BACKGROUND: Staphylococcus aureus bacteremia in patients with cardiac implantable electronic devices (CIEDs) is often associated with infective endocarditis (CIED-IE). The CIED-IE diagnosis is syndromic. Diagnostic uncertainty is common. Frequently, these patients are classified as having possible CIED-IE, resulting in guideline-noncompliant treatment and heterogeneous outcomes. Improved outcomes require accurate diagnoses. In these patients, we evaluated whether metagenomic sequencing of microbial cell-free DNA (mcfDNA) in serial plasma specimens could improve diagnostic precision.

Diaphyseal fractures of the tibia is a common injury with an incidence of 16.9/100,000 population. Surgical treatment with intramedullary interlocking nail is the treatment of choice in closed diaphyseal tibial injuries. Removal of the nail is required in non-union, infected implant, implant failure or during exchange nailing procedure. Implant removal surgery presents some unique challenges to the Orthopaedic surgeons like broken nails, broken screws and failure of the extraction devices to engage with the threads of the nail due to stripping of the threads in the proximal part of the nail. BACKGROUND: Transvenous lead extraction (TLE) is rising in parallel to cardiac implantable electronic device implantations. Persistent left side superior vena cava (PLSVC) is a relatively common anatomical variant in the healthy population; TLE in patients with a PLSVC is rare.
METHOD: Data were collated from 6 European TLE institutes of 10 patients who had undergone lead extraction with a PLSVC. Patient demographics, procedural challenges and outcomes were reported.
RESULTS: Ten patients aged 73.4 ± 7.8 years (60% male) underwent TLE of 20 leads (3 left ventricle, 10 right ventricle, 7 righ

BACKGROUND: Soil sample pretreatment is a critical step in soil analysis, often involving complex processes and the use of organic solvents, which can lead to environmental pollution and inefficiency. In this regard, traditional methods are time-consuming and prone to human error, rendering them unsuitable for large-scale soil surveys. To address these challenges, this study proposes a novel rapid soil pretreatment method employing a device that integrates ultrasonic extraction with pure water and capillary electrophoresis using capacitively coupled contactless conductivity detection (CE-C4D).

We report the first known case of Neisseria sicca-associated pacemaker lead endocarditis-a disease whose incidence and mortality are growing. A woman in her 70s with a history of transcatheter aortic valve replacement and pacemaker placement 7 months earlier presented with recurrent fevers. She visited the emergency department several times during the past 2 months for these fevers, and she had been given oral antibiotics for presumed urinary tract infections. Investigations revealed blood cultures growing N. sicca Although transthoracic echocardiogram was negative, transesophageal echocardiog

INTRODUCTION: Cardiac implantable electronic device (CIED) complications present significant challenges in clinical practice, especially in elderly patients with multiple comorbidities. Common adverse events include infection, lead malfunction, and device migration. Twiddler's Syndrome, a rare but serious CIED complication characterised by patient manipulation causing lead displacement and device malfunction, is often underreported. The literature consists mainly of case reports and small series, providing limited guidance on prevention and management. As CIEDs are critical for managing cardia

BACKGROUND: As a result of longer follow-up after implantation of cardioverter defibrillators (ICD), fatigue of the leads has become a concern. The aim of this study was to determine the incidence and clinical presentation of ICD lead failures.
METHODS AND RESULTS: The study population consisted of 241 patients with 249 ICD leads who underwent implantation of an ICD with a transvenous lead system. After device implantation, the patients were routinely followed up every 4 months. Five lead failures (2.0%) occurred as an oversensing of artifact during the follow-up period (2.6+/-2.1 years); 4 o

The purpose of the present study is to evaluate the iridial microcirculation by fluorescent iridial angiography after laser cataract extraction by using a new extraction device, i.e. "RAKOT", based on the Nd:YAG-laser with wave length of 1.44 mcm. 25 eyes (21 patients) aged 71 to 87 with mature senile cataract were examined. The method of laser cataract extraction was used in 14 cases. These eyes made up the main examination group. As for the controls (11 eyes), the manual technic of cataract extraction was used for them. The results in the main group show that the degree of the response of th

BACKGROUND: Active and passive fixation leads are both commonly used in transvenous cardiac implantable electronic device procedures. Although active fixation leads (AFLs) are often selected over passive fixation leads (PFLs) for more precise positioning and the possibility of a potentially simpler future lead extraction, current evidence remains inconclusive regarding the delayed, post-discharge complications of lead dislodgement and cardiac perforation. This systematic review and meta-analysis aimed to compare these complications between AFLs and PFLs.
METHODS: Observational studies enrolli

Transvenous lead extraction (TLE) for cardiac implantable electric device (CIED) infection is becoming increasingly common, but is believed to be particularly risky in elderly patients. This study aimed to clarify the safety and effectiveness of TLE in the elderly, evaluating the use of both non-laser and laser extraction tools. We retrospectively analyzed the characteristics, device type, indications, procedures, and clinical results in younger (YG; age: 15-79 years; n = 48) and elderly groups (EG; age: ≥ 80 years; n = 27) of patients who underwent percutaneous TLE between April 2014 and Dece

BACKGROUND AND AIMS: The need for transvenous lead extraction (TLE) is rising due to increased cardiovascular device implantation and an aging population. While the superior access is standard, complex cases may benefit from the Tandem approach, combining femoral and superior access to improve efficacy and safety. This study evaluates outcomes and predictors associated with the Tandem approach as a primary strategy.
METHODS: A retrospective analysis was conducted on 148 patients who underwent Tandem TLE at a high-volume UK center between September 2020 and December 2024. Data on procedural su

BACKGROUND: Many techniques for the endovascular retrieval of lost or misplaced foreign objects have been developed, and the removal of almost every foreign object has become possible. In this paper, we report our experience in retrieving foreign objects lost during cardiac device implantations or previous extraction procedures.
METHODS: This study was a retrospective analysis of the case records of all patients referred to our institution for transvenous retrieval of intravascular foreign objects.
RESULTS: Over 10 years, 45 consecutive patients underwent procedures for the retrieval of intr

BACKGROUND: Subcutaneous implantable cardioverter-defibrillators (S-ICDs) are increasingly used in patients without a pacing indication, and may reduce venous complications, endocarditis and extraction morbidity. Subcutaneous implantable cardioverter-defibrillator displacements may be less obvious than their transvenous counterparts.

Pacemakers are cardiac implantable electronic devices (CIEDs) that deliver electrical impulses to regulate heart rhythm and manage conditions such as cardiac arrhythmias. While transvenous pacemakers (TP) have a generator with pacing leads, leadless pacemakers (LPs) are anchored within the right ventricular endocardium via tined or helical fixation. Device extractions may occur due to infection, dysfunction, or device upgrade. This study aims to analyze the incidence and severity of extraction complications between TPs and LPs. A comprehensive narrative review was performed through the PubM

BACKGROUND: Superior vena cava (SVC) injury during pacemaker implantation is rare, with most cases reported during lead extraction rather than placement. In adults with congenital heart disease (ACHD), unexpected resistance during lead advancement requires immediate reassessment because technical or device-related factors can cause venous injury even when pre-procedural imaging does not show hostile venous anatomy. Evidence on the management of venous injury during implantation is limited.

Lead wire malposition is thought to be a rare complication of both permanent and temporary pacemaker implantation. The actual incidence and prevalence are unknown because of lack of reporting, which complicates consistency in treatment. Potential safeguards to prevent complications as a result of lead malposition are readily available, effective, and inexpensive, but underused. An 80-year-old white man presented to our institution with right-arm paresthesias and weakness, as well as facial numbness, 4 months after undergoing single-chamber pacemaker placement by an outside hospital because of

INTRODUCTION: As the number of primary total hip arthroplasty (THA) procedures continues to increase, so too does the demand for revision surgery, with a 43-70% rise in revision THA anticipated by 2030. Femoral stem extraction in revision THA is particularly challenging and may lead to complications like femoral bone loss or fractures. However, increasing catalogue of femoral stems available for primary and revision THA has led technological advances in extraction devices to potentially overcome these limitations. This review aims to discuss the identification of femoral implants and the vario
