve the hybrid layer and improve adhesive durability. PURPOSE: This systematic review and meta-analysis evaluated the effect of dentin pretreatment with CHX on the clinical performance and longevity of resin-based restorations compared with no CHX ap

Dentin hypersensitivity. Occlusion of dentinal tubules by brushing with and without an abrasive dentifrice.

Daily toothbrushing with and without an abrasive dentifrice was performed on the ground surfaces of human cervical dentin attached to resin plates, which had been exposed to the oral cavities of 4 subjects for 8 weeks. Brushing with a dentifrice containing calcium hydrogen phosphate as an abrasive caused most of the dentinal tubules to open. The patency of dentinal tubules is thought to induce dentin hypersensitivity. Brushing without a dentifrice, on the other hand, resulted in occlusion of the dentinal tubules with organic pellicle-containing minerals. Furthermore, abrasion did not occur; ho

Dentine hypersensitivity. I. Effects produced by the uptake in vitro of metal ions, fluoride and formaldehyde onto dentine.

Dentine has been shown to possess affinity for a large number of varied compounds, many of which have been shown effective in clinical trials, for the treatment of dentine hypersensitivity. The mode of action of these compounds is unclear. The aim of this study was to investigate the uptake of metal ions, fluoride and formaldehyde in solution onto dentine in vitro and determine whether therapeutic effects could be mediated through occlusion of dentinal tubules. Etched dentine sections were exposed for 1 h to 1000 ppm solutions of fluoride and metal salts. Levels of fluoride and respective meta
