Pharmacological Prescription at the End of Life: Quality Assessment in the Transition of Care to a Community Palliative Care Support Team.

Appropriate pharmacological management is a cornerstone of quality in palliative care (PC), focusing on comfort and quality of life. Therapeutic review is crucial in PC, aiming to optimize symptom relief, reduce adverse effects, and manage drug interactions. This study aims to delve into the real-world pharmacological prescription practices within a Community Palliative Care Support Team (CPCST) in the northern region of Portugal, comparing practices at admission and at the last consultation before death. It is an observational, cross-sectional, retrospective study without intervention involvi

Association between proton pump inhibitors and severe hematological toxicity in patients receiving pemetrexed-based anticancer treatment: The prospective IPPEM study.

OBJECTIVE: Pemetrexed is associated with hematological toxicity. Drug-drug interactions (DDIs) between methotrexate and proton pump inhibitors (PPIs) induce a higher risk of hematological toxicity due to the inhibition of methotrexate excretion by PPIs. As pemetrexed and methotrexate are both excreted by human organic anion transporter 3 (hOAT3), this study investigates the hypothetical DDI between pemetrexed and PPIs in lung cancer patients. The primary objective was the occurrence of severe (grade ≥ 3) hematological toxicity. The secondary objectives were to describe the type of hematologica

An App-Based Proton Pump Inhibitor Discontinuation Tool: A Multicentre, Open-Label, Randomised Superiority Trial.

BACKGROUND: Proton pump inhibitors (PPIs) are frequently used without long-term indication. Discontinuation remains challenging because of rebound symptoms. Whether digital guidance tools improve discontinuation is unknown. We evaluated whether an app improves PPI discontinuation. METHODS: In this open-label adaptive randomised controlled superiority trial, adults using daily PPI for ≥ 4 weeks without chronic PPI indication, according to the Dutch College of General Practitioners guideline, were recruited from outpatient clinics of Internal Medicine, Gastroenterology, Rheumatology and Nephrol

Complex intervention programme to improve patient safety and facilitate deprescribing in frail older patients living at home (COFRAIL): A process evaluation of a cluster randomised controlled trial.

INTRODUCTION: Frailty is associated with negative health outcomes in geriatric patients. A large proportion of frail patients is affected by polypharmacy, which in turn may be a possible cause of frailty. The cluster randomised controlled COFRAIL trial investigated the effects of family conferences to improve the care of frail patients. Deprescribing and communication about prioritising health goals between patients, relatives and general practitioners were key components of the intervention. An accompanying process evaluation was conducted to investigate whether the study intervention was imp

Stakeholder Perspectives on the Use of Telehealth to Support Deprescribing for Older Patients Transitioning From the Hospital to Home Health Care.

BACKGROUND: Multimorbidity affects most older adults in home health care (HHC), leading to inappropriate polypharmacy and increased risks for adverse outcomes. While deprescribing can mitigate these risks, fragmented communication during hospital-to-home transitions remains a major barrier. This study explored stakeholder perspectives on using telehealth to facilitate deprescribing for older adults with multimorbidity while receiving post-acute HHC. METHODS: We conducted semi-structured interviews with 44 stakeholders across 12 U.S. states, including 14 HHC patients and 30 clinicians (physici

Antithrombotic therapy at the end-of-life-continue or stop?

Antithrombotics at the end-of-life pose a clinical challenge where indication-specific time-to-benefit, bleeding risk, and patient priorities must be reconciled over short prognostic horizons. In this review of the literature, we found that a substantial number of individuals remain on anticoagulants and antiplatelet therapy till the point of death. Prospective ultrasound surveillance studies show a high baseline incidence of asymptomatic proximal deep vein thrombosis at admission but a low short-term incidence of new events during typical hospice stays. Additionally, large home palliative coh

Budget Impact Analysis of the Balanced Opioid Initiative: A Cluster Randomized Trial Aimed at Deprescribing Opioids for Chronic Pain in Primary Care Settings.

PURPOSE: This study compared the budget impact of 4 deimplementation strategies designed to promote guideline-concordant opioid prescribing. METHODS: In this cluster randomized trial of 32 primary care clinics, deimplementation strategies targeted the system, clinic, and prescriber levels. All clinics received system-level educational meetings with audit/feedback reports. At month 3, one-half were randomized to add practice facilitation, a clinic-level strategy. At month 9, one-half were again randomized to add prescriber peer consulting, a prescriber-level strategy, resulting in 4 groups: sy

The effects of diuretic deprescribing in adult patients: A systematic review to inform an evidence-based diuretic deprescribing guideline.

In this systematic review, we report on the effects of diuretic deprescribing compared to continued diuretic use. We included clinical studies reporting on outcomes such as mortality, heart failure recurrence, tolerability and feasibility. We assessed risk of bias and certainty of the evidence using the GRADE framework. We included 25 publications from 22 primary studies (15 randomized controlled trials; 7 nonrandomized studies). The mean number of participants in the deprescribing groups was 35, and median/mean age 64 years. In patients with heart failure, there was no clear evidence that diu

Are pharmacists' knowledge and practice the key to promoting deprescribing of potentially inappropriate medication: a missing link between treatment and outcomes.

OBJECTIVES: Deprescribing is a novel strategy whereby medical professionals aim to optimize a patient's prescription program by removing redundant medications. Few studies have looked at the viewpoints of community pharmacists and other healthcare professionals on deprescribing in daily practice. This study's objectives included evaluating community pharmacists' deprescribing knowledge, attitudes and practices, as well as identifying the obstacles to and enablers of deprescribing in daily practice. METHODS: Five pharmacy students in the last year of their studies polled employees of neighbour

Cardiac Resynchronization Therapy in Inotrope-Dependent Heart Failure Patients: A Systematic Review and Meta-Analysis.

OBJECTIVES: The purpose of this study was to evaluate outcomes after cardiac resynchronization therapy (CRT) in inotrope-dependent patients with heart failure (HF) to ascertain the viability of CRT in these patients. BACKGROUND: During the last decade, significant numbers of trials have demonstrated the beneficial effect of CRT in the treatment of patients with HF and systolic dysfunction, prolonged QRS complex duration, and New York Heart Association functional class III or IV. However, it is currently undetermined whether CRT may benefit patients who require inotropic support. METHODS: The

Effects of deprescribing from inhaled corticosteroids in people with cystic fibrosis: protocol for a target trial emulation using the UK CF Registry.

INTRODUCTION: Observational data are increasingly used to study and draw causal inferences about the effects of treatments. Target trial emulation (TTE) is a framework for mitigating biases in causal investigations through specification of an observational study, targeting a specific causal research question, based on a real or hypothetical randomised controlled trial. Investigations into the effects of treatment discontinuation are of growing interest and particularly relevant in cystic fibrosis (CF), where treatment burden is high and new transformative therapies are becoming widespread. Dopaminergic Drugs and Valproate-associated Refractory Hyponatremia Over Levosulpiride-induced Parkinsonism in an Elderly Female Patient When Deprescribing was the Only Therapy.

INTRODUCTION: Prescribing cascade is a condition in which a drug administered to a patient causes an adverse reaction that is misinterpreted as a new condition, resulting in the addition of a new drug. CASE PRESENTATION: Here, we report the case of an elderly female patient who suffered from metabolic, neurologic, and urinary consequences of a prescribing cascade of antiemetic, antiepileptic, and dopaminergic drugs. While levosulpiride caused Parkinsonian symptoms, the dopaminergic drugs and valproate caused refractory hyponatremia followed by altered sensorium, and clidinium contributed to u

A comprehensive systematic review of the types of medication classes targeted by deprescribing tools and the tools/interventions applied to each class.

INTRODUCTION: Polypharmacy and potentially inappropriate medications (PIMs) contribute to adverse outcomes. Deprescribing, the supervised withdrawal of PIMs, is a key strategy to reduce these risks. Identifying the most targeted PIMs and commonly used tools for deprescribing remains essential. The aim of this systematic review was to identify the medication classes targeted by deprescribing tools, stratified by tool type and frequency and to evaluate the availability of medication-specific tools. METHODS: A systematic search of Embase, PubMed, Scopus, and Medline (2010-2023) identified observ

High Anticholinergic Burden of Patients Hospitalized for Hip Fracture Reveals a Target for Deprescribing.

OBJECTIVES: Anticholinergic medications can produce harmful side effects, such as ataxia and delirium, in older adults. Older adults with hip fractures are particularly vulnerable, yet they are often prescribed these medications. This study aimed to evaluate the anticholinergic burden (ACB) before and after hospitalization for hip fracture, to identify potential targets for deprescribing. DESIGN: This is an observational retrospective cohort study. Patients with hip fractures between 2012 and 2018 were identified from Medicare Provider Analysis data. Medications were identified from Medicare

Deprescribing benzodiazepine receptor agonists in older adults: a mixed-methods study to adapt the Canadian D-PRESCRIBE intervention to the Belgian community setting.

OBJECTIVE: Guidelines recommend deprescribing benzodiazepine receptor agonists (BZRA) in older adults, yet implementation in clinical practice remains limited. Adapting effective, evidence-based interventions to a new context is a resource-saving strategy. In Canada, the D-PRESCRIBE intervention comprised a patient educational brochure and a pharmaceutical opinion inviting physicians to revise BZRA prescribing and consider safer alternatives. Due to its effectiveness on BZRA deprescribing among Canadian older adults, we aimed to adapt the D-PRESCRIBE intervention to the Belgian community setti

Defining key deprescribing measures from electronic health data: A multisite data harmonization project.

BACKGROUND: Stopping or reducing risky or unneeded medications ("deprescribing") could improve older adults' health. Electronic health data can support observational and intervention studies of deprescribing, but there are no standardized measures for key variables, and healthcare systems have differing data types and availability. We developed definitions for chronic medication use and discontinuation based on electronic health data and applied them in a case study of benzodiazepines and Z-drugs in five diverse US healthcare systems. METHODS: We conducted a retrospective cohort study of adul

Effect of antihypertensive deprescribing on hospitalisation and mortality: long-term follow-up of the OPTiMISE randomised controlled trial.

BACKGROUND: Deprescribing of antihypertensive medications is recommended for some older patients with low blood pressure and frailty. The OPTiMISE trial showed that this deprescribing can be achieved with no differences in blood pressure control at 3 months compared with usual care. We aimed to examine effects of deprescribing on longer-term hospitalisation and mortality. METHODS: This randomised controlled trial enrolled participants from 69 general practices across central and southern England. Participants aged 80 years or older, with systolic blood pressure less than 150 mm Hg and who wer
